ABOUT FAMILY & COMMUNTIY SERVICES Centacare's Family and Community Services provide comprehensive, client focused services for individuals, couples, families, children, youth and adults. Our services are informed by an approach that values human dignity and the belief that positive relationships lead to positive lives and wellbeing. The service design aims to ensure that our programs are inclusive, responsive to the needs of clients and operate at the highest levels of quality and professionalism. Services are integrated, strengths-based and provided by staff that are trained and qualified. Regular travel will be required across the Diocese catchment and to locations other than office bases. PORTFOLIO PROGRAMS This Coordinator position is part of the Integrated Family Services and Cradle to Kinder leadership team within Family and Community Services. Integrated Family Services Central Highlands The Centacare Integrated Family Services (IFS) is a case management program delivered in partnership with the Department of Health and Human Services, Child FIRST and an alliance of local family services programs. IFS aim to promote the safety, stability and development of vulnerable children, young people and their families, from before birth to 17 years of age, by providing a case work services and linking families with relevant support services. The entry point for Family Services is through Child FIRST. Referrals can be made by anyone with a concern for the wellbeing of a child (from unborn babies to age 17 years). POSITION PURPOSE: The Coordinator Integrated Family Services is part of the IFS leadership team and is responsible for assisting the Manager Family & Community Services with the overall operations of the IFS teams in Ballarat and Bacchus Marsh. A key role of this position is to provide leadership and supervision to IFS case managers, including the development of annual individual and program works plans, performance appraisals, and the identification of training and development requirements. The incumbent will monitor budget requirements, allocation of caseloads, assist with quality processes and ensure a high quality of service delivery. The incumbent will provide leadership in the workplace by modelling the values of the organisation via high standards of work performance, positively influencing teams and individuals, negotiating, mediating and resolving issues where possible at the local level. REPORTING LINES The role will report to the Manager, Family and Community Services (IFS and Cradle to Kinder). LEVELS OF AUTHORITY Significant decisions will require the approval of the Manager, Family & Community Services. The extent of financial decision making will be as per the Centacare instrument of delegation. This role will be required to supervise staff in allocated portfolio programs. Page 2 of 8

RELATIONSHIPS Internal: Works under direction from the Manager, Family & Community Services (IFS and Cradle to Kinder). Develop a working relationship with all components of Centacare s management and staffing structure. External: A range of business contacts of the Family & Community Services portfolio including the Department of Health and Human Services, Alliances and partnerships and other stakeholders as required. Role Specific Duties & Responsibilities Team Leadership Demonstrate sound leadership to enable an outcome focused flexible and coordinated service. Responsibility for the day-to-day coordination of the IFS team in Ballarat and Bacchus Marsh. Provide supervision/support to program staff. Prepare and monitor staff rosters. Monitor staff performance objectives. Coordinate staff records, including time sheets and leave arrangements. Facilitate team meetings that encourage staff input and support collaborative decision making processes. Provide general leadership support to other organisational staff as may be required. Foster the learning and development of program staff and liaise with the Manager regarding training and development opportunities. Program Coordination Assist in the development and implementation of program planning. As part of the leadership team assist in the required project work to ensure current documentation of program processes. Assist with monitoring program expenditure. Monitor and supervise the maintenance of client records, systems and client data recording in accordance with funding body and legislative requirements. Review and collate data to ensure entry in a timely and accurate manner in accordance with funding body requirements. Identify and progress opportunities for the IFS teams to engage with relevant stakeholders to enhance client outcomes. Represent the organisation at relevant networks and meetings as directed. Liaise with funding body representatives as directed. Facilitate completion of and/or prepare reports in accordance with funding body requirements. Liaise with peer Coordinators and Managers as required on referral pathways, practice guidelines, program delivery and service models. Service Delivery & Client Lead and support staff to develop enhanced service provision that is Support outcome focused, flexible and coordinated across the organisation and the broader health and community sector. Develop and maintain relationships in the IFS and the community sector. Provide direction to staff in the promotion of IFS team. Coordinate the allocation of client caseloads. Coordinate and review intake, assessment and allocation processes in consultation with management and staff members. Page 3 of 8

