CareTech Awards How to enter Choose the category or categories for which you want to nominate your colleague. If you are entering more than one category separate entry forms must be completed. Write an account of no more than 300 words saying why your colleague should be considered. An additional account from a colleague or service user/family carer, photos or videos is helpful, but not essential. Please make sure that the name of the nominee and the award category is included on all separate pieces of supporting information. Complete the entry form and attach it to all other separate pieces of supporting information. Please post or fax any supporting testimonials and include the name of the nominee and category. Closing dates for entry forms is July, 31st 2015. Post any entries and or supporting testimonials to Sonja Mills, CareTech Care Awards, CareTech Community Services, Metropolitan House, 3 Darkes Lane, Potters Bar, Herts, EN6 1AG or fax to Sonja Mills 01707 655265. Rules for entry The awards are open to all staff, including front line staff as well as office based staff across the UK. All entries are treated confidentially. Entries will be judged by a panel of Caretech Employees, Service Users & Non-Executive Directors. Entries will not be returned. Use one entry form per entry. Photocopies are acceptable. Regional judging will take place on 31st August 2015. Winners of the regional awards are automatically invited to attend the finals. The final will be held at Whittlebury Hall, Northampton on Thursday, December 3rd 2015. Important A statement of up to 300 words saying why you think the nominee should be included.
