MEET THE Speakers Keynote BRIAN BAUER TUESDAY, NOVEMBER 15 10:15 to 10:45 am Shareholder in the law fi rm of Hall, Render, Killian, Heath, and Lyman, PLLC Health Law with over 20 years of experience in health law. Brian concentrates his practice in the areas of health care regulatory compliance, business transactions, reimbursement and governance. CMS Bundled Payment Initiatives; Potential Impact on Rural Patients and Communities. The program set goals to improve long-term health outcomes by reducing barriers to transportation, gather data to support the theory that transportation does, in fact, improve patient health outcomes and create a replicable and sustainable program for Missouri. DirectConnect: Telemedicine for Rural Healthcare Michelle Bekemeier & Rebecca Weddle, Corporation Account Managers CoxHealth has been developing and utilizing telemedicine services to increase healthcare options availability to patients in rural areas. CoxHealth s Direct Connect is a solution that allows patients to connect with providers from the comfort of their home or workplace for a variety of healthcare needs. WEDNESDAY, NOVEMBER 16 8:30-9:30 Recruiting for the Generations Joni Adamson, Recruitment and Workforce Development Manager, MO Primary Care Association When implementing a recruitment plan, employers should have a clear understanding of what is important and valued by each of the different generations. Recruiting efforts should not only include the available positions, but the types of people that are needed to fi ll the positions. Employee retention can be signifi cantly increased when generational differences are recognized and taken into consideration. Title VI: Components of a Solid Civil Rights Plan Angela Sutton, Senior Civil Rights Specialist, MODOT A successful Title VI program gives the public the assurance that agency services and resources will be allocated fairly. Transportation touches the daily lives of everyone living in this country. Whether getting to work, the market, visiting family or friends, or accessing the health care system, our public roads take us there. In this workshop, we will discuss what Title VI is and what is necessary to ensure compliance with its rules, laws and regulations. Understanding Non-Emergency Medical Transportation Guidelines Nanci Nikodym, Missouri Dept. of Social Services Nanci has worked with the Department of Social Services for almost 25 years. She is a manager for MO HealthNet in the Program Operations unit. In addition to her other responsibilities, she oversees the state contract with LogistiCare, the current non-emergency medical transportation broker. She thoroughly enjoys the work she does for MO HealthNet. Nanci will discuss Missouri s NEMT program for Medicaid benefi ciaries. Using Community Health Information to Improve Care and Health Leslie Porth, Sr. VP, Strategic Quality Initiative, MO Hospital Association Aligning population health and quality of care is an essential step in transforming care delivery to achieve the goals of better health and better care. We received a MODOT 5310 contract this year and track transportation needs through a Electronic Health Record and in Excel. We have established relationships with other organizations and reimburse them for rides they provide to our clinics. There is a key networking component that is vital to the success of this project as well as strengthening our Rural Health Network. PRESENTER Showcase JONI ADAMSON As the Manager of Recruitment and Workforce Development, Joni is dedicated to increasing access to health care services in underserved areas. She works closely with healthcare professionals of various disciplines and specialties to assure they are aware of career options and employment opportunities in Missouri s Safety-Net System and to identify resources to help them reach their goal of working in an underserved area. She provides a great deal of assistance and guidance to community organizations to assist with recruitment and workforce development. She currently serves on the National Advisory Council for the National Health Service Corps; Board of Directors for the Rural Recruitment and Retention Network (3RNet); and Program Advisory Committee for Concorde Career College. She holds a Master of Business Administration degree from William Woods University and a Bachelor of Arts degree in Business Management and Administration from Columbia College. SUZANNE ALEWINE Prior to forming Community Asset Builders, LLC, Suzanne Alewine was the Director of the Community Health Assistance Resource Team for the Missouri Department of Health and Senior Services.Her expertise includes community