Those under the age of majority must have their ideas approved, endorsed and submitted by a responsible adult - whether a parent, guardian, community volunteer, school representative or administrative representative from the non-profit organization on behalf of which the application is being made. Tbaytel employees, dealers and contractors are not eligible to submit an application for funding, but are permitted to be involved in or connected to a project, whether personally or through family connections. This may include advising community members about eligibility or providing applicants with input on what to include in a submission. TBAYTEL SERVING TERRITORY The three (3) funding levels are open to applicants and projects of Northern Ontario located within Tbaytel s serving territory. The territory includes but is not limited to the following communities: Atikokan Fraser River Marathon Searchmont Batchwana Bay Geraldton Miraki Shabaqua Beardmore Hawk Junction Montreal River Sioux Lookout Bellevue Hemlo Morson Terrace Bay Caramat Hornepayne Nakina Thunder Bay Chapleau Hudson Nipigon Upsala Dryden Ignace Perrault Falls Wawa Dubreuilville Kenora Rainy River White River Ear Falls Longlac Raith Fort Frances Manitouwadge Red Lake PROJECT DURATION The Tbaytel for Good Community Fund will only fund projects with a planned activity timeline of a maximum of one (1) year in duration. These criteria are non-negotiable and we will not accept applications for projects that intend to last longer than one (1) year. Decisions regarding the awarding of funds are made by an independent Tbaytel for Good Community Fund Selection Committee comprised of Tbaytel employees or representatives and community stakeholders from across the region. Meetings are held to review submissions and determine which $500 submissions will receive funding, as well as which $1,500 and $5,000 submissions will advance to the public voting phase. The two (2) submissions that receive the highest number of public votes in each of the two (2) higher funding categories will receive project funding. All decisions made by the Selection Committee are final. By applying for funding, applicants and fund recipients explicitly agree to abide by these Funding Eligibility & Submission Guidelines as well as our. 3

Funding Eligibility Criteria & Submission Guidelines WINNERS REPORTING Our involvement doesn t end with awarding funding to successful Tbaytel for Good Community Fund applicants. Once a successful project has been completed we require a project report that allows us to assess if the participant has delivered the project outlined in the initial application. We will require copies of receipts or invoices, visual documentation that demonstrates the project in action, and for larger projects, community impact evaluations. Our team will work with funding recipients to help them through the process. PAYMENT OF FUNDING Funding is paid by cheque as soon as a successful project has been selected to receive an award and the lead contact has returned a completed funding agreement package to Tbaytel. We expect all funding to have been fully spent by the end of 12 months from payment. For more information about current campaigns and closing dates, please visit our website at: www.tbaytelforgood.net SUBMISSION GUIDELINES Follow these easy steps to submit your application for funding: 1 2 3 4 After reviewing these guidelines and the full Tbaytel for Good Community Fund, complete the short online application form at www.tbaytelforgood.net with all pertinent information, identifying your requested funding level.
