We are dealing with your request under the provisions of the Freedom of Information Act 2000. You requested information regarding wound care. Specifically you asked: 1. Please supply me with your most up to date wound care formulary (please indicate if you do not work from a formulary)? Answer: Please see the attached two documents. 2. Who has operational responsibility for the formulary including the addition or deletion of products? Answer: Matron Tissue Viability 3. Please provide information on how compliance with your formulary is enforced? Answer: In community GPs have issued an order form and they question any off formulary requests. In Acute setting staff can only order from the wound formulary, any other dressings must be approved by TV. 4. Please provide information on how products are evaluated prior to being accepted onto the formulary? If this is part of a clinical evaluation or clinical trial, please provide the results? Answer: Regional formulary group, like for like products are assessed by DN teams but no formal evaluations as all products are CE marked. If a product is placed on

the formulary and after a settling in period does not perform then it would be changed otherwise the main criteria is spend In Acute setting TV with ward staff evaluate products during dressing formulary review period only and formal evaluations are undertaken and held within procurement during the review process. Same as community if the product doesn t meet what is needed then it would be removed from the formulary. 5. Please provide information on who is responsible for the cost of wound care in your setting is it CCG or Acute? Answer: In community it is the CCG. Acute in hospitals budgets held at ward level, budgets monitored by TV matron & procurement periodically 6. Please provide information if the wound care products is on prescription FP10? Answer: All products are provided through FP10 in community. 7. Please provide a list of the wound care products that you are planning to keep on prescription FP10? Answer: As above at this time. 8. Please provide information if you are planning to take wound care products off prescription FP10? Answer: Not at this time 9. Please provide a list of the wound care products that you are planning to take off prescription FP10? Answer: NA 10. Do you hold information about your patient demographic? If so, please provide information on what is your patient demographic and what are the most commonly treated wounds from your patient demographic? Answer: Community patients 50% leg ulcers, 25% problem surgical wounds, 15% pressure ulcers 10% other wounds new and chronic. Acute approx. 60% surgical, 10% trauma, 10% pressure ulcers, 20 % other including chronic wounds. 11. Please provide any information that you have relating to the average monthly usage of each product in the formulary?

Answer: Community usage; Not available this is held by the North East Commissioning Unit. Acute Usage next to each category. 12. Please provide information on the number of wound care products used that are not currently on the formulary? Answer: No information available for community. None used in acute 13. If so when does it expire? Answer: Under NHSSC contract ends 31/12/20. 32. Please provide information on how many delivery points do you have for wound care deliveries? Answer: 4 x community hospitals 1 x ward each, Main sites Delivered into stores, BAGH delivered into stores. 33. Please provide information on how many community locations are holding stock of wound care and prescribing to the community? Answer: No stock as use FP10. 34. Please provide information on what software platform do you use for ordering wound care products? Answer: Cardea, Materials management PDU. 35. Do you have a wound care clinic in place for your local demographic and where do your referrals come from? TV run a leg ulcer clinic in Darlington for Darlington patients only funded by the CCG. Referrals come from GP s. Dressings clinic run by plastic unit, referrals are own consultant patients.
