Applicants applying for ST4 posts in paediatrics may use this certificate to successful, satisfactory completion of Level 1 paediatric competences, as defined in the RCPCH Level 1 Paediatrics and Child Health curriculum. It is NOT required if you: A) Are currently in a UK approved paediatric training programme and are on track to obtain a satisfactory ARCP outcome (You will be required to provide evidence of this prior to commencing an ST4 post). B) Have completed Level 1 training in an approved UK training programme, within 44 months of August 2018, and have evidence of satisfactory ARCP outcome. 1 If you are provide the above evidence then you will be required to provide a fully completed Certificate of Completion of Paediatric Level 1 Competency. No other form of evidence will be considered. Please note: If you are not currently in a UK approved paediatric training programme you will be required to provide evidence of achievement of Level 1 competences at the time of application. Failure to do so will result in your application being rejected. Instructions to applicants: 1. This certificate can only be signed by a consultant or equivalent 2. 2. Consultants should only sign this certificate if they have worked with you for a minimum of three months whole-time equivalent within 3.5 years prior to the start date of the post you are applying for 3. You will not be eligible to be considered further for ST4 paediatric training in this recruitment round if the form is incomplete, or if the person signing it for you fails to complete fully the section about themselves 4. You must be rated Demonstrated to completion of Level 1 for each and every competence listed on this certificate. If you cannot that you have achieved all your competences in one post, you may submit additional Certificates to the full set of competences. If you cannot each and every competence, you will not be eligible for ST4 paediatric training. 5. Before you pass the form to the signatory, please complete and sign the declaration below 6. It is recommended that you check the form after they have completed it, using the attached checklist 7. You must then scan, upload and attach to your application form on the final page of the application before submission. In order to process their application, we need to know that they have achieved the competences listed in this certificate to the standard expected in accordance with completion of Level 1 paediatric training, as detailed in the RCPCH Paediatric Curriculum for postgraduate training. Dealing with complaints 5.0 Team working Skills i. Able to work well with colleagues in multi-professional and multidisciplinary teams. ii. Avoid acting in a discriminatory manner towards patients and colleagues 7.0 Acute Care 7.1 Core skills in relation to acute illness: i. Promptly assess and manage the acutely ill or collapsed patient ii. Effective leadership of the resuscitation team (may via simulation) iii. Re-assess ill patients appropriately after starting treatment iv. Request senior or more experienced help when appropriate 6

Considers appropriateness of interventions according to family s wishes, severity of illness, and comorbidities 7.2 Demonstrates the knowledge, skills and behaviours to be recognise and treat: i. critically ill patients ii. cardiac arrest iii. shock including early septic shock iv. anaphylaxis v. unconscious patient 7.3 Demonstrates the knowledge, skills and behaviours to be able safely to manage the following presentations when on call: i. acute asthma ii. acute continuing seizures iii. meningitis/encephalitis iv. acute abdomen v. the collapsed neonate 7.4 Competence to manage acute child protection cases: i. Recognise features in the presentation where child protection may be an issue, for example where there are patterns of injury, delay in presentation, and inconsistencies in the history. ii. Understand the need to initiate a safe response where abuse is suspected, while treating the family with respect and courtesy at all times. 7.5 Demonstrates the knowledge, skills and behaviours to be manage the majority of other important acute presentations. 7

Be perform an accurate assessment of the baby at birth ii. Be initiate and lead appropriate resuscitation when required of term and pre-term babies iii. Have the knowledge and skills to be able to assess and initiate management of neonates presenting with: a. early feeding-related dehydration b. respiratory distress c. early signs of neonatal sepsis d. cardiac problems at birth e. surgical problems of the neonatal abdomen f. identify newborns with HIE and initiate cooling appropriately. vii. Be initiate and appropriately manage respiratory support for babies as required, including ncpap and mechanical ventilation 9.0 Care of patients with other conditions and long term conditions i. Be aware of management in the community setting/palliation and appropriate pathways/multi-disciplinary Team (MDT) working 10.0 Knowledge of the NHS and hospital management Understands the structure of the NHS and the management of local healthcare systems in order to be participate fully in managing healthcare provision 8

Know the contraindications and complications of procedures iii. Know the local and national guidelines for obtaining informed consent, for undertaking investigations or procedures and for providing sedation and pain relief for practical procedures iv. Know the anatomical markers for procedures v. Know and practise scrupulous aseptic techniques vi. Be aware of safety issues for patients and staff in relation to investigations of body fluids and radiation vii. Be use all equipment required to undertake common procedures and investigations viii.