assessment and data analysis, budgeting, accounting, resource development, and event management and planning. Suzanne completed an undergraduate degree in Business Administration ion and a Masters in Public Administration at the University of Missouri Columbia. She is a founding member of the Association for Community Health Improvement - and served as an Advisor from inception; is a past co-chair of the national Healthy Communities Network; and is a graduate of the Health Forum s Creating Healthier Communities Fellowship. AMANDA ARNOLD Amanda Arnold RN, BSN, Director of Nursing. Amanda Arnold is a Board Certifi ed Registered Nurse. Amanda has been at Health Care Coalition since 2013 where she has taken on the role of the Health Home Director for the state of Missouri s Patient Centered Health Home initiative. DORIS BOECKMAN Doris is co-founder of Community Asset Builders, LLC, providing community assessment, strategic planning, and resource development services to primarily non-profi t organizations; i.e., federally qualifi ed health care centers, hospitals, health systems and state associations. She is also a co-administrator of HealthTran, a program operated by the Missouri Rural Health Association. HealthTran links health care coordination and transportation mobility management services to create a simple, effective approach to improving access to care. She is the Missouri Rides to Wellness Director for the Missouri Rural Health Association, serving as the liaison between local, state and national organizations on issues that relate to improving health care access by reducing transportation barriers. Doris has previously been employed with the Missouri Department of Health and Senior Services, Missouri Hospital Association, Missouri Infertility Prevention Program and Missouri Medicaid Program. She has a BS Degree in Business Administration from Lincoln University, Jefferson City, MO. CYNDI BUSSEN Certifi ed Application Counselor Manager for CoxHealth in Springfi eld MO, serves as the hub leader for the Cover Missouri Coalition in the Southwest Missouri Region. The Cover Missouri Coalition was established by the Missouri Foundation for Health as a component of the Expanding Coverage Initiative. Cyndi has many years of experience in the healthcare industry working directly with physicians in an effort to build their practices and in administration. SHANNON CALHOUN Ms. Calhoun is Regional Vice President, Southwest, for the National Rural Accountable Care Consortium. She served as the Executive Director for the American Rural ACO in 2015. During her 20 years in rural healthcare, Mrs. Calhoun has been responsible for developing products and services for rural networks that create sustainable solutions and an economy of scale and scope for both the network and the network s rural healthcare members. Her work includes development of a rural PPO network, provider/payor contracting and credentialing, fee strategy development, direct contracting with employers, and Medicaid. She has been awarded grants totaling $9 million. All of Mrs. Calhoun s work is focused on programs that better position rural providers for healthcare reimbursement reform. Ms. Calhoun was awarded the Texas A&M University s Aggie 100 in 2011, 2013 and 2014. She also serves on the board of the Texas Health Services Authority by appointment of the Texas governor. KATE CONN Kate Conn, Certifi ed Application Counselor for CoxHealth, in Springfi eld is a part of the SW MO Regional Hub, established by the Missouri Foundation for Health (MFH), as a component of the Expanding Coverage Initiative. She is a member of the Cover Missouri Coalition and presented for the Enroll America Conference in 2016. TRENT DEVREUGD, MHA DeVreugd is a Program Quality Improvement Specialist for Primaris where he is working with communities to build capacity to address the growing numbers of diabetics throughout the state. He has worked extensively to improve care coordination between healthcare providers, community and patients. DeVreugd has 23 years of non-profi t leadership experience focusing on mental health, comprehensive neighborhood delivery systems, and health and aging initiatives in the Kansas City area. He resides in Kansas City with his wife Erin and 3 children. BRIAN R. DOBBINS, ESQ Mr. Dobbins was named Aetna s Chief Executive Offi cer in 2014; he has more than 20 years of experience in Medicaid and Medicare health care administration. He enjoys improving the overall operations of an organization and is skilled in implementing continuous quality improvement programs, enhancing customer value, and developing and implementing