Response Submission Requirements: Letter of Intent Any bidder planning to submit a RFP response for on-site dialysis services SHOULD submit a letter of intent via postal mail to the address in the RFP or email form to: Stacey Glass- Smith@cdcr.ca.gov. [ See page in the RFP.] Faxed forms will not be accepted. Voluntary Letters of Intent are due on May 24, 2010. Bidders may utilize the Letter of Intent form located in Section 5 in the RFP (page ). Additionally, refer to www.bidsync.com for registration and access to RFP addendums and alerts.

Response Submission Requirements Deliver five (5) bound copies (with one marked Original Copy to the address located in the RFP (See section 1.4). Responses MUST be received at CPHCS Medical Contracts by June 24, 2010 at 2 p.m. Proposals that are postmarked but not received by June 24, 2010 at 2 p.m. will not be accepted.
Response Submission Requirements Submissions must be typewritten in Times New Roman (preferably)12-point font, single spaced, with one-inch margins. 8 ½ x 11 white paper, double-sided Question number (and title if applicable), followed by response (example provided in RFP) Page limits are identified for each of the response sections. Narrative response sections must be numbered with page numbers located in lower right-hand corner of the response. Attachments are not included in page limits. PROPOSALS WITH A NARRATIVE RESPONSE SECTION THAT DOES NOT MEET THE RESPONSE SPECIFICATION REQUIREMENTS OUTLINED IN RFP WILL NOT BE ACCEPTED OR CONSIDERED.
