Purpose: this message solicits applications from qualified active duty and reserve enlisted personnel for the FY19 Veterinary Corps Food Safety Warrant Officer Training Program. Command support in disseminating this message to assure timely submission of applicants is appreciated. Application deadline is 17 July 2018. 3. Selection process: HQ USAREC RCHS-SVD will convene a competitive selection board on or about 7 August 2018. All applications will be considered based on individual merit and documentation as specified in paragraph 10. Applicants in

Page 2 of 7 OCONUS assignments selected for this program will have their Foreign Service tour curtailed to meet training report dates. Individuals will receive official notification of selection following the board. 4. Program overview: applicants selected for this program will attend the Warrant Officer Candidate School (WOCS) at Ft. Rucker, AL prior to attending the Veterinary Corps Food Safety Warrant Officer Basic Course (WOBC) at Ft. Sam Houston, TX. Active Component (AC) candidates will be appointed as a Warrant Officer One (WO1) in the U.S. Army Reserve (USAR) with concurrent call to active duty and will incur a six (6) year Active Duty Service Obligation (ADSO) upon graduation from the Veterinary Corps Food Safety Officer WOBC. MOS 640A will be awarded upon completion of WOBC at Ft. Sam Houston, TX. 5. Sam Houston, TX course # 6G-640A/ class #191-19 6. Basic Prerequisites: a. Must meet general eligibility requirements of AR 135-100. b. Must hold the 68R or 68S Army PMOS for a minimum of five (5) years and no more than 12 years. The minimum and maximum service date calculation is based upon the graduation date at which the 68R or 68S PMOS was awarded. All applicants, regardless of MOS, must have documented experience indicating performance and/or supervision in food protection, sanitation, food defense and/or food inspection tasks. Non-68R personnel who apply based on military experience must have NCOERs which reflect experience in food protection and sanitation tasks relevant to the 640A MOS. MOS waivers will only be considered based on paragraph 6.f. of this message. c. Hold rank of SGT (P)/E5 (P) or higher and be graduates of the Advanced Leaders Course (ALC). Applicants currently enrolled in ALC must graduate NLT 13 April 2018 unless a waiver for ALC is granted. d. All applicants will have a minimum of an associate s degree from an accredited college or university. The degree will, at a minimum, show successful completion of a college level writing course. The degree completion will be documented on an official college/university transcript. For those applicants currently enrolled in a degree completion program, official transcripts must be provided to USAREC by packet deadline 17 July 2018.

Page 3 of 7 e. Have a GT score of 110 or higher. f. Active duty applicants must have a minimum of 66 months and no more than 144 months of active federal military service as of the date of application. Time in service or time in MOS waivers will not be considered except for the following: (1) Civilian education waivers for PMOS 68R or 68S: qualified applicants who hold the 68R or 68S MOS may request a waiver for minimum rank/grade, ALC and minimum time in service requirements if they have a BS/MS degree from an accredited U.S. university that included at least 30 semester hours in one or a combination of the following: Biological Sciences, Food Technology/Science, Epidemiology, Public Health, or related scientific fields that provided knowledge directly related to the 640A MOS. These waivers will be routed through the VC Food Safety Officer Consultant (640A) and will be considered provided the applicant has completed their initial service obligation, has reenlisted and has held the 68R or 68S PMOS for at least three years. (2) Civilian education waivers for non-68r/68s: Applicants with a BS/MS degree that included at least 30 semester hours in one or a combination of the following: Biological Sciences, Food Technology/Science, Epidemiology, Public Health, or related scientific fields that provided knowledge directly related to the 640A MOS, may also apply for a waiver provided they have at least three years of documented experience as a Civilian Food Inspector or Sanitarian. These waivers are subject to final approval by the Chief, U.S. Army Veterinary Corps. (3) 68R and 68S NCO applicants with prior service in another MOS may request a waiver for minimum time in MOS to no less than 48 months or a maximum time in service (TIS) waiver if exceeding 144 months of Active Federal Service. Waivers must be routed through the VC Food Safety Officer (640A) Consultant, subject to final approval by the Chief, U.S. Army Veterinary Corps. Maximum TIS waivers may only be requested by those exceeding 144 months with special circumstances, such as detailed duty as a recruiter or drill sergeant which delayed the applicant s eligibility to apply for Warrant Officer MOS 640A. There will be no other waivers for AC Soldiers with less than 66 months or exceeding 144 months of Active Federal Service. (4) Applicants requiring any type of waiver will contact the 640A Consultant or Senior Field Grade 640A Warrant Officer POC directly prior to application submission for additional information. Contact information is located in paragraph 12. All waiver requests are due NLT 19 June 2018 and must be reviewed by the VC Food Safety Officer (640A) Consultant before approval by the Chief, U.S. Army Veterinary Corps. g. Special notes for U.S. Army Reserve (USAR) applicants: (1) Maximum time in service requirements for USAR applicants are not applicable. (2) Waivers for minimum requirements for time in service, MOS and ALC will be considered with an endorsement memorandum from the first VC or AMEDD/Public Health LTC/O-5 within the applicant s chain of command based upon civilian

