What are we worried about? The family is worried that they do not have a place to live, have no financial support since minors were detained and do not have basic needs met. Complicating Factors: Substance Abuse (Meth) Homeless Mental Health Unemployment Parental History of Childhood Abuse What s working well? The family is open to receiving services and eager to work with agency partners to address child welfare worries. CFTs and CFT meetings are a fundamental part of the Core Practice Model. A CFT will be developed for each child CFTs include: child/youth, parents, caregivers, extended family members, and other supportive people from the family s community. A Shared Family Care Plan will be developed, reviewed, and revised during the CFT meeting. Regular, ongoing CFT meetings will occur throughout the case to address specific issues and work towards achieving specific goals. * Commonly known as CFT

CFT-Linkages Pilot provides opportunity to merge best practices in Linkages meetings and CFT meetings
