Welcome To Clarendon Medical Centre Practice History The original Clarendon House was built in 1967 and adjoined Donneybrook House, the whole building providing accommodation for 14 doctors. These premises were in fact one of the first purpose-built medical centres in the country from which such a large number of general practitioners provided medical services. In September 1993, the practice moved into temporary premises for about a year before taking up occupancy of the present-day building which had undergone substantial modification to become suitable for the provision of general medical services in a modern, spacious well equipped medical centre. If you are unhappy seeing your GP when there is a medical student present, please inform the reception staff or your GP and the medical student will be asked to leave. PRACTICE STAFF Practice Business Manager Mr David Mayne The Practice Business Manager will be able to help you with any administrative or non-medical aspects of the service we provide. Any suggestions about improving our services or indeed a complaint about the services can be made personally to the Practice Business Manager by telephone or letter. The Admin Team Admin Supervisor - Susan McInnes Mrs McInnes is responsible for the day- to-day running of the Administration Team which encompasses summarising, audit, prescriptions and secretarial work. Susan is also responsible for all Reception staff, Appointment Systems and deals with all aspects of patient registration. She will be happy to speak to you should you require any help whilst at the surgery. Receptionists All the Reception staff will help you and are your link with the rest of the health care team. When you telephone for medical attention the receptionist may ask for a few details. They have been instructed to make these enquiries so that the doctor can determine the urgency of your request in order to help you in the most appropriate way. Your reception staff includes: Deanne, Elaine, Joyce, Kirsty, Vicky Hett, Vicky Lawton and Tracey. Administration Team This section includes Ann (prescriptions), Kathleen (Typist) Tracey (Secretary / Typist), Louise (summarising) and Kath (data input clerk). If you require to speak to Ann about repeat prescriptions please call her between 2.30pm and 4.30pm on 0161 368 5224 option 5. Practice Nurses Elizabeth, Norma and Carolina We employ three practice nurses who are involved in the health promotion activities of the practice, such as Well Person, Asthma Clinic, Diabetic Clinic, Heart Disease Prevention, Menopause and Elderly Screening. In addition our practice nurses give vaccinations and advice for those travelling abroad, take blood tests and give injections. Norma is also fully trained in all aspects of family planning. Phlebotomists Louise and Vicky are trained to take blood. FBS District Nurses The district nurses provide home nursing services mostly for the housebound, elderly, chronically sick and people who have had recent operations. The district nurses hold clinics every weekday at Union Street Clinic. Appointments for ear syringing, dressings, removing stitches and injections can be made by ringing 0161 366 4100. Health Visitors The health visitors deal mainly with families and their children under the age of five years. They are involved with our child health-screening programme at Clarendon Medical Centre on Tuesday morning. They can be contacted at Selbourne House, telephone 0161 368 4242. Midwives The midwives attend our antenatal clinic on Wednesday afternoons and are available for advice at these times. This is for first time appointments only, others will be at Sure Start Clinic. In an emergency they can be contacted via Tameside General Hospital, telephone 0161 331 6159. Training The surgery closes once a month, usually the third Thursday afternoon, with the exception of August and December. Confidentiality All members of the Primary Health Care Team have the highest regard for confidentiality and will maintain your right to privacy. Access To Patient Information Healthcare professionals within the NHS and the private sector have access to patient information held at the practice which is relevant to the specific consultant to whom referral is needed. Written authorisation is required from patients in cases where copies of the medical records are requested from solicitors/insurance companies or for employment purposes. Under the Data Protection Act 1988, patients have the right of access to copies of their own medical records whether they are stored in computerised form, handwritten or stored in some other form. HOW TO CONTACT US Surgery Open Monday to Friday Saturday Sunday and Bank Holidays 8.00am to 6.00pm Closed Closed Your doctor normally holds a surgery every morning from Monday to Friday and most afternoons. Appointments Appointments can be made either by telephoning 0161 368 5224, by calling into the surgery or over the internet. If your own doctor is not available you may consult any of the doctors in the practice. However, we advise you to see the same doctor whenever possible as this ensures continuity of care. Please notify the receptionist as soon as possible if you are unable to keep your appointment so that we are able to offer the appointment to someone else. We will always see you on the same day if you need URGENT medical attention. There are three types of appointment - ROUTINE, URGENT, OR EMERGENCY. i) Routine Appointments You may book these up to six weeks in advance. If an appointment with your usual doctor is not available, you may accept an earlier appointment with one of the partners. ii) Urgent Appointments These are available every weekday, for patients whose medical condition will not wait until the first available routine appointment. Sometimes an individual problem requires longer than the allotted appointment time, and we may therefore run a little late. We ask for your patience in these circumstances. iii) Emergency Appointments These are for people who regard their medical problem as an on the day emergency. Your name, telephone number and brief description of your illness will be requested by the receptionist. These details will be passed to the nurse on-call who will return your call, discuss the problem further and assess the urgency. If appropriate you may be seen by another member of the Primary Care Team and not automatically the doctor. Only the immediate problem can be dealt with in these surgeries, as spending time on routine matters will cause long waits for other emergencies. Home Visits If you require a home visit please telephone the surgery BEFORE 10.00AM if possible. This will help the doctors to plan their visits for the day. If you telephone the surgery after 10.00am and the condition is not urgent, please inform the receptionist that a visit the following day will be acceptable. Home visits should only be requested for patients who are unable to attend the surgery because of the nature of their illness, which are those who are too poorly or physically incapable. Ill children will always be seen as soon as possible if brought down to the surgery. Please remember that the doctor can see several patients in the surgery in the time that it takes to make one house call. Out-of-hours Emergencies If you need urgent medical attention when the surgery is closed please telephone 0161 785 0805 or 0161 386 5224 which will put you straight through to the out-of-hours cover service. This is known as Go-to-Doc which is a co-operative of general practitioners from several practices across West Pennine. You will be asked for details of your illness and the doctor on-call will assess whether you need advice, to be seen at the treatment centre or require a visit. Your doctor is informed of all contacts made to Go-to-Doc. Telephone Consultations If you ever wish to speak to the doctor in person, it is better not to interrupt a surgery when he will be seeing a patient. There are telephone consultations available with each doctor. Please ask the receptionist who will explain the process. You may also obtain telephone advice from your health visitor, midwife or one of the practice nurses.