comprehensive training programs. DEANA TUCKER DOTHAGE Mobility Manager, Boonslick Regional Planning Commission Deana Tucker Dothage has served as the Mobility Manager for Boonslick Regional Planning Commission since August 2014. She was hired to create a one call one click program in a fi ve county region to help people with disabilities and older adults fi nd rides utilizing existing transportation, to create a website, and to educate the community that this resource existed. Under the direction of BRPC and guidance from the Eastern Missouri Coordinating Transportation Council, Dothage developed the program and a website, known as MO RIDES. Dothage is working to develop a volunteer driver program that will pilot in the counties served by BRPC. Prior to mobility management, Dothage was in the insurance industry for 29 years, (18 years with her own agency, 11 years as a casualty claims adjuster) and law enforcement for 3 years. MARY GORDON HealthTran, Project Manager Mary Gordon is an experienced community organizer and has worked with grassroots collaborations to bring positive changes. With a focus on high risk families and specifi cally youth, she has worked from an in-home case manager to leadership positions. Her work as the Workforce Investment Program Functional Manager of the Mexico Missouri Career Center lead various state programing and assisted low-income families, dislocated, and youth with employment and career assistance. it Director of the Retired Senior Volunteer Program in Callaway County Missouri for 3 years, she started the annual 55&GO Senior Expo serving over 200 seniors and families with resources and information. She is most proud of the team effort of the county community grass roots collaborative which resulted in a Federally Qualifi ed Healthcare Center offering dental, family medicine, and behavior health services into the area. PRESENTER Showcase STANTON TODD HUDSON Associate Director, Center for Health Policy Stan Hudson is a health literacy expert and health policy analyst with 20 years of experience in health services research and 12 years of experience in health literacy with a focus on curriculum development. He earned an M.A. in Sociology from the University of Missouri and B.A.s in History, Political Science, and Sociology from Columbia College. Over the last decade he has led the development and implementation of health literacy education programs for consumers, health professionals, public health agents, consumer advocates, and health educators. In 2014, he became an NCCJ-certifi ed Facilitrainer and has been developing and implementing inclusion and diversity workshops for education, health, public health professionals, and students. HOLLY KREIENKAMP Holly Kreienkamp is the Mobility Manager for Meramec Regional Planning Commission (MRPC). As a professional service representative she came in contact with people on a daily basis that did not have transportation for even the most basic needs. It was a natural progression for her to take her previous experience into account and began working on solutions to address transportation within the rural areas of Missouri. She takes transportation seriously! In September 2015 she began her position as Mobility Manager for MRPC.. Kreienkamp has also been working to develop new transportation routes and a Transportation Voucher Program for residents within rural areas of the Meramec Region. Kreienkamp resides in Cuba, Mo. ANN KAY LOGARBO, MD, FAAP Ann Kay Logarbo is the Chief Medical Offi cer of the UnitedHealthcare Community Plan of Louisiana. Dr. Logarbo joined UCH in 2012 as the Chief Medical Offi cer for the Community Plan of Louisiana. Prior to that she practiced pediatrics for 27 years in Covington, La., and was a founding partner of Northlake Pediatrics. While in practice, Dr. Logarbo served on the pediatrics committees of St. Tammany Parish Hospital and Lakeview Regional Medical Center, both in Covington. Since joining UHC, Dr. Logarbo has been recognized by New Orleans CityBusiness as one of their 2014 Women of the Year and as a 2015 Health Care Hero. She is also a member of the Louisiana chapter of the American Academy of Pediatrics. JIM MIKES, CEO, MISSOURI HOSPITAL ASSOCIATION Jim Mikes is Vice President for Rural Advocacy and Regulation at the Missouri Hospital Association. He began his career as a respiratory therapist and has over twenty fi ve years of hospital leadership experience. Jim holds a graduate degree from St. Louis University and a doctorate from Tulane University. SANDRA MORRIS, SENIOR HEALTHTRAN COORDINATOR A 21-year background in working for the Family Support Division, Eligibility Specialist, with