Marathon County 2018 Employee Wellness Program

Welcome to the 2018 Employee Wellness Program Marathon County is excited to offer our employees an opportunity to participate in our 2018 Employee Wellness Program! This program is available to all employees in regular allocated positions. You are encouraged to take advantage of these services and, by completing all program requirements, you will be eligible for great incentives! Health plan participants may be eligible for up to $75 in gift cards, 3% reduction of the employee contribution of 2019 s monthly health insurance premiums, and $120 gym membership reimbursement when meeting wellness program requirements. Nonhealth plan participants will be eligible for up to $75 in gift cards and a $120 gym membership reimbursement when meeting wellness program requirements. Most importantly, you can learn about and make improvements to your personal health and well-being! 2018 s WELLNESS FOCUS In the 2017 Employee Wellness Program, Aspirus Business Health reviewed the biometric and health assessment results to find that Marathon County s top 3 risk factors for the population are weight management (75%), stress (57%), and nutrition (57%). This year s wellness program will have wellness events, activities, and challenges that focus on those areas to help employees reduce their risk. Marathon County cares about the health of their employees and will continue in 2018 to work with Group Health Trust and Aspirus Business Health to work towards being the safest, healthiest and most prosperous in the state. WHAT IS DIFFERENT IN THE 2018 EMPLOYEE WELLNESS PROGRAM? Our 2018 Employee Wellness Program will consist of a few new features that will allow you to participate in more wellness activities and earn more wellness incentives. You will still be required to complete a health assessment, biometric screening and meet with our Health Coach for two health coach sessions to set your wellness goals. Additionally, you can choose to participate in more wellness challenges, events, and activities based on your interest to earn you additional incentives. WHAT DOES OUR HEALTH COACH DO? Our Health Coach is a wellness specialist and supportive mentor who encourages individuals to cultivate positive health choices. She is a resource to help educate and support employees in achieving their individual health goals through lifestyle and behavior changes. Our health coach is a confidential resource for all employees. YOUR ASPIRUS MANAGEWELL WELLNESS PORTAL In 2018, you will continue to have access to your own personal and confidential wellness portal. This is where most of the 2018 wellness program will be completed. All wellness program requirements will be available and tracked on this system. This includes the ability to schedule appointments (biometric screenings and health coaching sessions), complete your health assessment, and register, track and submit information for wellness challenges/educational activities. You will also have access to your wellness results, goals, and achievements - all of which are confidential between you and Aspirus Business Health. HOW TO REGISTER FOR THE ASPIRUS WELLNESS PORTAL? All employees who did not participate in the 2017 Employee Wellness Program must register in the wellness portal. Please keep an eye out for the Wellness Program registration emails to sign up. Feel free to contact your health coach with any questions on registering for the wellness portal. HOW TO QUALIFY FOR WELLNESS INCENTIVES Marathon County Health Plan Employees To qualify for the 3% premium reduction of the employee contribution of 2019 s health insurance premiums, you must complete all requirements of the BRONZE package by the identified program deadline dates, to earn 1,000 incentive points. To qualify for the additional $50 or $75 gift card, the BRONZE package must be completed plus a combination of SILVER or GOLD package activities totaling 2,000 or 3,000 total points completed by October 31, 2018. Marathon County Non-Health Plan Employees To qualify for the $25 gift card, you must complete all requirements of the BRONZE package by the identified program deadline dates, to earn 1,000 incentive points. To qualify for the additional $50 or $75 gift card, the BRONZE package must be completed plus a combination of SILVER or GOLD package activities totaling 2,000 or 3,000 points completed by October 31, 2018. See next page for details about the Bronze, Silver and Gold package incentives. 2

2018 Employee Wellness Program Marathon County Employee Incentive Packages HOW DO I PARTICIPATE AND EARN INCENTIVES? Marathon County s wellness program will start in January, 2018. The program is available to all employees in regular allocated positions. There are three packages of participation; Bronze, Silver and Gold for the 2018 program. Each package offers a different incentive available to you. Bronze Package: 1000 Points Required activities to be eligible for incentives. Required Activities: Biometrics... 250 points Health Assessment... 250 points Health Coaching 1... 250 points Health Coaching 2... 250 points Incentives: Health Plan Subscribers...3% Insurance Premium Reduction in 2019 Non-Health Plan Subscribers... $25 Gift Card Additional Wellness Incentive Rewards Program The Bronze Package must be completed to earn points toward additional incentives. Please note that employees can only earn Silver or Gold not both of the additional incentives. Pick any activity offered in this section to accumulate points to earn the Silver or Gold incentives. Must be a Marathon County or Group Health Trust (GHT) Sponsored Activity. 500 points each Annual Medical Exam (ONE PER YEAR) Wellness Lunch and Learns Wellness Challenges 250 points each Annual Eye Exam (ONE PER YEAR) Annual Dental Exam (ONE PER YEAR) 100 points each Online Educational Activities Completed Weight Watchers 12-17 week Session (ON-SITE SESSION ONLY) Group Fitness Class (PER 8 CLASS SESSION) Health Fair Wellness Events Flu Shot (ON-SITE AT MARATHON COUNTY ONLY) 50 points each Wellness Hour Activities Group Health Trust E-bites (SEE NEXT PAGE FOR MORE DETAILS) Silver Package: 2000 Points Health Plan & Non-Health Plan subscribers: $50 Gift Card * OR (CANNOT RECEIVE BOTH SILVER AND GOLD PACKAGES) Gold Package: 3000 Points Health Plan & Non-Health Plan subscribers: $75 Gift Card * + $120 Gym Membership Reimbursement* *Must be a Marathon County Employee and an active member of a fitness center between 1/1/2018-10/31/2018 to receive incentive. Must be an active employee on 12/31/2018 to receive any wellness program incentives. Gift cards and gym reimbursements are subject to taxation and will be provided January-February 2019. 3

