EMAIL QUESTIONS RECEIVED 1. Per page 7 under Mandatory Qualifications: Is it required that we are an approved city vendor with San Francisco at the time proposals are submitted or can we process this after the award(s) are announced? Will our CA Good Standing Certificate and business license be suffice? Answer: As the Minimum Qualifications states, it is required that applicants are an approved city vendor at the time proposals are submitted and must be a validly existing legal entity qualified to do business with the City and County of San Francisco. The CA Good Standing Certificate and business license by itself is not sufficient. 2. Can you please provide the current incumbent vendors providing PT, OT, SLP Services? Answer: The current contractors are: On Call Therapists; Preferred Healthcare Registry and Supplemental Health Care 3. Can you please provide the current incumbent hourly bill rates for PT, OT, SLP Services? Answer: it varies. Approximately, between $65 - $72 per hour 4. What was the total annual expenditure of the current contract in 2014 and YTD 2015?, 5. Approximately how many hours per week of OTs, COTAs, PTs, PTAs, and SLP services will be needed? Answer: Variable, according to need. 6. We submitted an RFP back in 2011 (RFP 5-2011) and we were not selected. Do we still need to submit an RFP for RFQ 17-2015? Answer: Applicants that were selected under RFP 5-2011 or RFP 16-2011 do not need to submit an application. However, if you were not selected under those RFPs, you may apply to this RFQ 17-2015 7. Do we need to have a JCAHO Certification in order to submit an RFP? Answer: YES 8. What is the anticipated award date? Answer: August 2015 9. How many OT, PT, PTA or SLP s do you foresee needing? Answer: To be determined according to need, about 1-5 10. Do you provide the diagnostic equipment, tests, evaluation tools, materials and computers/laptops to be used to provide the therapy services? If so, can you please list what kind of equipment and materials will be provided (For Example: computers, internet access, instructional materials, office supplies, fax/copy machine, etc )? Answer: All equipment needed to perform job duties is provided. 11. Do you intend to award to a single or multiple vendors? Answer: single award 12. Would you pay for mileage travelling between buildings on the same day? We normally charge based on the IRS standard mileage rate. 13. Would you consider certified therapy assistants, SLP-CFY or SLPA? Answer: no 14. Are resumes required at the time of proposal submission or only upon award? Answer: yes 15. What would be the payment terms of the San Francisco Health Network (Net 30)? 16. In the event that the San Francisco Health Network will like to cancel the contract for convenience, will the contract office provide the awarded vendor with a 30 days out termination written notice? Answer: Yes 17. What are the Required Documents for the Therapists to start working at the San Francisco Health Network? For Example: Fingerprint, State License, NPI Number, CPR Card, Criminal Background Check, TB Test, any other specific Certifications. Answer: see below MANDATORY TUBERCULOSIS (TB) SCREENING for the protection of patients: Documentation of two negative TB skin tests placed at least one week apart: both placed within the past year and at least one placed within the past 3 months before starting work at SFGH. OR Those with a new or previously positive TB skin test must have a chest x-ray and a complete evaluation by a health care provider for possible treatment and medical clearance within the past year before starting their clinical experience at SFGH. All documentation of TB skin tests must be on letterhead and include the date of placement, the date of reading, and the degree of in duration in millimeters (mm) and signed and dated by a health care provider. Documentation of chest x-ray and medical evaluation for clearance must be on letterhead, signed and dated by a health care provider. MANDATORY immunity for the protection of patients: RUBELLA (German or 3-day measles) RUBEOLA (measles) MUMPS VARICELLA (Chicken pox) All students/interns are required to demonstrate immunity to these four infectious diseases by providing serologic (blood test) proof of immunity (positive titers) from the laboratory that did the testing. Individuals who are not immune must receive 1-2 doses of vaccine before clearance. Titers should be repeated 1-2 months after completing vaccines to confirm immunity. If the result of a titer is indeterminate or equivocal, applicants should be considered non-immune. If immunity is not proven, the appropriate vaccine must be given to meet these requirements. If the MMR and Varicella vaccines are needed, they should be given at the same time; otherwise they should be given four weeks apart. If a second dose of any of these vaccines is required, it should be given four weeks later. If the MMR vaccine is needed, only one dose is needed for those born before 1957. 18. Can you provide the current incumbent vendor(s) providing services? What is the incumbent vendor(s) current fee rate(s)? Answer: see answer to question 2 & 3 above 19. We are located in the state of Florida and provide services nationwide. However, we do not have an office in CA. Is your preference for a company to have an office in the state CA? Answer: 1 location only 22. What is the productivity ratio? Answer: 7 billed patients per 8 hours worked 23. Who manages Home Health? 24. If Home Health what is the coverage area? 25. Which EMR platform do you currently utilize? Is experience required or preferred? Answer: Seimans Invision LCR and paper 26. What are the shifts/schedules, 8 or 10 hours? Are weekend rotations required? What is the daily patient ratio? Answer: 8 am -4:30 pm 27. Do you require bilingual travelers for any of your locations/facilities? Answer: no 28. Who will conduct the interviews? What are their contact information, direct number, email, and cell? Answer: David Snyder, Senior Physical Therapist. Contact info will be provided for qualified candidates 29. Industry standard is 24-48 hours for interviews after submission of candidate, what is the expectation? Answer: same 30. For "Physical, Occupational Therapists, Physical Therapy Assistants and Speech Language Pathologists - "What is the culture of the department? Who is the final decision maker? RFQ, Section Administration of Therapist Personnel, Page 6.B: Agencies will respond to hospitals within 4 hours of receipt of request please explain definition of respond is this the time frame the agency has to acknowledge request(s) of personnel. What will happen if there is no response from the agency? Answer: Need will be presented to another contractor 32. RFQ, Section Administration of Therapist Personnel, Page 6.D: Agencies will provide hospitals with resume, license and license verification for hospitals to review within 5 days of request what will happen if we present candidates outside of the 5 day window? 33. RFQ, Section Cancellation of Service Request for Therapist Personnel, Page 6.A: 5 day cancellation clause? I have concerns with those personnel who have confirmed assignments and are housed by our company. Can we negotiate for a 2 week cancellation? Answer: This may be negotiated with qualified candidates. 34. RFQ, Section Cancellation of Service Request for Therapist Personnel, Page 6.C: If therapist refuses an assignment in a clinical area, hospital will be credited for 8 hours of pay at prevailing rate, if replaced within 2 hours of shift - this is applicable for LOCAL due to turn around time of replacement, correct? Answer: need further discussion. Unable to answer at this time.
