The Need for Change Historic levels of 6% investment are not sustainable The cost of care for a senior is three times higher than for the average person 5% of the population account for 66% of costs Challenges require leadership and cooperation 2

The Opportunity Health care is overflowing with opportunities for reform Providers and organizations in the system are ready for change and eager to contribute Technological advances Robust body of evidence 3

Our Vision To make Ontario the healthiest place in North America to grow up and grow old A system focused on wellness Faster access to family health care that serves as the hub of their health care system Better integration and accountability 4

Transforming the System Integration & Funding Reform Delivery Streams Wellness & Prevention Execution Initiatives Patient-based payment Labour agreements Health Links Primary Care Reform Mental Health & Addictions Strategy EMRs Community Clinics Scope of Practice Seniors Strategy Long-Term Care Homes & Community Use Healthy Kids Smoke-Free Ontario Public Health Strategy Principles Evidence Based Standardization Access Appropriateness Co-design Innovation Quality Productivity Competition Safety 5

What Transformation Will Mean for the System Greater satisfaction Full scope of practice making better use of HHR Timely care post discharge Fewer avoidable readmissions Transparency in decision making open communication on priorities Higher quality based on evidence Achieving improvements in quality, outcomes and value More time with patients less chasing information Integrated services providers working together differently Reduced time from referral to specialist Reduced inappropriate ED visits Greater accountability shared responsibilities Increased access to primary care 6

Making Community Health Links Happen
