PERSONAL and HOME CARE SERVICES DEFINED Incidental Services: Any service that is not provided on a routine basis, and is not specifically identified in the service agreement. Billed in 15 minute increments. Service may be given by either a nurse or a home care aide, depending on the level of care needed. Bathing Assistance-Level One: Set-up of bathing supplies and clean up of bathing area Stand-by assistance if needed, or assistance with washing and drying. This service allows for approximately 30 minutes of staff time. Bathing Assistance-Level Two: Set-up of bathing supplies and clean up of bathing area Assistance with washing and drying. Applying lotion, nail care and shampooing hair as requested. This service allows for approximately 30-45 minutes of staff time. Bathing Assistance-Level Three: Set-up of bathing supplies and clean up of bathing area Assistance with washing and drying. Applying lotion, nail care and shampooing hair as requested. This service allows for approximately 45-60 minutes of staff time. Whirlpool tub Dressing and Grooming-Level One: Set up grooming supplies and lay out clothing. Needs minimal assistance with buttoning shirt, putting on socks and shoes. Assistance given once or twice per day. Services provided for 15 minutes Dressing and Grooming-Level Two: Assistance with putting on and/or removing clothing. Assistance given once or twice per day. Includes assisting with oral care and personal hygiene. Includes putting hearing aid in and taking it out. Changing batteries as needed. Includes assistance with TED hose/compression stockings. (Includes washing and drying stockings, and applying lotion if necessary.) Services provided for 15-30 minutes

Assistance with TED Hose or Compression Stockings: Includes assistance with putting on stockings in the AM, removing them at night. Service also includes applying lotion to legs, assistance with shoes, and washing stockings at night. Assisting with ted hose or compression stockings 5 minutes each time. Toileting Assistance/Incontinence Care: Charges according to the number of times per day. Assist with transferring on and off toilet or commode with limited transfer assistance. Changing incontinence products. Providing hygiene after changes and changing clothing as needed. Ordering of incontinence supplies, if requested. Emptying of commode as needed. Monitoring of skin condition. Catheter Care: Staff assists with emptying of leg/night bag. Staff monitors for signs of infection in urine and reports to nurse. Staff measures output and documents Ostomy Care: Changing/emptying of ostomy bag by nursing staff. Safety Checks: Charges according to the number of times per day. Staff members checking on the tenant to assure that tenant is safe/secure. Escort Assistance- Level 1: Assistance within the facility for those tenants with mobility limitations. Need staff to push tenants wheelchair. Includes escorts to meals and activities up to 4 times a day. Escort Assistance- Level 2: Assistance within the facility for those tenants with mobility limitations. Needs assistance of one staff and gait belt with ambulation/guidance. Includes escorts to meals and activities up to 4 times a day. Transferring Assistance-Level 1: Requiring assistance to transfer from bed to chair. Limited assistance from one staff person for transfers. Monthly fee based on clients transfer status during other services, with additional fees for those services: ie toileting, dressing, escorts etc.

Transferring Assistance-Level 2: Requiring assistance to transfer from bed to chair. Requires assistance to transfer per 2 staff person. May use sit to stand mechanical lift, but must be weight-bearing. May use full body Mechanical lift. Turning and Repositioning Assistance: Requires assistance to turn and reposition in bed Levels vary depending upon the number on times for assistance Safety Devices: Need use of bed alarm Need use of Wheelchair alarm Need use of door alarm Need use of wanderguard Charges per each safety device Medication Assistance/Administration: Home Care Staff assist with self-administration of medications daily as ordered by physician. Administration of eye drops, nasal sprays, creams/ointments and any oral medications that tenant cannot self-administer. Order medications as needed from pharmacy upon admission. Daily documentation of medications administered. Medication Set-Up Level One: Medication set-up weekly in medication containers by a licensed nurse. Tenant administers oral medications independently. Order medications from pharmacy upon admission. Licensed staff monitors medication compliance. Medication Set-Up/Insulin Level Two: Medication set-up weekly in medication containers by a licensed nurse. Setting up of insulin in syringes by licensed nursing staff. Tenant administers oral medications independently. Order medications from pharmacy upon admission. Licensed staff monitors medication compliance.

Medication Administration/ Eye Drop-Level 1 Home Care staff will assist with instilling eye drops as ordered by physician. Charges according to the number of times per day. Administer eye drop up to 2 times a day Medication Administration/Eye Drop-Level 2 Home Care staff will assist with instilling eye drops as ordered by physician. Charges according to the number of times per day. Administer eye drop 3 or more times a day Medication Administration/Inhaler-Level 1 Home Care staff will assist with inhalers as ordered by physician. Charges according to the number of times per day. Administer inhalers up to 2 times a day Medication Administration/Inhaler-Level 2 Home Care staff will assist with inhalers as ordered by physician. Charges according to the number of times per day. Administer inhaler 3 or more times a day Medical Monitoring: Blood pressure checks as ordered by physician or per tenant request. Monitoring of weights as ordered by physician or per tenant request. Administration Injectables (Insulin)- Level 1: Administering/assisting 1-2 times a day Administering/assisting tenant as ordered by physician. Ordering supplies as needed. Documentation of administration. Administration Injectables (Insulin)- Level 2: Administering/assisting 3+ times a day Administering/assisting tenant as ordered by physician. Ordering supplies as needed. Documentation of administration. Medication Order Changes: Transcribing new orders. Notifying pharmacy of new orders. Instructing staff on usage and side effects of new medication. Training staff if specific orders are given.

Central Storage of Medications: Medications are stored in a locked drawer in apartment if tenant receives medication assistance or administration. Glucose Monitoring: Charges according to the number of times per day. Blood sugar testing as ordered by physician. Contacting physician with level outside of defined parameters. Ordering supplies as needed. Documentation of daily readings. Restoravite Program Charges according to the number of times per day Provide range of motion as prescribed by PT or MD Ambulation program besides escort servoces 1:1 Companion Awake staff member providing 1 on 1 assistance with the tenant Provide socialization Provide personal care Nail Care: Soaking feet. Trim toenails and fingernails. Apply lotion. This service is for tenants who do not have medical conditions that require licensed staff for nail care, or tenants who receive Bathing Assistance Level 2 or 3. Diabetic care is provided by a licensed staff Skin Care: May include applying lotion daily or other skin care as ordered by physician or requested by the tenant. Oxygen Management: Ordering O2 as needed. Assistance with management of equipment. Ordering supplies as needed. Filling portable oxygen if needed. Changing tubing monthly. Washing filter weekly. Monitoring liters ordered. Consulting with physician with any problems or compliance.

Concentrator Use: Fee for additional electricity associated with using concentrator in apartment. Pre-Post Cataract Surgery: Daily administration of eye drops as ordered by physician. Ordering drops as needed. Monitoring for abnormalities in eye. Correspondence with physician. Nebulizers Treatment: Daily monitoring and cleaning of equipment. Ordering of supplies as needed. Daily administration of nebulizer treatment. Documentation of administration. Enhanced Assisted Living Respite Care: Available to spouses of AL tenants with memory care. Staff will provide respite care per hour up to 7 hours a day in the memory care unit In room dining: Per meal charge for meal delivery to apartment. Includes delivery and pick-up of meal tray. Laundry Services: Staff washes 3 loads of laundry a week. Staff washes, folds and puts clothing away. Tenant may have additional loads washed at an additional charge Daily bed making Making bed daily
