Forms can be opened and completed online, printed, signed by you and your principal then faxed to us at 214.827.7042 or 214.818.3951.

Guideline Basics All contact with The Meadows Foundation must be made by the teacher sponsor, not a student All projects must be on the Eligible Projects list to receive the $500 award Review the Ineligible Projects list to be sure the project(s) you are planning is NOT on this list

Requirements / Information Districts/Diocese with over five participating schools will have an assigned liaison (listed on our website @ www.mfi.org, then select Charitable Schools Program) Applications will be emailed mid-september Teacher sponsor(s) may review guidelines, forms and all Program information at www.mfi.org under Charitable Schools Program An email confirmation will be sent to you following receipt of your Application and Final Report (call our office if you do not receive a confirmation email within one week) Donations to a 501(c)(3) organization must be related to a student volunteer project and should not benefit students at your school

Complete Final Report form by April 1, 2013: Final Report Steps for Teacher Sponsors After opening on our website at www.mfi.org under Charitable Schools Program, the Final Report forms can be completed online. indicate number of students participating and total volunteer hours (i.e., 100 students x 2 hours = 200 hours) describe your project(s) and who it benefits in detail complete project budget information indicating how you spent the $500 award you and your principal must sign DON T WAIT UNTIL APRIL! Upon completion of your project(s) return by fax or mail if you want to include pictures

Examples of Eligible Projects Blood drives Food bank drives Tutor, mentor, or reading program with elementary students Coat/toiletry drive for homeless shelter Children s hospital projects/activities for patients Holiday projects (Adopt-an-Angel, Toys for Tots, Salvation Army, Goodwill, etc.) Food Baskets for needy families Texas soldiers/veterans projects Environmental/recycling projects Habitat for Humanity projects Campus beautification (students must be directly involved in the project) Drug/alcohol/tobacco prevention materials for student presentations Transportation to and from volunteer project(s) Supplies for volunteer project Renovation project for a local nonprofit agency Animal shelter project/supplies Baby items (blankets/supplies) for hospitals or shelters Senior citizen projects/activities @ a nursing home T-shirts (incentives for volunteer projects) Toy/stuffed animal drive Foster group home projects Special Olympics/special needs event Homeless volunteer/service projects Community Clean Up School spirit project If your project idea is not on this list, please call for approval.

Examples of Ineligible Projects NO GIFT CARDS School uniforms Out-of-state or foreign disaster relief or missionary work Toy drive for children outside Texas or U.S. Teacher appreciation breakfasts, luncheons or gifts Tuition reimbursement/scholarships Conference or convention fees Refreshments or decorations not associated with a volunteer project Drill, band/color guard uniforms, equipment or sports equipment Speaker s fees/honorariums Camp fees Six Flags/theme park tickets Project that only benefits one or a few individuals PAL training & annual conference fees/sponsorships/transportation Classroom textbooks, curriculum or orientation packets Purchasing backpacks or school supplies for students at your school Donation of $500 award to your school Donation of entire award to a local nonprofit without a volunteer project Entire $500 on volunteer incentive gifts Rolling over any remaining award to next year Gifts or parties for school teachers/staff General operating expenses for your school Equipment for your school (e.g. TVs, ipods, Nooks, ipads, etc.) Adopting families at your school Assistance with utility bills Transportation for regular school field trips Wondering if your project qualifies?
