SUMMARY OF BENEFITS January 1, 2018 December 31, 2018 INTRODUCTION TO SUMMARY OF BENEFITS This is a summary of health services covered by Advantage (HMO-POS SNP) for 2018. There are different types of Medicare health plans. Advantage is a Dual Eligible Special Needs Plan (D-SNP), which is a Health Maintenance Organization (HMO) for people with Medicare and Medicaid. This HMO plan offers Point of Service (POS) access to some out-of-network services. Advantage is approved by Medicare and Medicaid and run by a private company. This booklet is only a summary. It does not list every service that we cover or every limitation or exclusion. Please read the Evidence of Coverage (Member Handbook) for the full list of benefits. The Evidence of Coverage (Member Handbook) is provided upon enrollment and annually by December 31 st. You can also view it on our website at www.villagecaremax.org or call Member Services for a copy. You have choices about how to get your Medicare and Medicaid Benefits One choice is to get your Medicare benefits through Original Medicare (fee-for-service Medicare) or your Medicaid benefits through Medicaid fee-for-service. Things to Know About Advantage (HMO-POS SNP) Which doctors, hospitals, and pharmacies can I use VillageCareMAX has a network of doctors, hospitals, pharmacies, and other providers. As a member, you will be able to get covered services from certain out-ofnetwork providers without prior authorization : Doctor Office Visits (Primary Care Provider and Specialists). Certain procedures may require authorization. Please refer to Chapter 4 of the Evidence of Coverage (Member Handbook) for details Outpatient Rehabilitation (Physical Therapy, Occupational Therapy, and Speech Language Therapy). Prior authorization is required after the first 20 visits Chiropractor Services. Prior authorization is required after the first 20 visits Podiatry Services Otherwise, if you use the providers that are not in our network for other services, the plan may not pay for these services. Things to Know About Advantage (HMO-POS SNP) What do we cover Like all Medicare health plans, we cover everything that Original Medicare covers - and more. Members of our plan get all of the benefits covered by Original Medicare. Members of our plan also get more than what is covered by Original Medicare. Some of the extra benefits are outlined in this booklet. Members of our plan get most benefits covered by Medicaid Fee-for-Service We cover Part D drugs and also Part B drugs such as chemotherapy and some drugs administered by your provider.» You can see the complete plan formulary (list of Part D prescription drugs) and any restrictions on our website at www.villagecaremax.org. Monthly Premium, Deductible, and Limits on How Much You Pay for Covered Services How much is the monthly premium $0 plan premium per month. You must continue to pay your Part B premium.
