AMGA will also otify those who were ot selected to be icluded o the 2018 coferece program. Geeral Submissio Rules Sed completed proposals to Proposals@amga.org with read receipt optio eabled. Proposals submitted to ay other AMGA email address will ot be accepted. Proposals must be received by 5:00 p.m. PT o Friday, July 7, 2017. Submissios must be received by email. Hard copy submissios by fax or mail will ot be accepted. Proposals must be submitted i Microsoft Word (PDF will ot be accepted); please use Times New Roma fot, size 12. Please avoid special formattig styles. Your orgaizatio must be a AMGA member i good stadig for your proposal to be accepted for review. Additioally, if your orgaizatio s member status should chage prior to the coferece, the presetatio could be removed from the program. AMGA Full Disclosure Policy As a accredited provider of cotiuig medical educatio activities, it is the policy of AMGA to esure balace, idepedece, objectivity, ad scietific rigor i all its educatioal activities. AMGA is required to idetify ad resolve all potetial coflicts of iterest with ay idividual (or their spouse/parter) i a positio to ifluece ad/or cotrol CME activities. A coflict of iterest will be cosidered to exist if the idividual has received fiacial beefits (e.g., grats, research support, hooraria, employee, cosultat, board of directors, stockholder) i ay amout from a commercial iterest (ay propriety etity producig healthcare goods or services cosumed by or used o patiets) withi the past 12 moths ad that idividual is i a positio to affect the cotet of CME regardig the products or services of the commercial iterest. All idividuals i a positio to ifluece ad/or cotrol the cotet of AMGA-sposored CME activities are required to disclose to the AMGA ad subsequetly to learers that the idividual either has o relevat fiacial relatioship or ay fiacial relatioship with the maufacturer(s) of ay commercial product(s) ad/or providers of commercial services discussed i CME activities. All disclosure iformatio provided to AMGA will be reviewed to esure that o coflicts of iterest exist prior to the cofirmatio of the idividual for the educatioal assigmet. Additioal iformatio may be requested. It is the resposibility of the idividual to otify AMGA of ay chages i the disclosure iformatio provided to AMGA. The itet of this policy is ot to prevet a speaker with a potetial coflict of iterest from makig a presetatio, but to esure that ay potetial coflicts are idetified opely so that the learers may form their ow judgmets regardig possible bias. I keepig with this policy, CME faculty relatioships shall be disclosed to participats prior to educatioal activities i brief statemets i coferece promotioal materials, hadouts ad also i post-meetig publicatios. Refusal to disclose relatioships will disqualify the speaker from the plaig ad implemetatio of the activity. It is also the policy of the AMGA to maitai complete idepedece i the use of cotributed fuds. All fuds from commercial sources will be i the form of educatioal grats made payable to AMGA for the support of programmig. The terms, coditios, ad purposes of such grats will be documeted by a agreemet siged by the commercial supporter ad AMGA. No fuds from a commercial source shall be paid to the director of the CME activity, faculty, or others ivolved with the supported activity. Further, AMGA will provide upo request, iformatio cocerig the expediture of fuds provided by each commercial supporter. - 7 -

Speaker Reimbursemet Policy The AMGA has established the followig reimbursemet policy for speakers at the AMGA 2018 Aual Coferece: Oe complimetary coferece registratio The maximum travel ad hotel reimbursemet is $800 Please ote that this is per sessio, NOT per perso. If you have determied that there will be more tha oe speaker for your sessio, this reimbursemet may be distributed as idicated by each speaker withi the presetatio. You may use the travel ad hotel allotmet for airfare, groud trasportatio, icidetals (meals ad sacks) icurred durig travel ad hotel room ad tax charges. All receipts must be provided for reimbursemet. No-reimbursable Persoal Expeses AMGA does NOT reimburse preseters for audiovisual materials such copyig expeses, icurred before or durig the aual coferece. Other items that will NOT be reimbursed are phoe calls, Iteret coectios, gym/spa fees, movies, ad group meals. Forfeiture of Reimbursemet Please ote, i order for AMGA to produce attedee materials i a timely fashio, attetio to deadlies must be eforced. If materials ad forms are ot retured by the stated deadlies, your reimbursemet request will ot be hoored. I additio, AMGA will adjust the requested reimbursemet amout for ay chages to audio/visual equipmet orders made less tha 24 hours before cofirmed presetatio based o a pricig schedule to be determied at the time of the speaker s cofirmatio. AMGA will reimburse all speakers upo receipt of a completed speaker reimbursemet form followig the aual coferece. Speaker reimbursemet requests must be received withi 30 days of the aual coferece or the request will be forfeited. AMGA will solicit idustry support through educatioal grats for may sessios.
