Kids College is a summer enrichment program designed to spark the imagination and expand the knowledge of youth entering grades 1 through 3 in the fall of 2016. ELIGIBILITY Any child who will enter grades 1 through 3 in the fall of 2016 can enroll. Students who do not display appropriate behavior will be removed from the program without a refund. DATES & SCHEDULE Monday Friday, July 11-15, 2016 8:30 a.m. 9:45 a.m. Class #1 9:45 a.m. 10:00 a.m. Break/Snacks 10:00 a.m. 11:15 a.m. Class #2 11:15 a.m. 11:45 a.m. Lunch 11:45 a.m. 1:00 p.m. Class #3 1:00 p.m. 1:15 p.m. Break/Snacks 1:15 p.m. 2:30 p.m. Class #4 (Pick-up time is 3:00 p.m.) REGISTRATION AND TUITION No registration is complete without a signed application, registration, a signed release of waiver of liability and payment for each student (p. 7-9). Complete and return all required forms to the UACCB community education office, no later than May 27, 2016. After May 27, late registrations are accepted with a $25 fee (no exceptions). Siblings pay $25 less per week for the second child. Fees are as follows: $180 full-day classes, includes lunch and two snacks each day $105 half-day classes, includes one snack each day $ 25 additional per week for lunch (optional for half-day student) SCHOLARSHIPS A limited number of partial scholarships are available. Parents or guardians must furnish a copy of their child s free/reduced lunch eligibility letter on school letterhead signed by a school official. You may request for the school to fax the free/reduced lunch eligibility letter to Fax# 870.793.4988. If the student is home-schooled, parent or guardians must furnish a copy of the 2015 individual income tax return (1040). The completed scholarship application (p. 10) must accompany the $50 payment and the child's application, registration and release of waiver of liability. (p. 7-9). Deadline to apply for scholarship is May 27, 2016. REFUND POLICY Cancellations and requests for refunds must be received in writing (letter, fax, email) seven (7) business days prior to the start of each class session. RELEASE OF WAIVER OF LIABILITY (PART 3 OF 3) I give permission for my child to participate in Kids College camp held at the University of Arkansas Community College, 2005 White Drive, Batesville, AR, owned and/or operated by the Board of Trustees of the University of Arkansas for and on behalf of the University of Arkansas Community College at Batesville (University). KIDS COLLEGE 2016 SCHOLARSHIP APPLICATION The University of Arkansas Community College at Batesville Kids College Scholarship is for a partial scholarship. This scholarship may be applied to the tuition costs for one week of the 2016 Kids College. There is no guarantee of funding. Scholarship funding depends on UACCB s fundraising efforts and the number of applicants. Scholarships will be awarded based on a first come, first serve basis. In addition to completing this form, parents or guardians must furnish a copy of their child s free/reduced lunch eligibility letter on school letterhead signed by a school official. You may request for the school to fax the free/reduced lunch eligibility letter to 870.793.4988.