Page 4 of 7 education, civilian experience/credentials and/or deployment experience. (3) Advanced Leaders Course (ALC) may be waived for any USAR applicant who has at least one deployment as a 68R/68S NCO in which food inspection and food protection/sanitation duties are documented on NCOERs for the deployment period. (4) USAR applicants with a BS/MS degree from an accredited university that included at least 30 semester hours in one or a combination of the following: Biological Sciences, Food Technology/Science, Epidemiology, Public Health, or related scientific fields that provided knowledge directly related to the 640A MOS, may also be exempt from the ALC and time in service requirements so long as they have documented experience in the food protection field. (5) USAR Soldiers must include an endorsement from their Unit Commander or Senior Veterinary Corps Officer which supports their technical qualifications to serve as a Food Safety Officer. (6) All waiver requests are due NLT 19 June 2018 and must be reviewed by the VC Food Safety Officer (640A) Consultant before approval by the Chief, U.S. Army Veterinary Corps. Meet physical requirements for Warrant Officer appointment as prescribed in chapter 2, AR 40-501, and be within weight standards prescribed by AR 600-9. Applicants must be able to pass the standard event APFT which includes the sit-up, push-up and two mile run. Applicants will submit their last three RAPFT Scorecards (DA 705) for verification. Certain Soldiers with injuries that occurred under extraordinary circumstances may request an exception to policy for APFT with the 2.5 mile walk as an alternate event for the 2 mile run. These requests must be approved by G3, HQDA prior to submitting an application for this board. 7. Basic prerequisites listed in paragraph 6 are non-waiverable unless specified above. 8. Applicant submission: All Active Duty applicants must scan and e-mail packets to the Veterinary Corps Program Manager at the following address: usarmy.knox.usarec.list.hq-hsd-640a-vc3@mail.mil. Packets must contain the following subject line: FY19 640A Application: Last Name, First Name and Middle Initial. All USAR applications, send to: ACRD Special Missions (MSG Edward Campos), 85 Chamberlin Street, Fort Knox, Kentucky 40121-5107. All documents should be placed in a manila folder in sequence according to this message. Do not bind, place in plastic protectors or punch holes in documents. Applications postmarked after 17 July 2018 will be returned to the originator without action.

Application special instructions: refer to the USAREC Warrant Officer recruiting website (http://www.usarec.army.mil/hq/warrant/index.shtml), and follow the steps exactly as written below to ensure the correct Warrant Officer AMEDD appointment application checklist is used. Do not use the non-amedd USAREC Warrant Officer application. a. On the left column of the page select the Warrant Officer MOS List quick link. Click on Warrant Officer MOS list tab. b. Scroll down to the 640A Veterinary Services Food Safety Officer MOS. Click any of the entries on the 640A line. c. Select the WO 640A Checklist. Only use this checklist. 10. Application forms and samples can be downloaded from http://www.usarec.army.mil/hq/warrant/wopdownload_forms.shtml. Application documents must include: a. Warrant Officer AMEDD appointment application checklist. Submit the number of originals and copies as indicated on the checklist. b. Basic application memorandum. Memo must include: (1) Statement: "IAW MILPER Message xx-xxx (DTG of MSG) Subject: FY19 Warrant Officer Applications for Active Duty and Reserve Veterinary Corps (VC) Food Safety Officer (MOS 640A), I hereby make application for the Veterinary Corps Food Safety Officer Training Program. Upon successful completion of the Warrant Officer Candidate School, I will, if tendered, accept appointment as a Warrant Officer in the USAR with concurrent call to active duty as described in said MSG." This last statement does not apply to USAR applicants. (2) Endorsement by current Unit Commander recommending approval/disapproval of the applicant for appointment as a Warrant Officer Candidate does not count as a letter of recommendation as listed in paragraph 10.l. below. c. DA Form 160 (Application for Active Duty). For Active Duty applicants only. d. Statement from Military Personnel Service Center verifying that favorable local records check has been made to ensure applicant is administratively qualified for admission to course and appointment as a Warrant Officer and that application is not in contravention to AR 600-8-2. e. Statement from Unit Security Office that the applicant has a secret clearance or has initiated or completed National Agency Check and Local Agency Check (NAC/LAC), as applicable. Applicants who do not have a secret (or higher) clearance must sign a statement indicating their understanding that, if they are selected as a candidate before completion of NAC/LAC, they may not be enrolled in the program if

