Policy: The House of Virtue shall design and implement a staffing plan that includes the type and role of employees and contractors and reflects the: 1. Needs of the population served; 2. Types of services offered; 3. The service description; and 4. The number of people served. The House of Virtue director shall develop a transitional staffing plan for any new services, added locations, or changes in capacity. House of Virtue offers individuals whom have a primary diagnosis of ID (intellectual disability) person-centered programs to meet their needs and goals for successful community leaving. Our priority is to ensure that all medical needs of the individuals are addressed for immediate plan of care. The home will provide an atmosphere, which integrates each individual into the normal daily rhythms and routines of life at the home and in the community. Our programs shall strive to create a safe, secure and permanent setting for individuals who are in need of long-term housing and supervision. Our residential ratio shall be (1-4) for every four (4) individuals with flexibility to provide a 1:2 ratio based on individuals needs. Individuals will be assisted in their efforts in attaining maximum potential for self-sufficiency and independence through assessment and learning opportunities. Learning programs shall be developed according to the individuals strengths, capabilities, and desired outcomes as written in each individuals Person Centered Plan. v House of Virtue LLC will offer support to males and females 18 years of age and older, with a primary intellectual disability diagnosis (ID). House of Virtue LLC will also provide supports for individuals with autism, visual and or hearing impairments. Additionally individuals with a secondary diagnosis to include; Insomnia, Anxiety disorder and obsessive-compulsive disorder. v Individuals receiving supports from House of Virtue LLC may have needs in personal care, medication management, behavior intervention, money management, health care supports, transportation, mobility, fine / gross motor skills, accessing community supports and domestic skills. v House of Virtue LLC will serve 4 individuals in its residential program. v The staff to individual ratio shall be 1:4 for ambulatory individuals in both residential and community settings for each home licensed by the DBHDS and being operated under the license of House of Virtue LLC. v House of Virtue LLC shall provide (24) hour a-day coverage (7) days a week, based on an individual s needs unless their individual assessment has determined that the individual does not require 24-hour supervision. 1-
B. House of Virtue LLC transitional staffing plan for new services, added locations, and changes in capacity shall include the following components: 1. House of Virtue LLC when making modification changes in its program services shall submit an application to the office of DBHDS and get approval prior to the changes being implemented. 2. House of Virtue LLC shall make the request for modification changes in writing. 3. House of Virtue LLC when submitting its written request to the office of DBHDS regarding a Notification of Changes shall address the type of change requested such as: a. Agency name change or its organizational or administrative structure; b. Agency change in the location where services shall be provided; c. Agency services description d. Significant changes in employee or contractor qualifications or in their job description; e. A change in the number of beds that will be provided for services. v The Chief Executive Officer is responsible for the oversight of House of Virtue LLC The Chief Executive Officer will have knowledge of business management and decision making responsibilities for House of Virtue LLC. The Chief Executive Officer will renew and maintain all necessary licenses and certifications for program operations. The Chief Executive Officer shall make final approval on personnel hired and approve all advertising. The Chief Executive Officer will be responsible for public relations with other agencies such as Community Services Boards, Behavioral Healthcare Agencies, and other Placing Agencies. The Chief Executive Officer will also require knowledge of billing procedures for Medicaid, Private Pay Contract Services and Purchase Orders. The Chief Executive Officer will manage the agency budget and will work in conjunction with qualified professional companies in regards to bookkeeping, accounting services, insurances and agency banking business transactions. The Chief Executive Officer shall ensure that the organization stays financially solvent to meet the needs of all individuals receiving services and to ensure all staff is compensated fairly and in a timely manner. The Chief Executive Officer will be accountable for managing and overseeing agency expenditures, revenue and contractual agreements. The Chief Executive Officer requires knowledge of DBHDS, Office of Human Rights, Medicaid Waiver Guidelines and Regulations as well as fair labor standards. The Chief Executive Officer will also be responsible for serving as the Community Liaison for House of Virtue LLC. 2-
