Community Health Assessment/State of the County Health Report: Every four years the Iredell County Health Department in conjunction with Iredell County Healthy Carolinians conducts a Community Health Assessment. The Community Health Assessment details the results about leading health concerns among residents. The Community Health Assessment is contractually required. The State of the County Health Report is a document produced every year to provide a local health status update. Public Health Development and Promotion Division continued Iredell County Healthy Carolinians Citizens of Iredell County, working together, promoting preventive health practices to improve the quality of life and health status of all individuals. Coordinates Healthy Carolinians Taskforce Monitors health status of the community Conducts the Local Community Health Assessment Develops Annual State of the County Health Reports Researches statistics for various Iredell County Health Department divisions Coordinates subcommittee work to improve priority health issues. ADMINISTRATION DIVISION This division provides support for the business management functions of health department services. It is also responsible for managing Vital Records. Vital Records Original certificates for deaths and births that occur in Iredell County are received. Certificates are verified for accuracy and then processed. Copies of the certificates are forwarded to the Iredell County Register of Deeds, from whom certified copies may be obtained. Original certificates are forwarded to the State Vital Records Office in Raleigh, where they are recorded and permanently stored. CLINICAL SERVICES DIVISION Provides health screenings, treatments, counseling and education on disease prevention as well as dental care and nutrition services. Case Management services assist clients in identifying and obtaining needed health related services. Child Health Clinic Well child examinations for Iredell County Residents are performed by appropriately trained and licensed providers. Abnormal findings requiring further testing or evaluation are referred to local and regional health care providers. Comprehensive medical, social, developmental and nutritional history. Visit includes age appropriate vision, hearing and developmental testing. Immunizations are reviewed and given according to the NC Immunization Schedule. Lab work for low iron, cholesterol and lipids if at high risk, sickle cell and other screening test are conducted as needed. Insurance, Medicaid and private pay based on a sliding fee scale is accepted. Guidance and education for parents on development, nutrition, discipline and safety. 4 Public Health Development and Promotion Division continued Community Health Education and Information Provides health related education and information to residents: Car Seat Checks Puberty Education Postponing Sexual Involvement Education Alcohol/Drugs/Tobacco Information Breast Self Exam/ Testicular Self Exam Information Health department services and healthy lifestyle promotion via: Attendance at health fairs Partnerships with businesses and organizations Health topic information and awareness Urgent and non-urgent health communication disseminated through the Public Information Officer Social Marketing Health Promotion Provides nutrition education; promotes physical activity and tobacco education/prevention that support environmental and policy change in the community. Heart/Stroke Education Contraception Education Breast Self-Exam STD/HIV Prevention Education Coordinates the Iredell County Eat Smart Move More Coalition. Membership in the coalition is open to all persons interested in improving the health of Iredell County citizens through increased physical activity and healthier eating. 13

Environmental Health Division continued Private Drinking Water Wells Performs site evaluations to determine if well sites meet setbacks from sources of pollution, etc. Issues permits for new well construction, repair and abandonment Performs grouting inspection of new and repaired well during construction phase Performs well head inspection on new or repaired wells for proper seals, vent, etc. Collects samples for bacteriological, inorganic chemical and nitrate/ nitrite analysis on new wells constructed as of July 1, 2008 and forward (included in permit fee) Collects bacteriological, inorganic chemical, volatile organic compounds and pesticide samples for analyses of water from existing private wells upon request and payment of appropriate fees Provides limited consultation on water quality problems Environmental Health Services and On-Site Water Protection Rules http://www.deh.enr.state.nc.us/rules.htm Public Health Development and Promotion Division The division supports healthy lifestyles through providing quality health education programs and services, as well as assessing the need for environmental and policy changes. The division aims to partner with agencies and individuals to develop, implement, monitor, and enhance public health programs for residents. Professionals