Page 6 of 7 found to be ineligible for appointment as a Warrant Officer. f. DA Form 3575, certificate of acknowledgment and understanding of service requirements for individuals for appointment in the USAR under the provisions of AR 135-100 or AR 135-101, as applicable. g. Original DD form 2808 (Report of Medical Examination), DD form 2807-1 (Report of Medical History), HIV test and drug and alcohol test (DAT), completed on/after 14 May 2018. The reviewing medical authority must indicate that the applicant meets the medical standards for qualification and appointment as a Warrant Officer under chapter 2, AR 40-501. h. DA Form 61 (application for appointment). APFT height and weight statement should be in item 41, IAW AR 135-100, paragraph 2-1. Statement must be certified by Company Commander. i. Resume: Resume should briefly depict military assignments pertinent to MOS 640A and any military training and/or civilian education to qualify him/her for favorable consideration as a candidate for the Veterinary Corps Food Safety Warrant Officer. Resume must include DoD email address. Utilize USAREC Warrant Officer Resume. j. Official DA military photograph taken within past 12 months. k. College transcripts: collegiate studies listed on resume and/or enlisted record brief (ERB), must be documented by transcripts that bear the official stamp/seal of the institution. Transcripts must be sent directly from issuing institution to the address listed in paragraph 8. Transcripts must be received by packet deadline, 17 July 2018. l. Letters of recommendation: One (1) letter from the applicant s Unit Commander and two (2) additional letters of recommendation that comment on applicant s qualifications and potential for training as a Veterinary Corps Food Safety Officer. Factors for consideration include: duty performance, self-improvement achievements, oral and written communication skills, and applicant s tact, diplomacy, good moral character, and similar quality characteristics that are compatible with the Veterinary Corps Food Safety Officer Training Program. At least one letter of recommendation must be from a Field Grade Officer (VC or Public Health/Environmental Science Officer) or Veterinary Corps Senior Warrant Officer (CW3-CW5) supporting the applicant s relevant food safety experience. The remaining letters of recommendation can be from anyone of the applicant s choosing. Note: Applicants may contact the VC Food Safety Officer (640A) Consultant POC listed in paragraph 12.a. for guidance. Original letters of recommendation will be either mailed directly to HQ, USAREC, RCHS-SVD-VC ATTN: MAJ Dana Y. Gray, 1307 Third Avenue, Fort Knox, KY 40121-2726 or scanned and sent by email directly to the USAREC VC Program Manager, MAJ Dana Y. Gray at usarmy.knox.usarec.list.hq-hsd-640a-vc3@mail.mil. Letters of recommendation from active military members will be submitted on a USAREC Form 3.3 and will include a digital signature.

Page 7 of 7 m. Soldier record brief (SRB). Local military personnel service centers will review each reproduction for accuracy and legibility and will certify them as true copies. The following statement must be included on the original documents: "I have reviewed my ERB or 2A/2-1. It is current and accurately posted." n. Submit all NCOERs/AERs and any DD 214 or NGB 22 forms contained in official military personnel file. o. Letter of motivation and intent: applicant will personally prepare a single page, hand-written letter, dated and signed, to explain his/her motivation for seeking Veterinary Corps Food Safety Officer Training. This letter should reflect applicant s expectations upon completion of training and being appointed as a Warrant Officer. Applicant will reflect on how their talents and experiences will strengthen the Veterinary Corps. 11. Applicants are responsible for timely submission of application documents and for verifying that all appropriate documents and transcripts have been received by U.S. Army Recruiting Command. 12.
