If you re in a nursing home: You and your loved ones will be supported by a nurse care manager who will: Provide you with a detailed whole person care plan with the goal of helping you live a higher quality of life. Coordinate your care with staff, identify issues, and speak up on your behalf. Provide health updates to you and your doctor. With your OK, he or she will communicate with your loved ones too. Be your extra pair of eyes to ensure proper care. Coordinate transportation, if needed, for medical services. Do everything possible to make sure your wishes and preferences are respected. If you re at home: Your loved ones will have peace of mind knowing that you have access to a program that lets you stay at home. It provides personalized, in-home service to help you remain independent. The program will: Provide you with a detailed whole person care plan with the goal of helping you live the healthiest life possible. Help you remember to take your medicines. Help you schedule doctor appointments. Coordinate transportation to your doctor appointments.
Are you enrolled in the Aging Waiver? Our plan works with your current services to help keep you independent at home. We will support your decision to stay in your home. You will not lose your waiver supports by enrolling in our plan. We will be a resource for how Medicare and Medicaid interact. You will not pay any member premium or cost-sharing.
