Beat the rush and call the appointment line AARCC 684-6250 for a school physical appointment. You may download the forms from our website. If there is any chance your child will be playing a sport or involved in girl or boy scouts make sure you download all the right forms for your state (Kansas or Missouri) and the forms for scouts. The Health Center can receive up to $1000.00 for each completely satisfied survey returned.