How to Achieve Your Points Bronze Package Activities Silver Package Activities Gold Package Activities Biometric Screenings 250 Points Height, Weight, BMI, and Blood Pressure Fasting Blood Draw: - Total Cholesterol - HDL (High-Density Lipoprotein) - LDL (Low-Density Lipoprotein) - Triglycerides - Glucose Please do not eat or drink for 12 hours before your scheduled appointment water and black decaf coffee is ok. Please take any medications as prescribed. Make sure to drink plenty of water because staying hydrated will make for an easier lab draw. If you have had all of the above measurements taken since 10/1/2017 with your primary care provider, you can submit those results to the Health Coach instead of fasting again. Biometric screenings must be completed or turned in between January 1st -March 24th, 2018 to be eligible for incentives. Biometric screening vouchers may be given to employees in outlying areas and/or with special circumstances with approval from health coach. Health Assessment 250 Points Filling out your health assessment online in your personal wellness portal. Please have your health assessment completed between April 1st-April 30th, 2018 to be eligible for incentives. Health Coaching 250 Points Each Complete your first health coaching session to discuss your biometric screening results and wellness goals for the next 3-6 months between May 1st-July 31st, 2018. Complete your second health coaching session to discuss progress and or challenges of your wellness goal between August 1st-October 31st, 2018. Participants must allow 60 days between your first and second health coaching session. *Your health assessment, biometric screens, and health coaching sessions are confidential and will only be accessible through your Aspirus ManageWell Portal and Aspirus Business Health. You may attend your biometric screening and your Health Coach meeting during your regular work schedule with supervisor approval. Comp time and overtime is not permitted for any meeting attendance outside of normal work hours. Telephonic health coaching sessions are available for outlying areas and special circumstances. These must be approved by health coach before they can be scheduled. Pick any activity offered in this section to accumulate points to earn the Silver or Gold incentives. All activities must be a Marathon County or Group Health Trust (GHT) Sponsored Program. 500 Points Each Annual Medical Exam: complete your annual medical exam and turn in proof of the exam to the health coach with the preventative form on the wellness portal, an EOB statement, or visit summary. Only one per year will be accepted. Wellness Lunch and Learns: Attend a wellness lunch and learn to learn about a wellness topic. Watch for upcoming dates for these activities. Wellness Challenges: Complete a 4-8 week wellness challenge sponsored by Marathon County or Group Health Trust. 250 Points Each Annual Eye Exam: Complete your annual eye exam and turn in proof of the exam to the health coach with the preventative form on the wellness portal, an EOB statement, or visit summary. Only one per year will be accepted. Annual Dental Exam: Complete your annual dental exam and turn in proof of the exam to the health coach with the preventative form on the wellness portal, an EOB statement, or visit summary. Only one per year will be accepted. 100 Points Each Online Educational Activities: Find these on the wellness portal throughout the wellness program year. Read educational handouts for the wellness topic and take the online survey to receive points. Watch for upcoming dates for these activities. Weight Watchers 12-17 Week Session: Successfully complete on-site weight watchers session by only missing 2 classes. Your completion will then be uploaded automatically into your wellness portal by the health coach. Please make sure to sign in at every class. Group Fitness Class Session: Complete 6 out of 8 of the fitness classes of each session offered to Marathon County Employees to earn points. Your completion will then be uploaded automatically by the health coach. Please make sure to sign in. Watch for upcoming dates for these activities. Health Fair: Attend the Marathon County Health Fair and sign in to receive points. Wellness Events: Attend a wellness event put on by Marathon County and sign in to receive points. These can be anything from hikes, group skiing, snowshoing, etc. Watch for upcoming dates for these activities. Flu Shot: Get your flu shot at a Marathon County on-site location and sign in to receive points. 50 Points Each Health Coach Educational Check In: In addition to your two health coaching sessions, visit with your health coach to check in on wellness goals, weigh in, get a blood pressure check, etc. These activities will be held during the various Marathon County on-site wellness hours. Group Health Trust E-bites: To complete the Group Health Trust E-bites activity, please read through the information sent through email or posted in your department to answer the questions in the wellness portal. All activities are subject to change. Points will only be awarded if the activity was successfully completed. Watch for upcoming dates of the activities above. 4

Your Health Is Important We are proud to offer this wellness initiative and will continue to build onto the wellness program to provide further opportunities for employees to incorporate wellness into their daily lives at home and at work! We hope that you will seriously consider participating in the 2018 Employee Wellness Program. Note: Your personal Health Assessment, biometric screening results, and personal health coaching conversations are confidential between you and Aspirus Business Health and will not be shared with Marathon County. Aggregate results of the entire program will be available to Marathon County and will be used to develop on-going health and wellness initiatives. Your participation of this Program and Program components will be reported to Marathon County by Aspirus Business Health for purposes of incentive distributions. Your personal health assessment, biometric results, and/or your choice to participate or not in this wellness program will not impact your future health insurance benefits. YOUR HEALTH IS IMPORTANT! Join your co-workers by learning about your health risks and how you can promote a healthy lifestyle. Complete the Bronze Package in 2018 and qualify for a 3% health insurance premium reduction in 2019. Contribute to making our County the safest, healthiest and most prosperous in the State! EMPLOYEE GYM MEMBERSHIP REIMBURSEMENTS To earn the gym membership reimbursement, you must complete the activities for the Gold package and turn in proof of membership to our health coach by 10/31/2018. Accepted proof of gym membership includes: Receipt of employee membership payment, employee attendance printout, or employee membership printout. Proof must be dated between 1/1/2018-10/31/2018 to receive the incentive. Gym memberships must be in Employees name to qualify for this incentive.