the last 13 years working from Ozark Medical Center in West Plains, MO. Sandra has a broad experience in the needs and barriers of southern rural Missourians. Her dedication, knowledge, and ability to communicate effectively with all types of people with all types of people make her an invaluable addition to the HealthTran team. ANTHONY NICHOLS For the past year, Anthony Nichols has serves as Mobility Manger for MO Rides in the central region which covers 8 counties. Apart from the primary service of ride to referrals to existing providers, Anthony has begun to utilize community partners for transportation pilot programs. His singular focus is to use any and all means at his disposal to help as many people as possible reach their destinations. Anthony s prior work involving youth and elementary education benefi ts his mobility management position. NANCI NIKODYN, NEMT Nanci has worked with the Department of Social Services for almost 25 years. She is a manager for MO HealthNet in the Program Operations unit. In addition to her other responsibilities she oversees the state contract with LogistiCare, the current non-emergency medical transportation broker. She thoroughly enjoys the work she does for MO HealthNet. CARRIE PORTER Carrie Porter is the Transportation Specialist/Older Americans Act Consultant for the Greater Wisconsin Agency on Aging Resources, Inc. GWAAR is an Area Agency on Aging serving 70 of the 72 counties and 11 tribes in Wisconsin. Carrie provides training and technical assistance to aging programs across WI in mobility management, transportation coordination, partnership building and program development. Carrie is a Certifi ed Mobility Manager, a board member of the WI Association of Mobility Managers (WAMM) and member of the WAMM Certifi cation and Training Committee. LESLIE PORTH PH.D., MPH, R.N. In the last four years, her expertise has been instrumental in successfully implementing and launching programs that improve access to care and health care delivery to underserved populations DANA SCHMITZ, MS Dana Schmitz received her BS & MS in Nutritional Sciences and Food from the University of Missouri-Columbia. She has worked in quality improvement, program management and data analysis with the Missouri Department of Mental Health as well as Department of Health and Senior Services. At Primaris, Ms. Schmitz currently leads the care transitions project team in developing a comprehensive quality improvement project, assisting communities in the selection of evidence-based interventions and compiling and interpreting the performance metrics that drive large scale change in those communities. JUDY SHANLEY, PH.D. Judy Shanley is the Vice President, Education and Youth Transition at Easterseals. She manages projects, provides technical assistance, conducts research, and develops materials regarding accessible transportation, mobility management, and coordinated transportation. Shanley also oversees Easterseals national education and youth transition programs. She serves as a project director to several large national centers, funded by the Federal Transit Administration (FTA), and the Administration for Community Living, including the National Center for Mobility Management. Shanley supports FTA in their implementation of diverse Rides to Wellness initiatives. Prior to Easterseals, Judy worked at the U.S. Department of Education, where she served as a technical assistance chair for United We Ride and has over thirty-years of experience in supporting individuals with disabilities. She earned her Ph.D. in Special Education from the University of Florida and an M.B.A. and a Master s in Rehabilitation Counseling, both from Syracuse University. BROCK SLABACH Senior vice-president for member services, National Rural Health Association (NRHA). Brock joined the NRHA in 2008. He has administrative responsibility for all areas of member services, including membership, communications and meetings/exhibitions. He also coordinates the Healthcare Quality Technical Assistance center. He was a rural hospital administrator for more than 21 years. He has been on the board of the National Rural Health Association and the regional policy board of the American Hospital Association, as well as many regional and state boards involving rural health. Brock earned a master of public health degree in health administration from the University of Oklahoma and a B.S. degree in biology from Oklahoma Baptist University. He is a fellow in the American College of Healthcare Executives. SUZANNE SMITH Suzanne Smith, Director of Network Development at HCC. She graduated from Central Missouri State University