Role Specific Duties & Responsibilities Ensure that all clients have a comprehensive risk and needs assessment and plan development based on the Best Interest Case Practice Model. Provide a high level of supervisory support to case managers in the case management of clients and families identified with greater complexities, risk or requiring a higher level of intervention. Facilitate case conferences for more complex clients and families. Provide supervisory support to staff members with regard to client management issues. Ensure regular review of client case direction, service planning and goal attainment. Supervise staff and monitor organisational achievement of funded service plans and key performance indicators. Advocate and actively seek innovative strategies to enhance outcomes for clients and other stakeholders. Quality Management Ensure that every IFS case is provided with a thorough risk and needs assessment process with regular review. Ensure appropriate and timely case recording for all clients. Actively participate in organisational quality and risk management processes including external accreditation requirements. Monitor achievement of funded service plan targets and key performance indicators and prepare associated reports. Actively participate in evaluation processes with a focus on client outcomes, service responsiveness, flexibility and staff satisfaction. Develop, implement and review client participation practices and feedback processes and incorporate learning s into enhanced service practice. Organisational Contribution Actively support Centacare Catholic Diocese of Ballarat mission, vision and values. Identify, correct and report any unsafe act, condition or behaviour according to Centacare s policies and procedure. Report any injury or incident by lodging a DINMAR within 24 hours and to the respective funding bodies as per their incident reporting guidelines. Risk Management Ensure that Centacare s Risk Management Policy, program and the application of sound risk management practices within the workplace and community are adhered to and complied with at all times. KEY SKILLS & EXPERIENCE Essential The successful candidate will have: Professional qualifications in Social Work, Welfare or other relevant tertiary studies. Demonstrated experience in service coordination of Government funded client centred programs. Demonstrated experience in a leadership and supervisory role. Knowledge of relevant Legislation. Sound knowledge of child development theories, the impact of trauma on children and working with families where complex issues can constitute risks to children and young people and the ability to demonstrate this knowledge in practice. Demonstrated understanding and ability to develop and undertake holistic risk and needs assessment processes such as the Best Interest Assessment and Case Practice Framework. Demonstrated excellence as a child/family service practitioner and ability to promote a high standard of practice in the delivery of Family Services Case Management or similar program. Sound understanding and sensitivity towards the complex issues facing vulnerable clients and families. Demonstrated experience of case management principles and practice. Highly developed interpersonal, verbal and written communication skills. Demonstrated capacity to network and engage stakeholders in the ongoing development and enhancement of client centred programs. Attachment One: Information Statement about the Disability Worker Exclusion Scheme Background In order to comply with the requirements of the Privacy and Data Protection Act 2014, Centacare provides this Information Statement for all prospective Disability Workers (as defined). Requirements for employment Centacare is required to provide a person s details to the Disability Worker Exclusion Scheme (DWES) Unit to check if their name is on the List before permitting them to commence work as a Disability Worker. If a prospective disability worker does not agree to participate in the DWES than they cannot be engaged by Centacare or another disability service provider as a Disability Worker. Additional Information Prospective Disability Workers need to be aware of the following information: If, during the recruitment process, information comes to light which, if correct, may satisfy the criteria for a person being registered on the DWES List (the List) of excluded workers, Centacare will be required to notify the DWES Unit. The DWES Unit may also notify other disability service providers or authorised labour hire agencies that currently engage the prospective Disability Worker. The consequence of having your name placed on the Disability Worker Exclusion List is that you will be prevented from being engaged by Centacare or another disability service provider as a Disability Worker in a disability service. The DWES Unit maintains the List and may advise other employers or prospective employers that are disability service providers, if the person is on the List or is the subject of a Notification. Process for checking a prospective Disability Worker against the List A preferred candidate will need to complete and sign a Consent and Acknowledgement Form provided by Centacare which will be forwarded to the DWES Unit to check a person s details against the List. In addition, preferred candidates will need to complete and sign a Statutory Declaration for Disability Workers supplied by Centacare. Page 7 of 8
