A Systematic Approach to Consultant Pharmacy Services The Initial Consultant Pharmacist Licensing and Consultant Recertification Course August 18-19th 2017 The Westin Ft Lauderdale 400 Corporate Drive. Fort Lauderdale, FL 33334

Target Audience All pharmacists who wish to become licensed as Consultant Pharmacists Pharmacists who wish Consultant Recertification credit Course Description The course consists of two days of comprehensive study of the laws, regulations and procedures for pharmacists who want to consult in a nursing facility or other work environment that requires the services of a Consultant Pharmacist. You will know what is required of you in an inspection of the facilities and be able to prepare your facility for an inspection. The workbook you will receive will serve as a resource for you when you become a Consultant Pharmacist. On the final day of the course you will take a comprehensive exam on the materials that were covered during the course. After passing the exam with a score of at least 70% and completing your 40 hours under the direction of a Consultant Pharmacist of Record, you will file your application with the State of Florida to become a Consultant Pharmacist. Learning Objectives At the conclusion of the course participants should be able to: List the licensing and proficiency requirements to obtain a consultant pharmacist license. Relate how a policy and procedures manual should be developed and utilized. Describe how to evaluate (review) medication orders in a hospital or nursing home: o For appropriate indication or diagnosis; o For drug interactions with other medication; o For duplication of therapy; appropriate drug dose; o For interval and route of administration State the requirements for handling all drugs including controlled substances. Analyze the source and rationale for State and Federal regulations governing pharmaceutical services in institutional pharmacy settings such as residential care facilities, jails, psychiatric care units and hospitals. The specific teaching goals are: Goals (1) Prepare pharmacists to serve as consultants in nursing homes and hospital pharmacies (2) Provide pharmacists with knowledge of the responsibilities of a consultant pharmacist (3) Prepare pharmacists to develop quality systems for assuring safe drug delivery and patient monitoring (4) Teach pharmacist readiness for AHCA inspections

The fee is as follows (meals, snacks, travel and lodging are not included): $420 Initial Certification $320 Consultant Recertification Cancellation and Refund Policy All cancellation requests must be in writing. Requests must be emailed to cesupport@ahc.ufl.edu Dates for refunds are listed below. On or before July 18th, 2017 Registration fee less $25 Processing Fee. On or before August 4th, 2017 Registration fee less $50 Processing Fee. After August 4th, 2017 No refund. Refunds will not be processed due to meeting costs incurred by late cancellation. The course attendance is limited and it may be difficult or impossible to fill your spot at this late date. There are no refunds for no-shows. In the event the program is cancelled, registration fees will be refunded in full, but nonrefundable travel-related expenses are not the responsibility of the College and cannot be refunded. Continuing Education Credit This course serves the July 1, 1983 revised Consultant Pharmacist Education Requirements in BOP Rule 64B16. Educational requirements include completion of a post-test at a 70% proficiency level and completing the evaluation. When used to apply for a Consultant Pharmacist License, the Initial Consultant Licensing Course cannot be used for CE Credit. However, it is accredited for 16 hours (2.0 CEUs) of General CPE Credit for pharmacists that do not wish to use the credit to apply for a Consultant Pharmacist License. The course is also accredited for 12 hours of Consultant Pharmacist Recertification credit. Consultant Recertification requires attendance only Friday and Saturday and completing the evaluation. University of Florida College of Pharmacy will submit CPE credit to CE broker and CPE Monitor within 2 weeks following completion of the course.
