Two (newest) Enrollment Applications (presently ANCS 7001 10-12). Two Fingerprint Cards. One New Member Course Examination answer sheet. (Form ANSC 7010) Coast Guard Auxiliary Association, Inc. Consent to Membership Form
Only the (newest) ANCS used. 7001 (10-12) must be Complete pages 1-5. (Three complete enrollment applications are to be made. Two are sent to DIRAUX and one for new member record.) All signatures must be in blue ink. Fill out the enrollment forms in black ink.
PAGE 1 SECTION IV (Re-enrollments): Please fill the old member number if applicable. Send first page of application if is less than 90 days). New Member Examination: Enter date and score. Submit a copy of the new member exam sheet. Read Privacy Act Statement. BOATING SAFETY COURSE CERIFICATE: check yes or no. (Look Chapter 8-6 for NASBLA approved courses) Note: Only certificates listed in the Auxiliary Manual are acceptable.

The Applicant Interview Record should be used as a check off sheet to make certain the applicant has been informed of the membership opportunities and obligations in the U.S. Coast Guard Auxiliary. Membership eligibility begins at 17 years of age. Must be U.S. Citizen or Naturalized Citizen. Section VI To be completed by applicant's parent or guardian. Applicants who are 17 must have at least one parent or guardian complete this section. Sign and date in blue ink
SECTION VII Applicant Statement and Signature 1. Is the New Member currently serving as a member of the U.S. Armed Forces? Yes or If Yes indicate branch. 2. Have you served as member of the U.S. Armed Forces in the past? Yes or no If yes, indicate branch. You must attach to the applications a copy of the DD-214 showing Reenlistment Code blocks.
If the New Member does not have a DD-214 at the time of enrollment. Attach a letter to the New Member Application that he/she has requested a replacement DD- 214. If the answer is yes the following must be done. Attach to this application a statement of specifics including date, city and state offense occurred, disposition, and any comments, including mitigating circumstances, along with a copy of his court documents. Applicant may submit his statement in a sealed envelope for the DDSL s eyes only. If SECCEN needs more information applicant will be contacted.
Applicant should sign in blue ink after reading the following: I affirm under the penalties of perjury as to the truth of all statements contained in this application and authorize verification for the official use of the U. The applicant must attest to U.S. citizenship by birth or by naturalization. The birth certificate or passport (or other forms of identification as listed in section IX ) must be seen by you to verify citizenship. The Citizenship Verifier, the Fingerprint Technician or Law Enforcement Officer may verify the citizenship by reviewing the document to ensure it is genuine.
A copy of the birth certificate, passport (current or expired) or whatever document is provided for proof of citizenship, must be photocopied and attached to New Member applications. Individuals who hold dual citizenship with the U.S. and another country may become members of the Auxiliary. Section B must be completed by an Auxiliary Fingerprint Technician or Citizenship Verifier (CV) or by a Law Enforcement Officer.
SECTION X PRIOR/CURRENT CLEARANCES I have or have not been issued a security clearance by a federal agency with in the past ten (10 ) years. If yes see instructions and complete 6, 8 & 9. To print page 6, 7, 8, and 9, Yes must be check in Section X. (Security Clearance must be within 10 years) Enter any prior/current clearances, if any and attach the source document
PERSONNEL SECURITY INVESTIGATION STATEMENT Agreement to undergo the requisite Personnel Security Investigation (PSI) is not a guarantee of membership An unfavorable PSI determination may result in your disenrollment despite any training, duties, or activities you may have performed and/or personal investments in time, effort, resources you may have expended as part of the Auxiliary. (Recommendation -- have the New Member initial the side of paragraph indicating statement was read and understood.)

PRIOR/CURRENT INVESTIGATION INFORMATION COVER SHEET SECTION X - Page 6 of 16 The investigation must have been within past ten years. The SF 86C (pages 8 & 9) must be completed, signed and attached to this cover sheet. Type or legibly print answers in ink ( if this form is not legible, it will not be accepted).
SF 86 Page 8 Fill out block 1 Identification --Complete this form referencing information contained in your most recent SF 86 or information disclosed upon the date of your last background investigation. All questions on this form must be answer. Block 2 Questions from the SF 86--Do not provide information you have already provided on your most recent SF 86. Any yes responses under block 2 must be explained in Block 3. If additional space is needed use blank sheet of paper. Each blank sheet of paper you use must contain your name and SSN at the top of the page. If you have any questions contact the DDSL/ADDSL.
Standard Form 86 Certification (SF-86C) I certify that the information includes all changes to my most recent SF 86 or since my last investigation. The united States Criminal Code (title 18, section 1001 provides that knowingly falsifying or concealing a material fact is a felony which may result in fines and/or up to 5 years of imprisonment
Completed New Member Enrollment Package 1. Two completed ANCS 7001 (10-12) Forms 2. Complete pages 1-5 or 1-9 (Security Issues). (two complete sets) All signatures must be in blue ink -- Enrollment forms in black ink. 3. One copy of the scored New Member Exam Answer Sheet (Form 7010). 4. Two copies of the document used for proof of United States citizenship Two copies of the birth certificate or United States Passport (Current or expired or other forms of identification listed in Section IX.)
Completed New Member Enrollment Package 5. Two copies of the DD 214. Two fingerprint cards (black ink only) 7. Copy of the Boating Safety Course Certificate must be submitted if the new member has taken an American Boating Course or Boating Safety and Seamanship Course. 8. Photo of the new prospective member either as a paper photo or digital on a CD. The photograph of the prospective New Member must have a red background and wearing a light blue shirt. Include the name of the individual and Flotilla number on the photo or on the CD. 9. A will not be accepted floppy.
Completed New Member Enrollment Package 10. Use of staples on an application is not acceptable. No Loose-leaf holed applications will be accepted. 10. Applications should be printed on one side only. Two-sided copies are not acceptable. ALL new member packages must be submitted to FC for final signature and forwarded to the DDSL/ADDSL within the Division via hand delivery or certified mail. Once the packet is submitted to the FC, the HR Department is no longer responsible for the membership. The Security Department (ADDSL/DDSL) is now responsible for the processing.
Once the member package is sent to the FC, the HR Department is no longer responsible for the processing. The process is now the responsibility of the Security Department. After a month Member listed as AP Status and Flotilla Commander did not receive the new Member Welcome Aboard Package and Certificate an entry should be placed in the D7 Help Desk at http://d7help.cgau.org/ using New Enrollment Category