in this division are trained to communicate public health information, assess the health of the community, advocate for health, strategically plan, manage change, and improve the efficiency and quality of processes, programs, and services. Clinical Services Division continued Care Coordination for Children (CC4C) Provides care management for Iredell County children birth to 5 years of age who are identified as high risk. Priority high risk factors include children with special health care needs, children in the foster care system, and children exposed to toxic stress in early childhood. Collaboration with other agencies to ensure complete and effective care for the child. Communicable Disease Control A communicable disease is an infectious disease that can be transmitted from one individual to another. The Communicable Disease Program promptly investigates disease outbreaks and unusual situations and implements control measures to minimize further transmission of disease. The program also monitors disease reporting by physicians and laboratories in order to detect trends and to assess the public health impact of diseases; provide communication between public health agencies, private physicians, and hospital and occupational infection control personnel, as an essential part of disease control efforts. In addition, public health interventions and information are disseminated to the community and the media in order to enhance disease control efforts. Dental Clinic Clinical Health Information Provides educational information for residents to encourage optimal pregnancies and healthy babies. 12 Healthy Pregnancy Education Newborn Care Education Nutrition Education Provides basic dental services to children and a limited number of adults living in Iredell County. Services are conducted by appointment. Accepts Medicaid, North Carolina Health Choice, some private dental insurances and private pay based on a sliding fee scale. Nitrous Oxide is available in this clinic. 5

Clinical Services Division continued Emergency Preparedness The mission of the Iredell County Public Health Preparedness program is to provide organized and quick responses during disasters and emergencies. Public Health emergencies can include natural events (flooding, tornadoes, hurricanes, etc.) and planned acts (like the use of weapons of mass destruction and bioterrorism). The Emergency Preparedness program provides services such as education, training and information on public health emergencies. ENVIRONMENTAL HEALTH DIVISION The Environmental Health Division functions as the local government arm of the North Carolina Department of Health and Human Services (NCDHHS). As agents of the State, staff provide environmental education as well as enforcement of state laws, rules, and local ordinances pertaining to food sanitation, wastewater disposal, private drinking water wells, and other public health concerns. Food, Lodging and Institutional Sanitation General Services Clinic Provides education and counseling about family planning and maternal health services for women at risk for pregnancy; provides private pay TB skin testing as needed for work and school. Pregnancy testing TB testing Immunizations Reviews plans and issues permits to operate food service facilities, hotels, etc. Conducts routine health/sanitation inspections of all establishments required by law Issues sanitation grades A, B, or C to permitted establishments based upon results of inspections Permits and inspects temporary food service establishments for special events (i.e. festivals, fairs, etc.) Investigates public health complaints Permits and conducts sanitation inspections for public swimming pools and spas Permits and inspects tattoo parlors/artists Provides all required and most recommended vaccinations to infants, children and adults. Some charges may apply. Clinical Services Division continued WIC (Women, Infant and Children) Nutrition Services WIC provides services for women who are pregnant, breast-feeding, or who have just had a baby, and who meet both income and medical and/ or nutritional eligibility requirements. Services are also available to children up to 5 years of age, who meet eligibility requirements. WIC provides food vouchers for nutrient dense foods. Appointments are necessary for all WIC services. Assessment to Determine Eligibility (income and medical/nutrition risks) Nutrition and medical assessment eligibility include: Medical history Anthropometrics weights and heights Lab test Hematocrit screening ( a finger stick is required) Income eligibility adjunctively income eligible if one receives food stamps, Medicaid and/or Work First or meets income criteria based on gross income and number in family Education Services (one-on-one and group education) Age appropriate and condition appropriate nutrition education Elemental and medical formulas for infants, children and women Folic acid education Food storage and safety Immunizations Lead Picky Eaters Anemia Food buying Infant formulas Meal preparation Sudden Infant