SUPER BOWL 50 OFFICIALLY LICENSED BADGE PROGRAM Thank you for your interest in the Officially Licensed NFL Super Bowl 50 Badge program. The following is a preview package for the available NFL Approved badge styles, and information regarding how to participate. The last page of the package is a form to indicate your departments request to participate in the program, as well as which badges you want to utilize for your department or agency. IMPORTANT: The Super Bowl 50 Badge program does not require any department or government funds to participate. Badges selected by participating departments/agencies are made available for personal purchase by authorized officers and employees. The Super Bowl 50 Badge Program is available to area California public safety departments that are participating in the Super Bowl Public Safety Program - participation is subject to approval from NFL Security. Step 1 Please review the information attached and return the final page to Contact@SuperBowlBadges.com or via fax to 914-517-2782. Step 2 Once this completed application form is received it will be sent to NFL security for approval. If approved, a separate set of forms will be provided via email to your department POC to specify the badge details and options that will be made available to personnel when ordering online. All officers will be able to order online individually, and choose from the options specified. For security purposes BADGES WILL NOT BE shipped to individual officers or employees. All badges will be shipped to a designated POC at each respective agency and must be distributed by the department / agency to officers and authorized personnel. How to Order All orders must be placed online at www.superbowlbadges.com Phone orders are not accepted. Once your program is chosen and the options and POC are established a special section at www.superbowlbadges.com will be created specifically for your department or agency. Orders can only be accepted from individual officers of participating departments and agencies.
