I understand that I will receive two separate decision letters one from the university indicating my admission status into George Mason University and one from the College of Health and Human Services indicating my admission status into the nursing program. I understand that failure to disclose all the institutions I attended, currently attend, or plan to attend on the George Mason Undergraduate Application prior to the start of the nursing program will jeopardize my BSN Application and the transfer of courses into the university. I understand the Supporting Documents Checklist, along with all supporting documents, must be submitted in one envelope at the same time, and received or postmarked by BSN Application deadline. I understand all prerequisites must be complete by the end of Mason s spring semester (May 20) for fall admission no exceptions. Prerequisite courses may not be completed during the summer session. I understand if I am offered admission to the BSN program and I fail to complete the nursing, Mason Core, and elective prerequisites by May 20 th the School of Nursing will rescind the offer of admission. Professional Conduct Policy: I understand all CHHS students are expected to conduct themselves professionally at all times. This means that certain behavior is prohibited, including excessive absences, verbal abuse, insubordination, and behavior that threatens the safety of a client, another student, a faculty/staff member, or other health care provider when the behavior occurs within the context of the academic program. CHHS reserves the right to place on probation, suspend, or dismiss any student in its programs who engages in such conduct. Students disciplined for such reasons have the right to appeal to their department chair or director. Academic Grade Standards: I understand that a passing grade is C+ (77%) in the BSN program. The grade of C- or lower in a nursing course may prohibit further progress in the major until the course is repeated and a satisfactory grade of C+ or higher is earned. Only one repeat is allowed. A second failure of a nursing course leads to termination from the BSN program. Alternate Education Plan: I understand that meeting eligibility requirements only allows my BSN Application to be reviewed and does not guarantee admission into the program. I understand admission to the BSN program is competitive and for this reason I have made an alternate (backup) education plan. Guaranteed Admission Agreement (if applicable): I have read and understand the Guaranteed Admission Agreement as described on the university main Admissions Office website (http://admissions.gmu.edu/transfer/gaa.asp).
