CareTracker Eliminates Paper Documentation: Improved Accuracy Copying, missed entries, and misinformation are common issues with paper forms. Even when the paper forms are accurate, retrieving the information is difficult. CareTracker makes it easy to record accurate and complete information to provide a much more precise picture of each resident. Increased Revenue Improved Facility and Family Communication Communication between front-line caregivers and other facility staff is critical for increasing accuracy and reliability of information. CareTracker helps staff obtain real-time information regarding facility functions and changes in resident conditions right from the kiosks in the hallways. In addition, CareTracker helps keep family members up-to-date on the condition of their loved ones, easing the burden on certain key employees. It s difficult to get paid for the right level of care when documentation is inaccurate and incomplete. CareTracker will help you get full credit for the true level of care you provide by improving the documentation process. CareTracker helps employees record more accurate data, and that provides your organization with an improved awareness of each resident s acuity level. Reduced Risk As residents age, it is common for them to experience a decline in their ability to get around. Unfortunately, the changes can be slight, and the staff may not notice the downward trend until the resident loses strength and falls. While documentation may have been recorded on paper, few facilities have the time to analyze that data for trends to prevent health concerns from becoming bigger liability issues. Nearly eight out of ten liability cases involve an intense examination of supportive documentation. How well does your documentation withstand this scrutiny? Improved Quality of Care By ensuring documentation is complete and accurate, CareTracker helps organizations identify risk points and changes in resident conditions to proactively address and resolve issues before they become bigger problems. CareTracker constantly monitors resident conditions, and it alerts staff when problems arise. At any time, staff members can review a dashboard of all of the alerts so that they know which residents have issues that need addressed first.

CareTracker s simple visual icons are designed to help minimize reading and learning time. Even new staff will be documenting accurately and completely in minutes. Wall-mounted touch screens located throughout your facility make it easy for caregivers to document resident observations and incidents as they occur. This means more frequent entries, better quality data, and more accurate clinical information about your residents. Simple picture icons with yellow backgrounds indicate categories the caregivers must complete during the shift. Once completed, the yellow turns gray indicating that at least the minimum documentation necessary for the shift has been completed. Caregivers use biometric thumbprint scanners or passwords for secure access to the CareTracker system. This allows every observation to be stamped with the date and time, and digitally signed by the caregiver.

Increase Quality of Care Clinical Intelligence & Decision Support CareTracker s Clinical Intelligence Tool analyzes millions of data records collected by your staff and quickly spots potential issues before they become a problem. Alerts indicate residents who are consistently declining in ADL performance, having behavior, mood, or skin issues, and more. Detailed information about each alert is only a click away. See resident-specific information that helps support the care decisions you need to make. See details about which ADLs are declining, when skin issues are occurring and what types of behaviors are most prevalent. Flags for Decline in or more ADLs

Quickly & Accurately Monitor Levels of Care CareTracker allows you to monitor each resident s level of care from week to week to pinpoint when someone begins to require more assistance from employees. This makes it easy to know when to adjust their care plan. Each category can be customized and the values are calculated based on your organization s unique needs.
