MENU MEASURE- IMAGING RESULTS ATTACHED

Menu Measure: Imaging Results Attached MENU Objective: Imaging results consisting of the image itself and any explanation or other accompanying information are accessible through CEHRT. Measure: More than 10% of all tests whose result is one or more images ordered by the EP during the EHR reporting period are accessible through CEHRT. Exclusion: Any EP who orders less than 100 tests whose result is an image during the EHR reporting period; or any EP who has no access to electronic imaging results at the start of the EHR reporting period.

Menu Measure: Imaging Results Attached Image Tests (denominator): This report is based on xray orders/general orders, not patients. This report will ONLY look at orders ordered during the reporting period. (order date not create date) Xray/general orders marked Other Provider are not included in this report Image Results Attached (numerator): This is the number of xray/general orders that have an image result attached (Image Attached checkbox has been checked and there is at least one report linked to the order).

Menu Measure: Imaging Results Attached To set up X-ray/Order Reports Navigation: Medical Records > Medical Records Setup > All Orders > X-ray Setup: Possibly also Orders if DI testing ordered under General Orders.

Menu Measure: Imaging Results Attached Note: It does not matter how the user answers the prompt Link this as a "Report" versus a regular document link? The report is simply looking that the documents have been attached to the appropriate areas, linked above and that the Image Attached checkbox has been checked.

Menu Measure: Imaging Results Attached To meet this measure, the image result must either be incorporated into MEDENT or accessible via a link to the image with accompanying information (if such information is available). Image results that are scanned into MEDENT may be counted in the numerator. The user must manually identify when images are attached to a test.

Clinical Quality Measures In 2014, EPs must report on 9 of the 64 approved CQMs CMS has identified core CQM s- which are encouraged but not required. 9 CQMs for the adult population 9 CQMs for the pediatric population 2014 Clinical Quality Measures by Domain All providers must select CQM s from at least 3 of the 6 HHS National Quality Strategy domains. Clinical Quality Measures To select CQM s that you would like to use, click on the deactivated tab and all of the measures will appear. You will have to contact Medent to reactive them. You can submit a support ticket with Medent with the measures.

Clinical Quality Measures Once you have selected the measures you want, and they are reactivated they will appear with all the other measures highlighted in purple as CQM formulas. Applicable CQM s will then appear on the patient s DM/HM screen.

CLINICAL DECISION SUPPORT

Clinical Decision Support CMS Definition: Use clinical decision support to improve performance on high priority health conditions. Measures: Measure 1: Implement five clinical decision support interventions related to four or more clinical quality measures at a relevant point in patient care for the entire EHR reporting period. Absent four clinical quality measures related to an EP s scope of practice or patient population, the clinical decision support interventions must be related to high-priority health conditions. Measure 2: The EP has enabled and implemented the functionality for drug-drug and drug-allergy interaction checks for the entire EHR reporting period. Exclusion: For the second measure, any EP who writes fewer than 100 medication orders during the EHR reporting period.

Clinical Decision Support Select the CQM that you would like to make a CDS rule and click Clinical Decision Support on the toolbar. Click enter/edit formula.
