PCC086.15 Declarations of Interests Dr King stated as follows: Deputy Secretary Derby and Derbyshire LMCs. Locum GP - up to now working at 2 of the High Peak Practices intermittently. Deputy GP for the Out of Hours Service DHU. GP Specialist Adviser for CQC (inspections out of County only). Occasional GP adviser for North Derbyshire GP Federation Board. Shortly to take up contract with HSCIC providing Clinical Assurance for their articles on NHS Choices. Share holder/director of Elmpharm Ltd. Operating company of Burlington Road community Pharmacy in Buxton. Ex-partner (left 1/4/15) Elmwood Medical Centre, 7 Burlington Road, Buxton. Spouse: My wife works as Nurse Consultant to EMIS software supplier based in Leeds. There were no further specific Declarations. There were no changes to the Register of Interests. It was noted that the confidential session of the Primary Care Co- Commissioning Committee had considered an item on practice infrastructure funding on which Dr Milton had a conflict of interests; the matter had been considered during the confidential rather than public session as it involved commercially sensitive information. Dr Milton had no input to the recommendations made. It was confirmed in the interests of complete transparency that Dr Milton s absence from the meeting arose from diary commitments. PCC087.15 Minutes of the NDCCG Primary Care Commissioning Committee Public Session of 22 October 2015 and action log Mrs Derricott reported that an email had been received from Helen Foster, the Practice Manager at Oakhill Medical Practice, raising two points concerning the minutes of the previous meeting (quoted below): 1. The Sub Group the Practice Manager was on did NOT discuss the Care Home Specification which is the spec we objected to. North Derbyshire CCG Primary Care Co-Commissioning Committee 17 December 2015 Paper B 2. Dronfield has 2 representatives on the PCDG but they do not routinely involve the other practices and did not feedback about any contract discussions. I think it has since been recognised that people were not aware they were representing their locality at this group and changes have been made but at the time, we were not advised of these contact discussion nor asked for our input. The Committee was reminded that the discussion at that point in the previous meeting had concerned LMC representation and the way in which practices were engaged. It was noted that the CCG had already begun work on practice locality engagement, of which practices were aware, and that the matter had been raised at the most recent CCG Membership event. PCC089.15 North Derbyshire CCG Primary Care Co-Commissioning Committee 17 December 2015 Paper B Meningo Outbreak Mrs Derricott presented the report and led discussions, outlining the event and subsequent actions. Some contractual issues had been discovered around immunisations for school-aged children when local GPs had been asked to mop up children not captured in the mass vaccination scheme. Consideration was given to the question of practices responsibilities on the basis that the contract for vaccinations had been awarded to DCHS rather than primary care. Dr King cautioned against directing criticism at primary care in this instance and it was noted that, although primary care providers were not to be blamed for raising valid points about contractual responsibilities, in the circumstances it may have been appropriate to provide the mop-up vaccinations and address the question of remuneration under individual service payments once the affected patients had been immunised. The question of remuneration under individual service payments was ongoing. Communication problems had been apparent throughout the process; it had emerged early on that Public Health England (PHE) had not had the correct contact details for the CCG, and there had been delays in PHE s communication with practices. Mrs Stringfellow explained that the incident had tested a communications cascade not used since the last reorganisation of the NHS. Mr Apsley sought confirmation that every child had been immunised; Mrs Derricott advised that every child had been offered an immunisation, though the CCG had no right to enforce immunisation if it was declined by the child s parents. It was noted that one of the practices in the affected area had opened on a Saturday morning to ensure that patients could receive immunisations. This group continues to meet monthly to further refine working arrangements and to plan for 2016/17. The purpose of this paper is to update on progress delivering the co-commissioning development work programme. Key Issues and Recommendations: Considerable development was undertaken in the lead up to April 2015 to ensure that effective working arrangements were in place at the point of delegation. Feedback from CCGs and the Primary Care Hub team indicate that these arrangements are working well and relationships are good. The focus of this programme plan is on the interface and working arrangements between CCGs and the Primary Care Hub. It should be recognised that there is also considerable further work within CCGs to support co-commissioning. This current overall programme is split into 4 main areas of work: A. 2015/16 arrangements for GP co-commissioning B. Developments to support 2016/17 GP co-commissioning C. Developments to support pharmacy, optometry and dentistry co-commissioning D. Primary Care Hub arrangements for 2016/17 Further detail of the activities within the programme together with a progress update as of the end of November is enclosed.

Primary Care Hub Derbyshire and Nottinghamshire Overall there is positive progress against the programme plan. Key risks include: Primary Care Hub arrangements for 2016/17. CCG AO s have confirmed support for the continuation of the Primary Care Hub arrangement for 2016/17 which is extremely positive. Mansfield and Ashfield CCG are working with NHS England, on behalf of all CCGs, to explore potential options to strengthen the hub arrangement so it can better support CCGs deliver against the transformation agenda whilst also providing a secure career path for staff. NHS England central policy could limit the options available. Overall capacity within the system to support an increasingly demanding primary care agenda including transformation, quality improvement, responding to vulnerable and / or challenged contractors, national initiatives (e.g. GP-Infrastructure Fund, clinical pharmacist scheme) and the increasing complexity that co-commissioning introduced. In mitigation resource constraints have been highlighted as part of the AO s consideration of 2016/17 arrangements. Retention of key staff in the Primary Care Hub is challenging with continued uncertainty about future co-commissioning arrangements compounded by an unsustainable workload. In mitigation, immediate actions have been taken to replace a key member of the team who is leaving for a role in a CCG, and there has been reasonable interest in the post. Potential delegation of pharmacy, optometry and dental commissioning for 2016/17. In mitigation our current understanding of policy indicates that there will be opportunity for greater CCG influence of pharmacy commissioning for 2016/17, but not for dental or optometry. Further guidance is expected after November although at the time of writing nothing had been received. Vikki Taylor, NHS England North Midlands Locality Director, is the Midlands and East Regional lead for co-commissioning, and is an active member of the national development group. We are using this relationship to seek information and influence policy where possible. The programme plan continues to be refined as further developments are identified from our experience of implementing working arrangements.