The Program Director is responsible for the overall day-to-day needs of each individual receiving service from House of Virtue LLC. Other responsibilities and duties include oversight of Person Centered Plans / PCP checklist and any behavioral support plans. The Program Director will also be responsible for the oversight of the admission, screening / assessment process and oversight of the Residential Program. The Program Director shall also provide oversight and ensure the Implementation and documentation of Person Centered Plans / PCP Support Checklist, and any behavioral support plans and programming. This position shall have knowledge of DBHDS, and the Office of Human Rights, and Medicaid Waiver Guidelines and Regulations. The Program Director will supervise direct care personnel and assist with the oversight of staff scheduling to ensure adequate staff coverage to provide individual supports for the program and participate in hiring all residential personnel. Additionally the Program Director is responsible for personnel management and evaluation of employees. This position shall require good oral and written communication skills and will provide Quality Assurance oversight for the support components of the Residential Program. The Program Director shall also serve as the QIDP. House of Virtue LLC shall secure a contractual agreement with a qualified individual to be the QIDP in the event that there are no personnel on staff to complete the duties and responsibilities of a QIDP. The Program Director shall be on-call to provide support and oversight of crisis and emergency situations. The Direct Support Professional Staff shall be responsible for the day-to-day needs of each individual receiving support under the license of House of Virtue LLC. This position requires knowledge of DBHDS Guidelines and Regulations, and the Office of Human Rights Regulations. Additionally other responsibilities and duties include the implementation of Person Centered Plans, support checklist, experience in behavior intervention and individual programming. This position will also assist with the implementation of proper medication management procedures and meet all certification requirements needed to execute the necessary job duties and responsibilities. The Program Director shall supervise this position. v In the event that an individual has to be moved due to request, abuse / neglect, compliance issues regarding regulations or agency policy or the supports do not match the preferences they require the following individuals will be notified: v The individual and / or their designated Authorize Representative v The Office of Human Rights v The Licensing Specialist and / or the Office of DBHDS v The individual s Support Coordinator v All House of Virtue LLC employees or contractors will be required to have an initial performance evaluation after 6 months of employment and annually thereafter. These reviews will be the responsibility of the Program Director. 3-
1. The Program Director of House of Virtue LLC will review and make the final decisions of all admissions and discharges prior to their implementation. Additionally no individual shall be admitted to a House of Virtue LLC program unless the individual meets admission and program criteria. v The Program Director shall provide supervision and evaluation for all employees and contractors. v Supervision of employees and contractors employed by House of Virtue LLC shall be provided by persons who have experience working with the population served and in providing the supports outlined in the services description. An employee or contractor with experience equivalent to the educational requirement shall perform QIDP supervision for individuals with an intellectual disability. The QIDP shall be responsible for: v Initial essential information of the individual applying for admission. v Identifying the needs of the individual. v Developing an individualized Person Centered Plan based on the identified supports. v Ongoing assessment of individual supports and progress on meeting the quality of life / desired outcomes and the activities and supports of the individualized Person Centered Plan. v Progress reviews and Individualized Person Centered Plan updates as needed and as required. v Discharge planning as appropriate. v Oversee the coordination of supports with Direct Support Professionals and with community based support providers (i.e., day programs). v Training of Direct Support Professionals with how to implement each individual s desired outcomes and providing suggestions regarding the provision of supports to help individuals develop the quality of life and desired outcomes according to their PCP. 4-
a. Provide modifications for desired outcomes, activities and strategies requested or required for the PCP of individuals receiving Intellectual Disabilities services; b. Ensure the implementation of Intellectual Disabilities services desired outcomes, activities and strategies as written in the PCP. c. Provide on-call assessment and decision making for individuals receiving Intellectual Disabilities services who may be in crisis and / or in need of emergency service intervention. 5-
D. The staff training and development will be coordinated and conducted by the Program Director as needed. Staff trainings and development beyond the scope of the knowledge, skills, abilities and certification requirements of the Program Director shall be contracted out to qualified and / or Licensed Professionals. Supervision Frequency / Format: The Program Director shall ensure all services are operating professionally, effectively and efficiently. Two Performance Evaluations within the first year and annually thereafter One to one monthly supervision (If applicable due to performance). Supervision Documentation shall be documented using the following; House of Virtue LLC (Form # 590-A). Listed on the House of Virtue LLC Administrative Monthly Report.
