EMPLOYEE NAME: Festus Department: Recreation Department Position Title: Van Driver Reports to: Chief Operating Officer New Hire Annual Re-Evaluation Evaluation Period: From: June 2009 June 2010 Position Summary: The overall purpose of your position is to provide transportation and assistance to the independent and assisted living residents of JCHC communities when they travel on the agency s van. Qualifications: To perform this job successfully, the following education, training, certification, language skills, mathematical skills, computer skills and/or experience is required: 1. Ability to read and write in English. 2. CDL (commercial driver s license) for a 16 passenger vehicle. 3. A High School diploma. 4. Ability to safely transport residents to the supermarket, shopping mall and other scheduled trips. 5. Ability to provide residents assistance with their packages after a shopping trip. 6. Ability to operate the wheelchair lift within the van. 7. Be able to offer aid to the residents as they get on and off of the van. 8. Responsible for van maintenance. Reasoning Ability: Based upon the Mission, Vision and Values of The Jewish Community Housing Corporation, the following abilities are required to successfully accomplish the essential duties of this position: 1. Posses the ability to make independent decisions as needed. 2. Assessment skills to evaluate maintenance needs of the van and recognize when those needs change and recommend adjustment of the program accordingly. 3. Ability to recognize an emergency situation either in or out of the facility and be able to call for the assistance required. Leadership Responsibilities: The following level or kind of leadership responsibility is assumed by this position: Driver Responsibility: To report to work on time, assist the residents on and off of the van, safely take them to their destination and upon their return provide assistance with the resident s packages back to their apartments. 1
Physical Demands: A. Light Work: exerting only a few pounds to bring residents their groceries. Communicates pertinent issues to the Recreation Director. Festus is very adept with meeting everyone s needs and flexible to accommodating schedules. Serves as advocate for residents Completes work orders as required or requested Participates as directed in any facility surveys Assures that Resident s Rights are upheld Attends all mandatory and non-mandatory facility inservices 2.0 Maintains department work areas in a clean and safe manner. Maintains van and brings it in for maintenance when needed. At times, Festus has not been able to attend trainings or staff meetings due to transportation schedules. Demonstrates ability to understand Reportable Evens Policies and Procedures Is punctual for scheduled shift Give specific examples of the employee s strengths and any additional accomplishments during this evaluation period: Mr. Richards has consistently demonstrated exceptional communication skills, conflict resolution and ability to balance the requests of the residents with the coordination of an effective transportation schedule. Festus provided invaluable support in our research for a new van for Lester. His technical knowledge of the vehicle coupled with understanding van specifications that meet the needs of our residents resulted in an excellent choice for our transportation needs. Mr. Richards also implemented all needed OSHA and Safety protocols and items in the vans. Mr. Richards takes on a leadership role with assisting the van driver at Lester.
