With two ew paediatric maikis purchased last year the teachig staff were very kee to utilise the maikis to their fullest extet, ad thus Maikis Across the Lifespa was created. Groups of 3-5 studets rotate through statios: SimNewB, where they perform a ewbor assessmet ad a APGAR score; Basic Life Support o a child who is ear drowig; ad diagose a heart rhythm ad admiister the appropriate drug to a collapsed adult patiet. Oe cohort of 82 studets udertook the two-hour Maikis Across the Lifespa sessio, which was evaluated as excellet by over 90 per cet of studets. Staff foud that a rage of maikis was a great advatage to studet traiig. Dr KT Waxma: Establishig Strategies for Your Simulatio Program KT has over 25 years of experiece as a urse leader i Califoria, ad is also the program director of the Califoria Simulatio Alliace (CSA). Usig the premise that some istitutios have purchased simulators but do t kow what to do ext, KT outlied i her 10 March semiar how other schools could beefit from simulatio ad trasform the educatio process. Whe itegratig simulatio ito school curricula, KT proposed that rather tha just a add-o to a course, simulatio should be icorporated ito every course, from fudametals to advaced. A vital part of this was i creatig a early strategy to phase this i, with the full support ad developmet of the faculty critical to its success. These days, Califoria boasts a amazig 90 per cet of its 135 ursig schools with a simulatio support program. KT oted that cliical placemet hours were beig replaced by simulatio at aroud 25 percet, sometimes up to 50 per cet, with Califoria ow offerig residecy programs for ursig graduates i hospitals ad Orego State offerig a state curriculum for ursig. SMALL TEAM MAKES BIG STRIDES A volutary ursig traiig program is makig big waves at Edith Cowa Uiversity s regioal Bubury campus, 175 km south of Perth. With limited resources the program ehaces studets cofidece i their cliical skills ad forges excellet relatioships with a multidiscipliary approach. The program was established i respose to the widespread educatioal literature statig that urses were t idustry-ready, ad that some ca really struggle i their first year out, says Seior Lecturer ad Program Coordiator Sue Tecer. We also oticed that may studets had verbalised a low self efficacy prior to their fial five-week practicum. The volutary Theory to Practice (T2P) program for third year studets is a safe eviromet for studets i their fial semester to stregthe their cliical skills, mius the stress of assessmet. The studets idetify the particular skills they eed more exposure or practice i, ad we as lecturers also idetify those skill deficits. The we match them to the appropriate patiet with that skill set. says Sue. Ru over a week, the te-bed traiig ward has te medium-fidelity maikis, each with a differet healthcare sceario. The 35 studets were allocated a patiet durig rotatig four-hour shifts, ad ursig care plas are developed by staff for each maiki. The studet is also required to carry out doctors orders after ward rouds with realism heighteed by all staff wearig uiforms ad experiecig hadover ad documetatio, as per a fuctioig ward. Described by Sue as the essetial ursig care skills required to provide best practice patiet care, the studets practice deliverig hygiee eeds, vital sig observatios, bladder scaig ad maagig itraveous therapy icludig blood Helpig improve patiet outcomes trasfusios ad admiisterig medicatios i a timely maer. With oly eight staff i the ursig program, over the three years the program has evolved ad T2P ow icludes medical studets from the Rural Cliical School, ECU SW Social Work studets ad staff from the local South West Health Campus. This multidiscipliary approach works from all agles: medical studets are provided with a diagosis, write admissio otes, order diagostic tests ad prescribe medicatios, social work studets do discharge plaig, geeral staff members act as disruptive family members ad security maages them. Of this iclusive approach, Sue otes: It may be difficult to implemet i a larger campus, but our staff-studet ratios are the same. The bottom lie is the dedicatio of the ursig program team to do it outside of the curriculum. I such a small team as ECU Bubury, every perso is as vital to the program s success as the other perso, from the program coordiator to the lab techicia. The future of this small yet high-impact program looks very bright, with a excellet respose from all participats. After a late- 2009 sessio, staff were amazed to fid the studets were reluctat to be set home earlier o the last day with may of them stayig o to complete their shift ad complete the ecessary documetatio. T2P was also recogised this year whe they received the 2010 Vice-Chacellor s Learig ad Teachig Award for Programs that Ehace Learig, ad a rage of cliical placemet facilities reported that the ew graduates show excellet cliical skills ad a marked cofidece i their ew role. Further opportuities are oly limited by our imagiatio, says Sue. Staff see T2P as a catalyst of richer iterdiscipliary learig occurrig well ito the future. SUCCESSFUL NewB PROGRAM AT RBWH With over 200 retrievals over 60 sites per year, the eoatal retrieval uit at Royal Brisbae ad Wome s Hospital (RBWH) is well placed for the itroductio of a specialised simulatio program for eoatal retrieval traiig. Sice 2009, mothly programs have bee ru i cojuctio with the Skills Developmet Cetre at RBWH, ad are provig to be very successful with participats. The four-hour program is usually atteded by two doctors ad two urses (two retrieval teams), ad uses the SimNewB maiki. Participats iitially receive a itroductio to Crisis Resource Maagemet, which looks at the o-cliical or huma factors i dealig with emergecies. Oe team is the set o retrieval ; they receive a sceario ad maage the SimNewB as if this were a eoate o a actual retrieval. Team members wear a retrieval uiform ad use the eoatal trasport cot ad retrieval pack. Twety- to thirty-miute scearios are videotaped ad observed by the other course attedees, ad are followed by a debriefig sessio. The teams the reverse roles, ad a secod sceario is coducted. Participats are at varied levels of experiece, from those who have ever goe o retrieval before, to those who are very experieced. Future plas iclude traiig up doctors ad urses with o experiece of retrieval to do the simulatio program prior to their first retrieval. Staff i actio at RBWH The RBWH s approach to simulatio traiig is provig so successful that participats ofte commet that they forgot they were workig with a maiki durig the highly realistic scearios. The US-based SSH was established i 2004 to represet the educators ad researchers who utilise simulatio for educatio, testig, ad research i health care. With a membership base of 2,000, SSH recogises that simulatio represets a paradigm shift i health care educatio, ad works to promote improvemets i techology, educatioal methods, practitioer assessmet ad patiet safety. Dr Kerry Reid Searl discussed her teachig strategy: High fidelity latex patiet simulatio (HFLPS), which is i use at a udergraduate ursig program at CQ Uiversity, Adelaide. Addressig some of the limitatios of maikis to produce realistic huma resposes, HFLPS uses life-like latex masks ad body parts adored over the body of the educator to mimic patiet resposes. Searl s paper icluded a hads o presetatio, where the audiece egaged with her (masked as the patiet), to experiece how the learig occurs. They discovered that studets who trai i this way are able to practice skills, receive immediate feedback ad react to upredictable resposes as though the situatio was real.
