A clinical pharmacy service is provided to all wards and a number of specialist areas to ensure safe, effective and economic use of these products. Essential Qualifications REQUIREMENTS Candidates must on the latest date for receipt of completed application forms: Be registered in the Register of Pharmaceutical Chemists for Ireland, or be entitled to be so registered; Have not less than three years satisfactory post registration hospital experience; Possess the requisite knowledge and ability (including a high standard of suitability) for the proper discharge of the duties of the office. Purpose of the Post MAIN ROLE, DUTIES AND RESPONSIBILITIES Development and delivery of strategies on the appropriate use of antimicrobials, ensuring the successful operation and implementation of the SARI objectives in the hospital setting. Objectives of the Post Promote best practice in antimicrobial usage at hospital ward level to ensure rational and cost effective antimicrobial prescribing. Principle Duties and Responsibilities 1. Lead on the implementation of hospital, regional and national antimicrobial stewardship initiatives/policies 2. Work closely with the Consultant Microbiologist and Infectious Disease Physician, in the development and implementation of evidence based hospital antimicrobial policies and the maintenance and review as appropriate of established policies. 3. Lead out on the audit, monitoring, analysis and reporting of antimicrobials usage within the hospital(s) and providing feedback on antimicrobial usage to the prescribers. Participate in regional and national antimicrobial
consumption surveillance and provide relevant data to the regional HCAI committee, Health Protection Surveillance Centre and Hospital Management as required. 4. Work jointly with the Microbiology Consultant & Infectious Disease Physician at ward level, providing good clinical pharmacy practice to ensure the best use of antimicrobial drugs according to hospital antimicrobial policies. 5. To advise the relevant pharmacy, medical and nursing staff on best practice in relation to antimicrobial prescribing so as to promote the safe, effective and cost efficient use of antimicrobials. 6. Deliver training & education to pharmacy, medical and nursing staff in relation to antimicrobial use, resistance and other specialized information relating to antimicrobials 7. Liase with primary care to ensure common approaches to antimicrobial stewardship by GPs, through the promotion of educational initiatives on community based antimicrobial prescribing polices. 8. Participate in multidisciplinary working groups/ committees as required (e.g. the antimicrobial stewardship committee/ HCAI committee/ infection control committee) As a Senior Pharmacist the following duties should be carried out where required to ensure the provision of a comprehensive pharmaceutical service: The supervision and supply of drugs, pharmaceuticals and other related items as may be required. To perform occasional duties in the dispensary if required on a rotational basis to ensure that the purchase, storage and supply of all items is operated on the most economical lines, consistent with quality at pharmacy, ward and department levels and with reference to legal requirements, transit, security and conditions of chemical and physical stability. The maintenance of records for purchasing, quality control, compounding and dispensing to the standards required. Participate in the provision, development and expansion of ward clinical pharmacy services. The provision of advice, as required, to medical and nursing staff on the proper and economic use of drugs and medicines including participation in schemes for the dissemination of information in relation to drugs and drug usage designed to deliver optimal care economically. To co-operate with clinical, scientific and nursing staff on the maintenance and development of new methods of treatment.
To ensure that the needs of patients and professional staff are being adequately met by the pharmaceutical service provided. This will involve providing help and advice to patients and staff on all aspects of therapy and collaborating with community pharmacists to ensure continuity of supply of medication where necessary. Participate in continuing education and in such activities consistent with the post. Attend such study days and continuing education courses as may be deemed necessary for the development of both the service and the individual To participate in the teaching and training (including in-service training) of pharmacy, medical, nursing and other staff as may be required and is appropriate to the post. To supervise and manage any pharmacist and other staff assigned to him / her. Where a Chief Pharmacist has been assigned responsibilities to co-operate with and assist him / her in the performance of his / her duties and responsibilities as required. To participate in the oncall pharmacist and weekend pharmacist rota. To perform such other duties appropriate to the office of Senior Pharmacist as may be assigned to him / her from time to time by the Chief Pharmacist. PARTICULARS OF OFFICE 1. The appointment is Permanent, Whole-time and Pensionable. 2. Annual Leave allowance is 26 days per annum. 3. The person appointed must not give less than one month s notice, in writing, of intention to resign. 4. Normal working hours will be 37 per week 5. You will be required to work the agreed roster/on call arrangements advised to you by your line manager. Your contracted hours of work are liable to change between the hours of 8.00am 8.00pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement.
