CareTracker Point of Care ADLs Decision Support
CareTracker s simple visual icons are designed to help minimize reading and learning time. Even new staff will be documenting accurately and completely in minutes. Wall-mounted touch screens and portable ipads make it easy for caregivers to document resident observations and incidents as they occur. This means more frequent entries, better quality data, and more accurate clinical information about your residents. Simple picture icons with yellow backgrounds indicate categories the caregivers must complete during the shift. Once completed, the yellow turns gray indicating that at least the minimum documentation necessary for the shift has been completed. Caregivers use biometric thumbprint scanners or passwords for secure access to the CareTracker system. This allows every observation to be stamped with the date and time, and digitally signed by the caregiver.
CareTracker Eliminates Paper Documentation: Improving Accuracy Copying, missed entries, and misinformation are common issues with paper forms. Even when the paper forms are accurate, retrieving the information is difficult. CareTracker makes it easy to record accurate and complete information to provide a much more precise picture of the resident s true condition. Increasing Reimbursement It s difficult to get paid for the level of care you provide when documentation is inaccurate and incomplete. CareTracker will help you get paid for the true level of care you provide. More than 80% of all facilities increased Medicare and Medicaid reimbursement by more than $50,000 annually after implementing CareTracker. Improving Facility Communications Communication between front-line caregivers and other facility staff is critical for increasing accuracy and reliability of information. CareTracker helps staff obtain real-time information regarding facility functions and changes in resident conditions from the kiosks and mobile devices. Front-line caregivers can also alert other employees to changes in resident conditions that may require immediate attention. Resident-Centered Programs Facilities can create and monitor the progress of resident-specific plans for restorative programs, behaviors, toileting programs, turn and reposition programs, and anything unique to a specific resident. Reducing Risk Nearly eight out of ten nursing home liability cases, and virtually all surveys, involve an intense examination of supportive documentation. How well does your documentation withstand this scrutiny? CareTracker helps eliminate inconsistencies and incompliance making documentation complete and ready for examination at any time. Improving Quality of Care Dehydration, skin problems, poor intake, weight loss, behaviors, and other resident conditions are not always reported immediately. CareTracker can monitor these types of conditions and alert staff when problems arise. A full picture of every resident is available in the easy-to-read Resident Dashboard. CareTracker can trend, compare, and analyze collected data to proactively monitor for changes in resident conditions such as vitals, weights, falls, and depression.
