PROGRAM OVERVIEW & TARGET AUDIENCE The purpose of this educational activity is to present case-based approaches to the diagnosis and management of patients with pulmonary hypertension. This activity has been designed for physicians, advanced practice nurses, physician assistants, respiratory therapists, nurses, residents, fellows and researchers in pulmonology, cardiology, critical care and rheumatology who care for or study patients with pulmonary vascular disease. Learning Objectives Upon completion of this activity, participants should be able to: Explain the approach to the diagnosis of pulmonary hypertension in different clinical presentations Describe the differences in treatment strategies used by the care team, using a case-based approach Outline the ways to risk stratify patients with different forms of pulmonary hypertension Discuss the role of advanced treatments used by the interdisciplinary care team for a patient with a pulmonary embolism Review the risks associated with pregnancy in pulmonary hypertension Hosting Department The 5th Alfred P. Fishman Symposium: Case-Based Approach to the Diagnosis and Treatment of Pulmonary Hypertension and Pulmonary Embolism is hosted by the Pulmonary Hypertension/Pulmonary Vascular Disease Program, Department of Medicine, Perelman School of Medicine at the University of Pennsylvania. Accreditation and Credit Designation In support of improving patient care, Penn Medicine is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team. Physicians: Penn Medicine designates this live activity for a maximum of 6.25 AMA PRA Category 1 Credits TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Physician Assistants: AAPA accepts certificates of participation for educational activities certified for AMA PRA Category 1 Credits TM from organizations accredited by the ACCME or a recognized state medical society. PAs may receive a maximum of 6.25 Category 1 Credits for completing this activity. Nurses: This activity is approved for 6.25 contact hours. Certificates and Transcripts: After the symposium has concluded, all participants will receive a follow-up email with instructions and a link. Once participants submit the request for credit, the earned credits will be available for viewing on a transcript and certificates will be available for printing at www.penncmeonline.com. Please note that the individual certificate will not be mailed. Registration Fees The registration fees for The 5th Alfred P. Fishman Symposium: Case-Based Approach to the Diagnosis and Treatment of Pulmonary Hypertension and Pulmonary Embolism are the following: $155 for physicians and industry professionals, $95 for advanced practice nurses, physician assistants and other healthcare professionals and $75 for residents and fellows. The registration fee for all Penn Medicine employees, faculty and staff is $55. The registration fee includes attendance to the symposium, a symposium booklet and a link to view the presentations online, continental breakfast, mid-morning and afternoon refreshments, lunch, and postsymposium reception. Cancellation and Refund Policy In order to process refunds for symposium withdrawals, cancellations must be received in writing by March 9, 2018 (subject to a service charge of 10% of the registration fee but not less than $15.00). No refunds will be issued after March 9, 2018. The University reserves the right to cancel or postpone any symposium due to unforeseen circumstances. In the event of cancellation or postponement, the University will refund registration fees but is not responsible for related costs or expenses to participants, including cancellation fees assessed by hotels, airlines, and travel agencies.