in 1989 with a Bachelor in Recreation and a minor in Business Management. She has served as the CEO of the Salt Fork YMCA. She provided leadership in a non-profi t setting to 60 staff members. Suzanne has been involved in fund development for over 12 years and has successfully written and secured over a million dollars worth of grant dollars from the Department of Economic Development for the State of Missouri Neighborhood Tax Credit program. At HCC, Suzanne is responsible for the Network Activities including general membership, local and federal grants, tele-health school based clinics, Community Health Workers, and networking outreach opportunities. Suzanne is supported by a full time network coordinator and fulltime transportation coordinator who assists with program and fi nancial reports and implementation. JORDAN SNOW, RESEARCH AND TA ANALYST, NCMM AND EASTERSEALS IOANA STAICULESCU Ioana Staiculescu is currently the research specialist at the Center for Health Policy at the University of Missouri and the interim project director of the Missouri Health Equity Collaborative. She has more than fi ve-year experience working on health disparities projects and health literacy education activities, with focus on mental health disparities, immigrant, refugee and LGBT health, as well improving the data collection processes and policies around race, ethnicity, language, sexual orientation and gender identity in hospitals and community health centers. Staiculescu is also a certifi ed FaciliTrainer by NCCJ St. Louis to assess, design, and deliver effective, quality diversity and inclusion programs. Ioana graduated the University of Medicine and Pharmacy Carol Davila in Bucharest, Romania in 2005 and from the University of Missouri with a Master s in Public Health in 2009 focused on health promotion and policy. 13

PRESENTER Showcase ANGELA SUTTON Angela Sutton is the Title VI/Nondiscrimination Program Coordinator for the Missouri Department of Transportation (MoDOT). In this capacity, Mrs. Sutton is responsible for developing and directing the Title VI/Nondiscrimination Program for MoDOT. She has nearly 14 and a half years with the Department, in which 4 ½ years of this time has been within the External Civil Rights Division. Mrs. Sutton began her government career while pursuing her education at Lincoln University. She earned her Bachelors of Science degree in Business Administration in 2003. MELISSA VANDYNE Melissa VanDyne has fourteen years of experience working at the Department of Health and Senior Services. Melissa began her current position as the Rural Health Manager which serves as the Flex/Rural Hospital Coordinator and the Director of the State Offi ce of Rural Health on April 8, 2013. During this time, she has overseen the FLEX grant, SORH grant and SHIP grant. Melissa has also monitored the contracts associated with the various Offi ce of Rural Health programs. She has developed and analyzed survey and feedback from rural hospitals. Melissa has developed work plans for the Offi ce of Rural Health programs. DENNY WARD Denny Ward was named Executive Director of SMTS in December of 2014. He began his career with SMTS in 2006 as Controller, and was promoted to Assistant Executive Director in 2010. Denny serves on the Board of Directors for Missouri Public Transit Association, and is an avid supporter of human services in his region. Founded in 1973, SMTS provides service in 20 Missouri counties. In the last fi scal year, SMTS provided 300,000 one-way trips, driving some 4.2 million miles with a fl eet of 175 vehicles. REBECCA WEDDLE Born and raised in southwest Missouri, Rebecca Weddle joined CoxHealth in 2012 as Corporate Account Manager for CoxHealth, focusing on enterprise growth through contracting, sales and business development. Rebecca is a member of the Springfi eld Area Human Resource Association (SAHRA) and is active in the Springfi eld, Branson and Lebanon Chambers of Commerce and enjoys spending time away from work with her two daughters, Ashlyn and Addyson. DOROTHY YEAGER Dorothy Yeager was named Executive Director of OATS in July 2012. She started with OATS in 1987 after graduating from the University of MO and has held many positions with the company in communications and administration. She serves as Vice President of the MO Rural Health Association and MO Public Transit Association Boards of Directors and is also a State Delegate for the Community Transportation Association of America. Founded in 1971, OATS provides service in 87 MO counties. Last year, the OATS fl eet of more than 800 vehicles traveled over 14 million miles providing 1.5 million one-way trips. 14
