Goulburn Valley Health ( GV Health ) is the Regional Public Health Service for the West Hume Sub Region of Victoria, with main campuses located at Shepparton, Tatura and Rushworth and additional sites in Shepparton, Seymour, Benalla, Cobram, Echuca and Wodonga. GV Health provides a range of acute, mental health, aged and primary health and community support services across the West Hume Sub Region. ROLE STATEMENT: This position is critical in the initial engagement and development of successful relationships with clients to ensure they have access to appropriate treatment and advice. Triage is the first important step in the recovery process. The Triage Clinician is key to providing high quality, evidence based screening, assessment and treatment to assist clients to address their mental health concerns and contribute to the prevention and reduction of mental health symptoms. This program is essential to the Mental Health Program and the services it offers patients, carers, services and the wider community. The Triage Service is a telephone based triage service and is one of two entry points into the Mental Health Program- the other being the Emergency Departments. As such it is crucial to the referral process across all sites where a Mental Health Assessment is required. It includes all referrals into the Adult, Child and Adolescent (CAMHS) and Aged Mental Health Service. This position is integral to ensuring that the program runs in an efficient manner. The Team Leader Enhanced Triage is responsible for the effective coordination of triage for all patients seeking services from Goulburn Valley Health. Strong liaison and communication skills and the ability to manage internal and external stakeholder relationships providing intake, assessment and continuity of care are key priorities of this role. Under the guidance of the Divisional Operational Director and in conjunction with other team members provides the highest quality direct mental health clinical care to people contacting Goulburn Valley Health. The incumbent is required to manage and support the clinicians in demonstrating capability to assess the level of urgency in situations including community, ED and on telephone triage for psychiatric crises and undertake a course of action to resolve or ameliorate the crises accordingly. Have minimum of three years community mental health experience, or other relevant experience in a similar role. Demonstrated experience working in Acute Mental Health setting conduction initial mental health assessments. Preferably experience working on intake/triage phone lines. Page 2 of 7

Demonstrated ability to implement a range of clinical interventions in mental health in line with best practice guidelines. Demonstrated commitment to service development and quality improvement principles. Highly developed customer service, problem solving and interpersonal skills in order to be able to work with all staff, stakeholders and members in a team based environment. Lead the development and implementation of the Annual Operation Plan and Annual Quality Plans to ensure risks are managed and improvement objectives are met Lead the development and provision of the monthly report for the Divisional Operational Director against key performance indicators for financial, human resources, quality and risk objectives Lead/develop and implement governance projects/quality committees and activities as required Provide a high quality service to internal customers and consumers that reflects best practice and adds value to GV Health 1. Quality and safe clinical care for consumers Support clinicians to work in collaboration with referring agencies, consumers, families/representatives or carers by providing timely information and response in accordance with the State-wide Psychiatric Triage Scale whilst involving them in care planning and treatment decision making Ensure safe systems are in place for Consultation and Liaison services to GV Health are in place and outcomes reviewed. Operationally manage the clinical practices and ensure standards of care maximise health outcomes by continually monitoring, evaluating and improving practices Ensure safe service provision is provided by maintaining appropriate staffing and skill mix levels Uphold and protect consumer rights, maintain strict confidentiality and continually practice to the principles of open disclosure Ensure compliance with best practice healthcare standards, legislation, and GV Health s Clinical Governance Framework and clinical practice guidelines Maintain accurate and current clinical records ensuring documentation meets professional and legal standards Measure consumer experience and respond accordingly to feedback and complaints including reporting findings to appropriate management and committees Lead and provide robust clinical governance including review and supervision systems to support clinical decision making. 2. KEY PERFORMANCE INDICATORS: Directly responsible to the Divisional Operational Director Meet necessary KPIs mandated by DHHS for Mental Health Care Service provision to be provided within the parameters of Mental Health Act 2014 Provide management and oversight within the GV Health CREATE values Applies sound clinical knowledge and an in-depth understanding of contemporary mental health nursing practice to ensure quality clinical outcomes are achieved. Oversee the provision of high quality engagement, assessment, treatment and support services to individuals and their families using best practice principles. Oversee that Triage provides a centralised intake function for all initial contact from clients with our service. Ensure staff receive and manage enquiries and referrals from people with mental health issues, family members, GP s and other providers. Implementation of receipt of enquiries via numerous avenues including phone, email, website, social media and face-to-face contact. Screen and prioritise requests for services including: Initial pre-assessment of urgency by prioritizing those that present immediate risk to individuals, children and other family members; Screening for additional needs and referral where necessary; Immediate referral to a Acute Response Clinician for those clients identified as complex or the need for extra support to engage in treatment; Provide outward referrals to day programs. Oversee the booking of assessments within one day or same day assessment for high risk contacts. Be responsible for staff navigating calendars of clinical staff throughout the clinic to make real-time assessment appointments. Oversee that the provision of timely, accurate advice and information to clients, families or referring parties is undertaken Manage and educate staff to provide brief opportunistic interventions to clients and/or family members. Where appropriate, contribute to Clinical Review as required. Be responsible that the department is within budget, performance and development reviews are completed on time, training and core competency requirements are met as per the GV Health Education Framework and leave balances are managed in accordance with the policy and procedure to minimise liability Quality, safety and risk plans and activities are implemented in accordance with the relevant frameworks and procedures 80% attendance and active participation at committees, working groups and meetings Rostering is completed in accordance with the relevant Enterprise Agreement requirements Registration is maintained and working within scope of practice KEY SELECTION CRITERIA: (Please respond to this section in your application) Mandatory Current registration with the Australian Health Practitioner Regulation Agency as a Registered Nurse with a minimum of 5 years clinical experience post obtainment of relevant qualification. Demonstrated ability to provide clinical consultation and education to other service providers. Well-developed interpersonal and communication skills (written and verbal) and proven ability to liaise, negotiate and communicate with staff and other service providers. Evidence of a sound knowledge of the development and delivery of consultation and liaison psychiatry in a general hospital setting Well-developed knowledge and understanding of the Mental Health Act 2014 (Vic) and the National Mental Health Standards, National Safety and Quality Health Service Standards and relevant legislation. Demonstrated ability in the planning, implementation and evaluation of psychiatric service provision. Knowledge and sound understanding of mental health triage and ability to manage the triage within a Centralised Triage Model inclusive of all age groups. The incumbent must have a demonstrated understanding of the State-wide Psychiatric Triage Scale and KPI s associated with the Triage function. Satisfactory Police & Working with Children s Check prior to commencement of employment. Desirable: Evidence of a relevant post graduate tertiary qualification or working towards same.
