Newsletter For OUR D214 Older Adult Residents Fall 2011 Dear Gold Card Member, We welcome another school year with a lot of buzz and excitement. I hope that the fall season finds you in good health and in great spirit. This issue of your newsletter brings a smorgasbord of interesting choices. There are performances, concerts, plays, classes, trips and many more that you, your loved ones and friends can choose from. Now that summer is behind us, we ve cooked up many wonderful things to keep you busy as we welcome fall. Hopefully by this time, you have received your copy of our Community Education booklet. I am a believer that it s never too late to learn something new. For starters, we have added a variety of more than twenty new classes this semester. From language to self-enrichment to computer/technology, these new classes, along with hundreds of classes brought back by popular demand, provide opportunities for everyone. We now have health and fitness classes, leisure interests and computer classes during daytime hours. You might find one of the following weekday classes interesting: Yoga, Pilates, Crochet, Setting Up Your Wireless Network at Home, Switching from a PC to Mac, E-mail Help Series and Cross-stitching. If you re interested in Computer/Technology, take a look at your choices this fall: Editing Photographs with Picasa, Social Networking for Intermediate Computer Users, or Facebook, Twitter, Flickr and more for Beginners. Our partner, Northwest Community Hospital, has an interesting article on page 3 on Medicare s Annual Wellness Visit or AWV. Make sure you check it out! If travelling is left unchecked on your to-do list this year, we are unveiling our exciting 2011-2012 schedule of day trips, tours and cruises on page 4. Remember, our trips are designed with your comfort and convenience in mind. We offer a unique one-stop service. If you haven t travelled with us, I think it s time that you give us a try! To Your Health Medicare s Annual Wellness Visit: Your Personalized Prevention Plan The Center for Medicare and Medicaid Services (CMS) initiated a new program in January of this year aimed at prevention for the over age 65 population. This is not a typical office visit with your primary care physician because it does not involve a physical examination. Instead the visit is a series of health screening questions and counseling about conditions which can put the over age 65 population at risk. The visit results in a personalized plan for your future care. This new Medicare benefit is called the Annual Wellness Visit (AWV) and it is offered to individuals who have been enrolled in Medicare Part B for at least 12 months. Medicare coinsurance and Part B deductibles do not apply to the AWV so there is no out-of-pocket cost to the individual. During the first 12 months of enrollment in Medicare Part B, an individual is eligible for an Initial Preventive Physical Examination (IPPE) known as the Welcome to Medicare exam. An individual is eligible for an AWV if they have not received either an IPPE or an AWV within the past 12 months. The Annual Wellness Visit presents a unique opportunity for you and your physician to reduce your health risks and promote self-management & wellness. During the visit your medical and family history are both updated. Blood pressure, height, weight and body mass index are measured to assess current and future heath risks. The visit screens for depression, cognitive impairment, functional ability and concerns regarding your home safety. All of this information is used to create a list of risk factors and recommended interventions for conditions that have been identified. Medicare preventive screening guidelines and eligibility are reviewed during the visit. Advanced directive planning can also be discussed if you are comfortable with that conversation. As a result of the Annual Wellness Visit, you and your physician will develop a personalized prevention plan which will act as a guide for further evaluation, treatment and/or screening that you may be eligible for. You can maximize this unique opportunity to create your personalized prevention plan by taking the time to prepare for your visit. Begin by writing down a list of your concerns. Gather copies of your pertinent medical records, including immunization records and family health history, with as much detail as possible. Bring a list and contact information of any physicians you are currently receiving care from. Make a list of medications and supplements, including calcium and vitamins, with a schedule of how much and how often each is taken. Bring a copy of any written Advanced Care Planning documents so that they can be put into your medical record. Make the most of this Medicare benefit and help your doctor by being an involved healthcare partner. This article was written by Susan E. Community Education News Social Media Made Easy There s no escaping it! Using Facebook, Twitter, Flickr and more to communicate is now part of our culture. D214 Community Education through its Continuing Education program is offering two sessions for beginners. You will learn how to use the free applications on the Internet to communicate better with friends, family and colleagues, as well as better manage your digital photos and documents. Session 1 will focus on opening accounts and learning basic navigation in Facebook and Google (gmail). Session 2 will delve deeper into all the applications and see how they work together. The cost for two sessions is $41. The first session is scheduled on Tuesday, October 11, 6:30 pm - 8:30 pm at Buffalo Grove High School. CE Programs Benefit from Annual Appeal District 214 Community Education (CE) is looking to intensify its Annual Appeal campaign this fall to raise additional funds for the many programs it supports. These programs include the Adult English as a Second Language, Read to Learn, Family Literacy and Adult Basic Education, The Women s & Children s Center, Kids on Track, Intergenerational and Older Adults, and Cultural and Performing Arts. Every year, Community Education relies on state grants and fundraisers to be able to support and sustain these programs that have benefitted thousands of people and their families. With the present economic climate, some of the funding has been lost due to cutbacks. The Annual Appeal campaign is an integral component of our fundraising goals, Mike Field, Director of District 214 Community Education, said. It helps our programs that have become crucial in helping people fight poverty and illiteracy. Gold Card Club members have been staunch supporters of the campaign for many years and Community Education is hoping the tradition continues.
