Submission Instructions Deadline: Friday, April 6, 2018 is a program that gives area high school students a chance to enhance their knowledge and develop leadership skills that will empower and motivate them to become involved and committed to their community. Students from Altoona, Augusta, Fall Creek, Memorial, North or Regis high school or Wildlands Charter school who will be juniors during the 2018-2019 school year are eligible to apply. Selection Process The Selection Committee will select 36 participants for the program based on the application and references. The Selection Committee will be unaware of the identity of the candidates. Participants will be notified of acceptance in May 2018. An orientation session will be held in June for all participants and their parents or guardians. No material fee is due before acceptance. How to Apply 1. Complete the first three pages of the application. If your application is not complete, your application may not be considered. 2. Sign your name at the bottom of page three. 3. Prepare and attach your essays to your application. 4. Request that your references complete the forms (pages four and five) and send them directly to the Eau Claire Area Chamber of Commerce or to a school representative. 5. Request that your high school principal complete the permission form (page six) and send it directly to the Eau Claire Area Chamber of Commerce or to a school representative. 6. If needed, complete the scholarship application, which can be downloaded at www.eauclairechamber. Click on the link and then on the Application and Tuition tab. This form will not be part of the application reviewed by the Selection Committee. To have a form emailed to you, please email your request to seelen@eauclairechamber.org 7. Application Please respond to the folllowing questions and attach your completed essays to your application. Be sure to type your response, using no less than 10 point type. This essay will be an important part of the selection process. A. Describe what it takes to be a leader and provide an example of when you demonstrated one or more of these skills. (Maximum one page, double spaced.) B. Perseverance Every obstacle we face in life comes with an opportunity to learn something new. Describe a challenge that you have faced, how you persevered and what you learned from the experience. (Maximum half page, double spaced) C. Future What do you see yourself doing in 10 years? Application Parent/Guardian Permission If accepted, my son/daughter has permission to participate in this program. I understand that this includes an overnight opening retreat on July 26-27, 2018, and the following program days on Wednesdays, September 12, October 13, November 7, December 5, 2018, February 6, March 6, April 10 and May 22, 2019. I understand, that the superintendents of the Altoona, Augusta, Eau Claire and Fall Creek school districts and the president of the Regis Catholic Schools have approved these days as excused absences. Materials Fee and Financial Assistance If selected, my son/daughter will be billed for the material fees of $100, which is payable in full by June 1, 2018. Youth Leadership Eau Claire has financial assistance available for individuals who demonstrate a need. If my son/daughter requires financial assistance to participate in the program, I understand that he/she must include a separate short statement indicating why he/she needs financial assistance. The Selection Committee will not be made aware of his or her financial status.
