Click Home Medications on the Navigator Home Medications Pathway

Click on Select Default Pharmacy Home Medications Pathway

Selecting a Patient s Pharmacy You may select up to two types of Pharmacy: One Retail and One Mail Order

Documenting a Retail Pharmacy Click on the Retail radial button. Enter the patient s zip code or the city and state. If the patient knows the name of the pharmacy enter it under the Name to limit the list of displayed pharmacies. Click on the Search button to display the pharmacies.

Select the correct pharmacy from the displayed list. Pharmacy Details will display in the bottom of the screen Click on the Select button to choose the patient s pharmacy.

The Patient s pharmacy will display on top the screen. Click on the Complete button to save the pharmacy.

Documenting a Mail Order Pharmacy Click on the Mail Order radial button. Select the mail order pharmacy from the displayed list. Click on the Select button to choose the patient s mail order pharmacy.

The Patient s mail order pharmacy will display on the screen Click on the Complete button to save the mail order pharmacy.

Deleting a Patient s Pharmacy

If a patient s pharmacy is incorrect or no longer being used by the patient, you will be able to delete it by clicking on the Delete button.

The pharmacy disappears from the patient s profile Click on the Complete button to save your changes

Eligibility Benefits

Eligibility Benefits Click Eligibility Benefits on the Navigator to request the patient s Pharmacy Benefit Selection. Click on the OK button to request eligibility

Once the benefits display on the screen, validate the information with the patient and select Yes or No Click on the Complete button to save your changes and exit the screen

Collect Home Meds

Collect Home Meds Display Changes If a clinician (such as an RN, Pharmacist or Pharmacist Technician) has been provided with a patient s medication list generated by an outside pharmacy or other external source, please verify with the patient ALL information contained on this list- including the patient s name and DOB are correct. An Advisory Note will display on the Collect Home Meds pathway as per Pharmacy Dept request. Read the Advisory Note and click the OK button to continue.

Prescription History Tab

Collect Home Meds Prescription History Tab A new tab Prescription History will be displayed under Collect Home Meds. The Prescription History comes from Surescripts. It displays a list of all of the medications that the patient has had for the past 6 months. The list comes from the patient s pharmacies.

The Prescription History tab includes the Medications History Date Range.

Click on the medication if you wish to view the Medication Details which include: Fill date, Quantity, Days Supply and Refills Provider Name Pharmacy

Each medication on the Prescription History tab needs to be validated with the patient, the same way that they are currently validated in the Existing Meds Tab. **The medications on this tab should be the first ones to be validated on the pathway** If the medication selected is correct, select the Add to Current Meds button to add the medication to the Current Home Medications.

If the Medication needs to be changed to a different dose click on the Search button to display the different doses on the pharmacy formulary and select the correct dose.

You may select the medication from the formulary (recommended) or you may enter the medication details on the bottom of the form and click on the Add to Current Meds button.

Under Medication Details enter the Dose, UOM, Route, Frequency and Last Dose if available Select the Add to Current Meds button to place the medication under Current Home Medication

Repeat the same process and validate each of the medications under the Prescription History tab with the patient Medication will display under Current Home Medications

Once all of the medications are validated under the Prescription History Tab, they will be highlighted in gray and transferred to the Current Home Medications of the patient.

Existing Meds Tab

Once you finish validating the medication under the Prescription History Tab, select the Existing Meds tab and continue to validate these medications as you did before on the previous pathway.

If there are duplicates of the same medication with different doses make sure to remove or not include the one with the incorrect dose on the patient s Current Home Medications list

Remove the medication with the incorrect dose by highlighting the medication and clicking on the Remove Med button

The Crestor 10 mg dose was removed from the Current Home Medications

Continue to validate the rest of the medications on the Existing Meds tab

This message will be displayed if the Home Medication collection has not been completed (medications not validated)

Medication History Reports

eprescribing Medication History Report Under the Prescription History Tab you may display a summary of all of the patient s medications by clicking on Prescription History Report

eprescribing Medication History Report Click on the Complete or Cancel button to exit the screen

Another way to view the Medication History of the patient is through the Imported Medication History on the Navigator.

Invision eprescription Frequent Q & A

1. How does the nurse know if the medication was printed or submitted electronically? The discharge packet will show if a medication was printed or electronically submitted. If electronically submitted it will display the pharmacy detail. On the Home Medications nurse s view under Display Home Meds, the icons of either printed or electronically submitted will display on the left side of the medications. If the space is blank, it means that the physician didn t issue any prescriptions for the patient. See print screen below.

2. How many pharmacies can be selected as default pharmacy for the patient? Only one retail and one mail order pharmacy may be selected at a time. The process of changing the pharmacy selected is manual and must be done if needed for multiple prescriptions to multiple pharmacies. Physicians also have the access to manually change pharmacies. 3. Does the eligibility benefit for the patient must be completed? Yes, this will benefit the patient during the discharge process to make sure that the medications selected are covered under their plan and it will display a substitute if needed. If this step is not completed, it will not be a hard stop for the nurse or physician to complete the discharge process. 4. Will social workers have the same view? If currently they have Home Medications on their view, they will now have the new options available. 5. Can the erx icon for physicians be removed as a mandatory function to electronically send the Existing Medications? No, this function is not a customizable option. It must be done in order to send a prescription electronically. 6. Can Eligibility Benefit be removed? No, this function may not be removed. It is a non-customizable function. 7. Can prescriptions for Narcotics be sent electronically? No, our system does not have the software that enables it at this time. 8. Can Medication History Report be printed? Yes, the option to print will display in the upper right corner as all of the other Net Access documents. 9. At Discharge Medication Reconciliation, are Infectious Disease medications restricted to only ID physicians? No, the system pulls the formulary from the NDDF files and it will not restrict certain medications only to specialties. 10. Can the Nurse reprint prescriptions? No, this function still remains a physician function. 11. How will charity patients prescriptions be handle? The function will remain the same as your current process. 12. What physicians will have access to eprescription? For implementation, the hospitalist will have the function available. Thereafter, any physician may be enabled as requested.
