Assigning Providers to Patients This document describes the assigning of a provider to a patient. There are two methods of assigning providers to patients. 1) An assignment is made from the patient card. 2) An assignment is made in the Patient Assignment Manager. Administrative personnel or provider group managers generally perform this action. Patient Card The Provider Assignments window allows the user to make a manual assignment in cases where it is known which provider will care for the patient and there is no need to review patient needs and provider requests before hand. Show Deleted: Indicates to refresh the list and show items that have been deleted as well as current assignments. Providers section Patient: Disabled field, displays the patients name. Provider: The provider Id of the provider being assigned. Is Primary: Indicates this is the Primary provider of care for this patient. Start Date: Indicates the effective date for this assignment. End Date: Indicates the end date for this assignment. Restricted Assignment section Restricted Tx: Indicates this provider is restricted to treatments of the specified discipline or to specific treatments. Discipline: Indicates the discipline provider is restricted to. When assignment is restricted to specific treatment the association is made via the <i> button and the Assign Treatments window. Treatments that are to be assigned to the provider are moved to the list on the right side of the window. The provider is then only able to modify these specific treatments within the patients chart. Patient Assignment Manager In order to utilize the Patient Assignment Manager, patients need to have their Patient Needs defined in one of three areas. 1) Patient Needs 2) A treatment has been planned and approved for the patient. 3) A general treatment plan has been created for the patient in the Treatment Planning module. Patient Needs are entered during the screening process via the Patient Card, or via right click in the Rolodex. This is a list of general patient needs that are not specific to ADA codes and have no associated dollar value. Show Deleted: Indicates to refresh the list and show items that have been deleted as well as current patient needs. Date: Indicates the date the patient need was added to the patient record. Assigned: If checked, indicates a provider has been assigned to fulfill the need. Need Code: Code to identify the specific need. Note: Optional note about the patient need. Planned Treatment Treatment that has been planned and approved will show in the Dental Chart as black text with a Status of P. General Treatment Plan Schools that are using the Treatment Planning Module with the General Treatment Plan tab enabled will be able to enter general Treatment Options for the patient in the initial treatment planning stages. In order to match the patient needs with the graduation requirement needs of the student, a list of Need Requests must have been generated via right click in the Rolodex or via the Create New Record icon in the Patient Needs tab of the Personal Planner. Provider: Disabled field, displays the name of the provider. This will default when a provider is logged in to axium. Otherwise, the provider can be selected via the button. Date: Disabled field, displays the date the need request was made. Time: Disabled field, displays the time the need request was made. Type: Indicates the type of need being added. Patient Need, General Tx, or Tx Procedure. Code: Indicates the need code being added. Patient Type: Indicates the Patient Type being requested for this need. Note: Note about the need request. Notice that the Patient Assignment Manager window is divided into two main sections. The top section contains information related to the provider, and the lower section contains information related to the patient. Each section is a search engine that will look for specific needs that have been requested. Each of the three sub-tabs in the upper section returns a list that matches the criteria filled in above (provider, provider group, date range, request type or specific code). Unassigned Requests is a list of all outstanding requests made by the providers. The list indicates the date of the request, full name of student, the year and program of the student, if there are any overdue conditions, and for the requested procedure the experience of the student in this procedure. Assigned Requests is a list of requests that have been fulfilled by an assignment. The list indicates the date of the request, full name of student, the year and program of the student, the need assigned, date on which the assignment was made, full name of the patient and patient chart number. Patient Assignments is a list of patients assigned. The list indicates the full name of the patient, the patient chart number, the full name of the student, the year and program of the student, the start date of the assignment, the end date of the assignment, indication of restricted assignment, and if restricted the discipline to which they are restricted. Via right click within the Unassigned Requests or Assigned Requests tab the user can access the following windows to aid in making an assignment. Show Request Details opens the Need Requests window to allow the user to see a list of all request made by this student. Date: Disabled field, displays the date of the need request. Time: Disabled field, displays the time of the need request. Type: Disabled field, displays the type of need (Patient Need, General Tx or Tx procedure). Code: Disabled field, displays the actual need code. Patient Type: Disabled field, displays the patient type if the patient has a type defined. Note: Field to enter a note about the need request. Show Tx History opens the Provider Plan Details window to allow the user to see the workload of the student. Provider: Disabled field, displays the name of the provider. Planned Tx: Lists all procedures currently planned for this provider. General Tx: Lists all general procedures currently associated with this provider. Show Assignments opens the Provider Assignment Preview window to allow the user to see the provider s patient load details. Provider: Disabled fields, displays the provider Id and full name. Need Code: Disabled fields, displays the need code, description and number of requests for this need made by student. Existing Assignments: Lists the existing assignments for the provider, indicating the date assigned and the date treatment was completed. Totals: Disabled fields, displays the total number of patients assigned to the provider that have this need, and the total number of those assignments that are completed. The lower section of the Patient Assignment Manager window is a separate search engine that will look for specific information on patients. The lower list can be run in Summary or in Detail mode. Summary Mode By highlighting an item in the upper list the Code field will be defaulted to the provider s requested need. The user can then generate a list of matching patients by clicking on the Search button. Detail Mode In detail mode the user chooses a sub-tab for the search and generates a list of patients by clicking on the Search button. Patient_Assignment.pdf Modified: 6-May-10 Page 6 of 8
The Criteria button allows the user to choose a pre-defined Info Manger report for use in the lower list. In Summary mode this option is not available. The OverDue button allows the user to view the Overdue Reasons of a patient. This is a list of items that are currently overdue for this patient. With this information the user can determine whether the provider being considered is the best option for the patient. If another provider is overdue for all assigned treatments for this patient the user may wish to re-assign the patient and give them to a different provider. The Details button allows the user to view the Patient Plan Details. Patient: Disabled field, displays the patients name. Planned Tx: Lists all procedures currently planned for this patient. General Tx: Lists all general procedures currently associated with this patient. Patient Need: Lists all patient needs currently associated with this patient. The Pt Card button allows the user to navigate to patient card to view further demographic information, insurance information etc.. for the patient prior to making the assignment. The Assign button allows a patient in the lower list to be assigned to the provider in the upper list, when the criterion in both sections matches. On <Add> the Patient Assignment window will open to allow the user to identify other needs that can be fulfilled with this assignment. Provider: Disabled field, displays the provider name. Patient: Disabled field, disaplays the patient name. Unassigned Requests: Lists all of the providers unassigned requests. Patient Need: Lists all patient needs currently associated with this patient. Planned Tx: Lists all procedures currently planned for this patient. currently associated with this patient. General Tx: Lists all general procedures The user will click on matches between the providers unassigned requests list and the patient s unfulfilled needs lists to assign these additional needs to the provider.
