(Arn l\ ~"ho.., M ; ;'>t"\m f'nnt'i"\i"mi~ l,lphlc..,.;:.it;; WiC:::{'(\nc:::in lmpm :on_ wn Purpose The purpose of this agreement is to establish and maintain a seamless articulated program between the institutions and to facilitate the transfer of students from Washtenaw Community College to Concordia University Wisconsin. Dual Admission Program Description The program provides a mechanism for students to earn the Associate of Applied Science Degree in Nursing and the Bachelor of Science Nursing completion (BS-Nc) Degree in a seamless, integrated curriculum. Students will begin in the A.D.N program at Washtenaw Community College, and then proceed to the RN-BS-Nc program at Concordia University Wisconsin for the BS-Nc coursework. The Associate of Applied Science Degree in Nursing from Washtenaw Community College will be awarded after successful completion of that program. The Bachelor of Science - Nursing degree from Concordia University Wisconsin will be awarded by the Registrar after successful completion of that program. To fulfill Concordia's degree requirements, a minimum of 6 credits must be taken at Concordia to earn a BS-Nc. These credits may include both online cohort AND liberal arts coursework. Application and Admission Requirements to the A.D.N program at Washtenaw Community College: As prescribed by Washtenaw Community College. Application Requirements for the RN-BS-Nc program at Concordia University Wisconsin: Submit an online application for admission to Concordia University and forward all required documents Include the offer of admission letter to the A.D.N program from Washtenaw Community College. Provide a letter of recommendation from the student's academic advisor or other appropriate official from Washtenaw Community College. Choose an entry term at Concordia University Wisconsin within two years of the anticipated date of graduation from Washtenaw Community College. Admission Requirements to the RN-BS-Nc curriculum at Concordia University Wisconsin: The candidate must: Maintain a Cumulative Overall Grade Point Average of.75 during pre-requisite coursework at Washtenaw Community College and for 'Full Acceptance' at Concordia University. Submit a criminal background clearance and child abuse clearance to Concordia University Wisconsin prior to admission as outlined in the Concordia University Wisconsin admissions application. International students or students with foreign credentials must submit an evaluation of foreign transcripts by the World Education Service (WES) or comparable agency. Physical and immunization status demonstrating the absence of communigable disease required prior to clinical placement. Personal essay regarding why CUW and why the advanced degree. Submit to an interview. Meet general admissions criteria as established by Concordia University Wisconsin. No record of disciplinary action at Washtenaw Community College. Eligible to sit for the NCLEX in the state in which student is planning to work. Currently working in healthcare (or have documented record of offer to hire) Current CPR certification

Admission Decisions Students who have been admitted to the A.D.N. Program at Washtenaw Community College and who meet the admission criteria established by Concordia University Wisconsin will be offered admission into the RN-BS-Nc program. Offers of admission to the RN-BS-Nc program at Concordia University Wisconsin, made at the time the student is accepted into the A.D.N. p;ogram at Washtenaw Community College are contingent upon the student fulfilling all of the Concordia University Wisconsin admission requirements upon completion of the A.D.N. program at Washtenaw Community College. Offers of admission to the RN-BS-Nc program, or withdrawal of admission offers, are determined by Concordia University Wisconsin at its sole discretion. Articulated Curriculum Students in this program will follow a specific curricular track at Washtenaw Community College that will include the prerequisite coursework needed to transfer into the RN-BS-Nc program. The required curricular track is set forth in Appendix I. Students who have completed the Washtenaw Community College curriculum (as referenced in Appendix 1), have met the admissions criteria as established by Concordia University Wisconsin, and have earned at least a 'C' in all prerequisite coursework will transfer into the RN-BS-Nc program. Maintenance The Nursing Administrator of Washtenaw Community College will serve as coordinator of the program and as advisor of students who plan to transfer to the Concordia University Wisconsin RN-BS-Nc program. The coordinator will: Be the responsible party for providing the institutional recommendation for current students; and Maintain files for those students who intend to transfer to the RN-BS-Nc program at Concordia University Wisconsin. Both the Colleges of Washtenaw Community College and Concordia University Wisconsin will assure that appropriate personnel in their respective colleges are made aware of the existence of this Agreement and are encouraged to support it. Washtenaw Community College and Concordia University Wisconsin staff will include: admissions staff, career planning and placement officers, transfer coordinators, and faculty. Personnel from Concordia University Wisconsin will visit Washtenaw Community College on a regular basis, but at least two times per year, to meet with the coordinator and other appropriate faculty and administrators. Meetings with students will also be arranged, including potential candidates who intend to transfer to the RN-BS-Nc program as well as other students who may simply wish to seek general information. The faculty, staff, and administration of Washtenaw Community College are encouraged to visit the campus of Concordia University Wisconsin.

To serve as a basis for evaluating the program, Concordia University Wisconsin will provide aggregate statistical information regarding application, applicant credentials, and admissions decisions. This information will be shared with Washtenaw Community College. Marketing and Recruitment Upon administrative and curricular approval, the Washtenaw Community College/Concordia University Wisconsin A.D.N. and RN-BS-Nc program will be listed in the Washtenaw Community College of Nursing Catalog in paper and/or electronic formats. Concordia University will supply Washtenaw Community College with promotional literature to be used in recruiting students into the RN-BS-Nc Programs. Promotional literature created by Concordia University Wisconsin or Washtenaw Community College must be reviewed and approved by all parties prior to distribution. Approval shall not be unreasonably withheld or delayed. General This agreement may only be amended, modified or supplemented by an agreement in writing signed by the parties. No Third Party Beneficiaries. None of the benefits or obligations of either party under this Agreement shall run to or be enforceable by any student or other third party. Assignment. Neither party shall assign, sell, or otherwise transfer this Agreement without the prior written consent of the other. Any such purported assignment, sale or transfer shall be void. At all times, Concordia University Wisconsin and Washtenaw Community College shall remain separate entities, each as independent contractors. Each shall continue to conduct its own business and affairs under the control of its own officers and Boards of Trustees and each of them shall remain solely responsible in all respects for the management of its own affairs. Length of Agreement This Agreement will continue in effect from year-to-year and will be automatically renewed annually unless terminated or amended. This Agreement may be terminated on June 0 of any calendar year by either partnering institution via written notice one year in advance. Students participating in the program at the time of termination shall be permitted to complete the program under the terms and conditions set forth herein. The addresses to which written notice is to be sent are specified below: No Third Party Beneficiaries.
