SCHOLARSHIP APPLICATION DOCUMENT CHECKLIST When submitting your completed Teacher Education Scholarship Program application, please ensure the following information and documents are included with your application. Do not staple this application or supporting documents as your application will be scanned on receipt. Documentation, as defined in the 100 point check, to support your identity. Refer to Appendix 1 of the information package, Identification Required by Applicants. Evidence of change of name will be required for first and family name changes. Documentation verifying: Australian citizenship, permanent residence, or visa permitting employment in Australia if not born in Australia. A passport size photograph must be provided and glued only to the front page. Section 9 Declaration by Applicant must be signed. Section 10 endorsement by referees requires signatures by two people who are not relatives but know you sufficiently well to be able to certify that the statements made by you in section 9 are correct. The Declaration and Employment Screening Consent form must be signed and dated. Please supply the original and not photocopy. Year 11 and Year 12 school reports or results to date. Please note, a school report is an official document outlining subjects and results obtained for each year of study. ATAR advice (or interstate equivalent) and admission advice from your university admissions centre is to be submitted once available. All documentation provided in accordance with the above checklist must be originals or certified copies of the original.
