Swain County Employee Worksite Wellness Program Starts January 12 th after Pre-Wellness Screenings in partnership with Swain Community Hospital 2016/2017 PROGRAM GUIDE & POINTS INFORMATION Worksite Wellness is a coordinated and comprehensive set of health promotion strategies and activities designed to encourage all components of employee wellness. All employees working 20 hours or more a week are eligible to participate in the program. Individual Points Levels: Motivated 10-499 points $25. Gift Card Enthusiastic 500-999 points $50. Gift Card Champion 1000+ points $100. Gift Card Your Gift Card will be given at the end of the pilot program. Groups: 1) Health Department 2) DSS 3) Sheriff s Office 4) Admin, Recs and Parks, Elections, Family Support Services 5) EM, EMS and Fire Marshall. Chamber staff 6) Housekeeping, Maintenance and Solid Waste Overall winning group (collectively with the most points) will win a grand prize award. To be announced Ways to Earn Points: All strategies and activities are free except when indicated. All points are cumulative. For example: If you complete 150 minutes of walking during the week (5 pts) and you go to a fitness class 3 times during the week (15 pts) and you track 10,000 steps 5 times during the week (5 pts), you will have a total of 25 points. We reserve the right to check documentation and accountability any time during the program.
Category 1-Provided by Health Department and Swain Hospital Mandatory Pre-Wellness Screening at Health Department to include cholesterol, blood glucose, blood pressure, health risk assessment, etc. and tutorial for online tracking system. You may participate in the program without points if you choose to opt out of the screening. 200 points + pedometer January 9 th 2017 8:15-11:15am at the Health Department; 12:00-2:30pm at DSS January 11 th 2017 8:15-11:15am at the Sheriff s Office; 12:00-2:30pm at Admin. Building Mandatory Pre/Post-Wellness Screening (will count as Pre-Screening for 2017/2018 registering participants) includes cholesterol, blood glucose, blood pressure, health risk assessment ect., and can earn up to 400 point if measurements are improved from prescreening 200-400 points June 12, 2017 8:15-11:15am at the Health Department; 12:00-2:30pm at the Admin. This service allows an individual to review his/her wishes for health care if ever they are unable to speak for themselves. If you are interested, please contact the hospital to set up an appointment. Harris Regional Hospital Tuesdays to Thrive: Is a community education program offered every other month on the third Tuesday. Programs vary. Senior Life Solutions (program at Swain Community Hospital, cost determined upon visit) Senior Life Solutions is a program dedicated to addressing the emotional and behavioral health of adult over the age of 65. 100 points each Diabetes Classes (physician referral required, co-pay possible) each

(ie- Ski Walking, Beach Body, Hiking, Walking, mowing with a push mower anything that elevates your heart rate). Personal Fitness Routine/STEPS Tracking can earn 1 point for every 1 point per 10,000 steps 10,000 steps entered into the online tracking system Maximum of 5 points per week Mobility (ie- stretching, yoga, balance) 1 point for every 15 consecutive (not cumulative) Maximum of 10 points per week Sleep (between 7 and 9 hours) 1 point daily Self-Lead Stress Management 15 minutes (ie-spiritual self-reflection, meditation, daily devotional) 1 point daily *Other programs to be announced HOW TO VERIFY PARTICIPATION IN EVENTS AND EARN POINTS: Points will now be tracked using an online tracking system (maintenance, in-home aids and cleaning will be able to use paper). With the help of Swain Community Hospital, we have made accountability and tracking for the wellness challenge easier than ever!