REPEAT PRESCRIPTIONS If you require regular medication for more than six months, your doctor may enter the prescription on the computer. This will enable you to obtain monthly prescriptions by presenting your computerised repeat slip to reception and collecting your prescription 48 hours later. Alternatively, you can post it to us (enclosing an SAE if you wish) or fax it to us on 0161 366 6303. Prescriptions can also be ordered via the internet. Please ask for details at reception. In order to avoid errors, prescription requests are not taken over the telephone. Patients on long-term treatment are asked to consult the doctor at six to 12 month intervals for a check on their medication and health. NEW PATIENTS When you register with a doctor he will wish to see you to get to know you and your family. He will also discuss the health questionnaire you will have completed. When registering you will also be given an appointment with the practice nurse for a general health check. DISABLED PATIENTS Our premises have suitable access for all disabled patients. COMPLAINTS PROCEDURE The practice has an in-house complaints procedure, set up under national guidelines. If you wish to learn more about it please ask for our leaflet compliments, comments and complaints from reception. LEGACIES / DONATIONS These are gratefully accepted for our equipment fund. For more information please speak to the practice manager or a doctor. Cheques should be made payable to Clarendon Medical Donation Account. OTHER SERVICES AT CLARENDON MEDICAL CENTRE Maternity Care All doctors at Clarendon Medical Centre are qualified to provide care during pregnancy in conjunction with the midwife. Most of your antenatal care will be provided at the surgery on either Monday or Wednesday afternoons. Please book an appointment at reception. Contraceptive Care We provide the full range of family planning services. Like all services provided by the practice, this is completely confidential. Please make a routine appointment with your doctor or practice nurse to discuss your requirements. Child Health Care The practice provides Child Development Clinics at Clarendon Medical Centre with the health visitor on Tuesday mornings at 9.15am. We feel it is important to find any problems there may be with a child's health as early as possible and the first six to eight weeks' check is always performed by one of the doctors at Clarendon. Child Immunisation Appointments can be made at your convenience with the practice nurses for all your children's immunisations. Minor Surgery We are able to perform some minor surgery procedures such as joint injections and the treatment of warts by liquid nitrogen. Flu/Pneumonia Vaccination Programme During October each year we commence our vaccinations against the flu virus. This is especially recommended for patients with chronic lung, heart or kidney disease, diabetics, asthmatics and the elderly. Cervical Smear Cervical smear tests are performed by our trained practice nurses. Travel Vaccinations Our practice nurses are trained to give both advice and vaccinations for foreign travel. Please make an appointment as early as possible, preferably six weeks before you travel. HEALTH PROMOTION CLINICS Well Woman Clinic These clinics are run under a doctor's guidance by the practice nurse, and offer a general medical check-up, including a cervical smear when indicated. Asthma Clinic We hold an Asthma Clinic for both children and adults. It is run under the doctor's supervision by the practice nurse who has been trained to advise on the care of asthma and the techniques used in the various forms of treatment. Diabetic Clinic We hold a weekly Diabetic Clinic run by a doctor and a nurse trained in the management of diabetes. A dietician also attends periodically, and we have excellent relations with local optometrists and podiatrists to anticipate complications of diabetes. We liaise with the Tameside Diabetic Centre. Heart Disease Prevention Clinic This clinic screens for risk factors associated with the development of heart disease, such as smoking, increased blood cholesterol and raised blood pressure. Appointments are available routinely.

Elderly Screening Clinic Screening for health problems associated with ageing is available by appointment each day with the practice nurse. Patient Recall The practice recalls patients with the following chronic diseases: Diabetes Chronic obstructive pulmonary disease Asthma We also recall at-risk patients for annual flu vaccination. Nurse advice is always available via telephone consultation. LOCAL PHARMACIES Cardiovascular disease Within Hyde all the pharmacies are part of the 'Minor Ailment Scheme'. This means that your local pharmacist can now treat some minor ailments and conditions on the spot, sometimes without an appointment. These include head lice, sore throat, cough, cold and hay fever. IF YOU ARE EXEMPT FROM PRESCRIPTION CHARGES, YOUR MEDICINE WILL BE FREE OF CHARGE. COMPLIMENTS, COMMENTS AND COMPLAINTS Our aim is to give the best possible service to our patients and we try to deal swiftly with any problems that may occur. We offer an in-house complaints procedure to deal with comments, concerns and complaints about the services we provide. A leaflet is available on request or full details can be obtained from the practice manager. Zero Tolerance We strongly support the NHS policy on zero tolerance. Anyone who attends the surgery and abuses the GPs, staff or other patients be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises. We may even request you have some exploratory tests to eliminate you from at risk areas. The practice would not be asking you to attend if we did not think you required them.
