JOB PURPOSE The pst hlder will be expected t prvide an accessible, apprpriate, high quality, culturally sensitive and effective health visiting/public health nursing service t all age grups in the cmmunity, by wrking in partnership with service users, care prviders and ther agencies. Using a cmmunity develpment apprach, wrk in partnership with clleagues, individuals, the lcal cmmunity, health imprvement staff and ther agencies t identify and address health needs. The pst hlder will functin as an independent autnmus practitiner by virtue f in-depth knwledge, expertise, prficiency and experience t lead and enable the health visiting/public health nursing team t prvide a quality service. The pst hlder will be the clinical expert wh will be accuntable fr case management and health visiting activity within the caselad, and/r lead, manage and supprt the health visiting/public health nursing team. 3. ROLE OF DEPARTMENT The health visiting/public health nursing service perates an pen access service t families and cmmunities within hme, cmmunity and CHP settings. T prmte a healthy lifestyle within the lcal ppulatin giving particular attentin t the identified health needs f the lcality, and target grups such as children 0-5 years, schl age and yung peple, vulnerable families T wrk in a manner which reflects the WHO principles f addressing inequalities in health thrugh participatin, cllabratin, empwerment and CHP. The health visiting/public health nursing service will als undertake case management which wuld include assessment f health needs, primary preventin, identifying vulnerability and targeting interventins apprpriately t meet the needs.

SCOPE AND RANGE The health visiting/public health nursing service perates an pen access plicy that is available Mnday t Friday excluding public hlidays. The service des nt perate a waiting list, hwever, needs are priritised t be able t prvide bth planned and unplanned interventins. This includes the ability t respnd timeusly t clients/families wh require an urgent visit. The health visitr/public health nurse will be respnsible fr the management f a caselad f families and/r leading and managing a team f qualified nursing staff and health supprt wrkers. The caselad ppulatin is derived frm either the GP practice t which the team are aligned, r within a defined gegraphical ppulatin and/r a defined client grup. The caselad cmprises apprx. 100-450 families with a range f vulnerability. Skills required include the prvisin f highly specialised advice, parenting skills, health prmtin, health educatin, psych-scial interventins and immunisatin. At times high-level health visiting interventins are required when dealing with issues such as child prtectin, dmestic abuse, addictins, peri-natal illness, elderly abuse and children affected by disability. The health visitr/public health nurse will prvide reciprcal cver and supprt fr the wider health visitr/public health nursing caselads in the absence f the designated caselad hlder in rder t prvide clinical leadership and ensure the cntinuatin f service delivery. Whilst there is n direct financial respnsibility with this pst, the pst hlder has respnsibility fr the rdering f nursing equipment within a defined budget (up t 750 per rder). The pst hlder will be expected t wrk cllabratively with the Lcality Childrens Services Manager and Head f Childrens Service t ensure effective utilisatin f resurces - this will include the nursing wrkfrce resurce, as well as the effective management f specialist nursing equipment and supplies. The pst hlder will be

respnsible fr apprpriate and cst-effective nurse prescribing including dressings, devices and medicatin. The pst hlder will wrk independently and autnmusly withut supervisin and be respnsible fr delegating and reviewing the wrk f his/her team. 6. MAIN DUTIES/RESPONSIBILITIES The pst hlder is respnsible and accuntable fr the nging management f the caselad t ensure the assessment, planning, implementatin and evaluatin f prgrammes f care. These will be develped in partnership with Patients, Family, Carers and Significant thers, and will be evidence based taking int cnsideratin the lifestyle, race, gender and cultural backgrund, and factrs which impact n physical, mental r emtinal health. The pst hlder will wrk in partnership with clleagues, individuals, the lcal cmmunity, health imprvement staff and ther agencies t identify and address health needs as a key member f the health imprvement wrkfrce taking accunt f natinal and lcal strategies. As part f the managerial respnsibility the pst hlder will be familiar with and ensure implementatin f lcal, reginal and natinal plicies, prcedures and guidelines pertinent t all staff within their area f respnsibility in the pursuit f the highest standard f care. The pst hlder will have an understanding f the clinical budget and its applicatin in the planning and implementatin f care in the area, in rder t meet service aims and bjectives. Althugh the pst hlder des nt have the budgetary respnsibility fr the health visiting/public health nurse service it is his/her respnsibility t maintain the ntinal allcated wrkfrce resurce r highlight any prblems within the team and the wider clinical area. The pst hlder will understand and wrk within the rganisatinal recruitment plicies which include equal pprtunity plicies, cnfidentiality and data prtectin plicies, when identifying and recruiting t vacancies within the team. The psthlder will be respnsible fr the recruitment and selectin prcess, taking cgnisance f the needs and dynamics f the wider health visiting/public health nurse team when making decisins. Maintain accurate up-t-date clinical recrds and care plans in accrdance with NMC standards fr recrds and recrd keeping, ensuring cmpliance with the Data Prtectin Act and the Freedm f Infrmatin Act.
