Tarrant County Hospital District d/b/a JPS Health Network is soliciting requests for qualifications (RFQ) for furnishing the services set forth in this specification. Felik Gwozdz Pl.) ON OR BEFORE SEPTEMBER 30, 2015 at 2:00 P.M. CST. All responses submitted to the Design & Construction Office must be in sealed envelopes or boxes. Any responses received after the date and/or hour set for qualifications opening will be returned unopened. If responses are sent by mail, the submitter shall be responsible for actual delivery to the Design & Construction office before the date and hour of submission. JPS Health Network Request for Qualifications I. GENERAL INFORMATION JPS has served Tarrant County since 1939. Since that time, JPS has experienced growth and changes in the population served, demands for its services, and technological and medical innovations; all in an environment of financial and operational constraints. JPS serves the community from its main campus which includes a Level I Trauma Center, over forty locations throughout Tarrant County including an ambulatory surgery center located in Arlington, and numerous school-based clinics. The JPS Mission is transforming healthcare delivery for the communities we serve. II. SCOPE OF SERVICES JPS Health Network requires the services of an architectural firm for schematic design, design development, construction documents, bidding and construction administration services to build an approximately 30,000 square foot patient medical home clinic - the Northeast Medical Home. The clinic will be located at 3200 W. Euless Blvd., Euless, TX 76022 on approximately 11.43 acres. The architectural firm will produce a project schedule and estimate of probable construction cost at the end of each phase of development. The architectural firm may team with an engineering firm to comply with the requirements of this RFQ. The architectural firm will work with JPS staff to develop the documents needed for construction. In addition, various user group leaders, physicians, and support department leaders may be involved in the design process. III. PROJECT DELIVERABLES The A/E team should have the ability to produce documents in the following format: Text Documents should be produced in Microsoft Office format using Microsoft Word, Excel, Access, and/or PowerPoint. Schedules should be produced using Microsoft Project. Drawings (as Builts) should be produced in AutoDesk Revit 2012 or later - 3 -
IV. SUBMITTAL FORMAT Submittals should be arranged in the following format, with tabs for each major heading (A, B, C, etc.) A. Executive Summary (Limit: one page) B. Team Members 1. Identify the project team. Please provide the following information on each firm or firms proposed: 1.0 Name of Firm(s) 2.0 Number of Years in Business 3.0 Project Responsibility (Primary Contact) 4.0 Principal Office Location 5.0 Location of Office Performing the Work 6.0 Number of Staff by Discipline 2. Provide an organizational chart of the proposed team, with specific project roles for each individual proposed. 3. Identify the individuals in the firm or firms who will be assigned to this project. Resumes should specifically address healthcare projects similar in size and scope to this project. Project Approach With regard to each of the following items, discuss your firm s approach and capabilities, and particularly identify any unique systems, practices, experience or techniques used by your firm. (Limit: one page) 1. Discuss your team s approach to working with Owners Representatives, MEP Consultants, and any authorities having jurisdiction over the project. 2. Discuss your firm s role as a team member of a coordinated project delivery and its approach to project control, including deadlines, quality of final product, and communication. D. Experience Identify three of the most recent projects similar to the proposed project, in which your firm has provided Architectural services. The following information is requested on each project: (Limit: one page for each project). 1. Facility Name and address 2. Project description (identify major elements of projects and/or unique features). 3. Project size (number of square feet, separate new construction from renovation) 4. E. Schedule Submit a typical schedule of activities with durations for the work and identify owner review milestones with appropriate durations. F. HUB Participation G. Claims Submit an overview of the firm s HUB/MWVBE (minority, women, veteran, ) utilization program and past performance on similar size projects. Describe how your firm will utilize HUB firms on this project or if your firm is certified (provide copy of certification if appropriate). Participation is strongly recommended. The District s participation goal is 25%. Describe all instances of project disputes, which, in the last five years, reached the level of formal mediation, arbitration, or litigation. For each dispute, describe the parties involved, the nature of the dispute, and the amount in dispute. Please provide this information for all the disputes arising out of the firm s projects, regardless of whether the firm was a party or a witness in the dispute. This information will be kept strictly confidential and used only for purposes of this selection. V. SUBMITTAL AND SELECTION PROCESS A. General Information All submittals received will be given a preliminary screening and evaluation based upon the criteria indicated. One or more firms may be selected to interview. The purpose of the interview will be to answer questions relative to the submittal, ensure an understanding of the objectives, and meet the project team members being proposed. JPS will make the final selection. JPS reserves the right to reject any or all submittals and to waive any informality in submittals received whenever such rejection or waiver is in the best interest of the project. It is understood and agreed by the submitting firms that submittals, interviews, etc. are voluntary and that JPS Health Network is not responsible for any compensation and /or other commitment associated with submittals or interviews. Questions and Clarifications Questions regarding the RFQ should be submitted to JPS Health Network, Attention Angie Morgan by the deadline indicated. Questions must be submitted by e-mail: amorgan@jpshealth.org Following receipt of substantive questions, responses will be submitted to all firms receiving this RFQ if necessary. D. Submission Submittals not received by the specified date and/or which do not conform to the enclosed outline will not be given further consideration.