Death Syndrome (SIDS) Under/overweight, slow weight gain Breast-feeding education Many others Referrals for: Child Service Coordination, Child Health, Dental Clinic, Prenatal Clinic, Family Planning, Medical Nutrition Therapy, Mental Health, Food Banks, Department of Social Services, Partnership for Young Children, Food Stamp Program, Parenting Classes, Childbirth Classes and Immunizations Programs Free breast pump rental Vitamins for breast-feeding infants 10 Clinical Services Division continued Family Planning Provides annual physicals, contraceptive education and services to women of reproductive age. Detailed history, lab work and physical exam Education on birth control methods, breast self-exam, pelvic exams and sexually transmitted diseases Counseling for related health problems Available methods include: Birth Control Pills, Depo Provera, Diaphragm, Spermicidal Foam, Birth Control Patches, IUD s, Condoms and others by prescription Tubal Ligations are available as funding permits An appointment is necessary By ruling of the NC Legislature, teenagers may be seen in clinic without parental consent Insurance, Medicaid and private pay on a sliding fee scale are accepted Pregnancy Care Management (PCM) Provides care management services to Medicaid eligible women of Iredell County to promote healthy pregnancies and positive birth outcomes. Utilize risk screening data and provider referrals to develop strategies to meet the needs of those patients at highest risk for poor pregnancy outcome. Arrange transition from the pregnancy medical home to a primary care medical home for patients who continue to remain and/or become eligible for Medicaid beyond the postpartum period. Migrant and Refugee Health Assists migrant or refugees in locating sources of health care. Reviews records to assure that migrant or refugee is free of communicable disease and appropriately vaccinated. 7

Clinical Services Division continued Prenatal Clinic Clinic established to help women receive proper care during pregnancy. Sickle Cell Program Provides free sickle cell testing for clients who self refer to ICHD for testing, for persons enrolled in health department programs and for those identified by private physicians as well. Testing also provides information concerning the reproductive risks for carriers of Sickle Cell traits or other traits that can affect their offspring; providing clients with the facts they need to make informed decisions regarding reproduction. Free sickle cell testing Free counseling for anyone identified with sickle cell disease or trait Regional counselor for genetic and further testing resources SIDS (Sudden Infant Death Syndrome) Program SIDS Counselors at county health departments have received specialized training and are a resource for caregivers, health professionals, first responders and families who have experienced a SIDS death. History, lab work and physical exam. Also offer counseling, iron and vitamins Maternity Care Coordination by a social worker also available to eligible patients at each visit Prenatal education classes and WIC services are provided at each visit Eligibility for Iredell County residents is determined by household income and number in household Medicaid, insurance and sliding fee scale accepted Services offered in Statesville only An appointment is necessary Provides education for professional and lay groups Counsels families in coping with their sorrow and loss 8 Clinical Services Division continued STD (Sexually Transmitted Disease) Clinic Provides its clients with the most up-to-date assessment and treatment possible for sexually transmitted diseases. Provides testing, diagnosis and treatment for a variety of sexually transmitted diseases Also provides risk reduction and prevention counseling Additional lab tests and education for HIV+ clients as needed Clinic operates daily in Statesville and at different times throughout the week in Mooresville Appointments are necessary By ruling of the NC Legislature, teenagers may be seen in clinic without parental consent No charges for these services HIV Testing Provides HIV testing and pre-test and post-test counseling Appointment needed for testing; HIV results and post-test counseling available without appointment during clinic hours No charge for this service. All services are confidential. Services provided in Statesville and Mooresville Tuberculosis (TB) Program Provides follow-up and treatment for people with TB and their contacts. This is not a service for immigration purposes. Quickly diagnose and treat tuberculosis disease Promptly investigates all suspected exposures to each and every case of TB Strongly encourage preventive treatment for people who are at risk of developing TB disease Provides state-supplied TB medications to clients under case management by the TB program Vasectomy Referral Program Educates and refers men for permanent birth control. Prepares paperwork and provides education to arrange vasectomies for men based on a sliding fee scale Program based upon funding availability 9
