Forms All forms to be completed need to be dropped off at a Wilmington Health Pediatrics office. Please allow three business days for processing. Visit www.wilmingtonhealth.com/patients/newpatient-registration to print off common forms for your visit. Vaccinations and Immunizations Vaccinations are one of the most important aspects of care that we provide your child. It is our expectation that your child will be immunized as part of well-child care. For the most recent vaccination schedule, please visit www.immunizenc.com. If you do not plan to immunize your child, then Wilmington Health Pediatrics is not a good match for your healthcare plan for your child.
