Why is Patient Safety and Pressure Ulcer Prevention important?

Don Berwick (2014) A promise to learn - a commitment to act: Improving the safety of patients in England 10 recommendations: The NHS should continually and forever reduce patient harm by embracing wholeheartedly an ethic of learning All leaders concerned with healthcare political, regulatory, governance, executive, clinical and advocacy should place quality of care in general, and patient safety in particular, at the top of their priorities for investment, inquiry, improvement, regular reporting, encouragement and support

The impact of pressure ulcers on patients, families, carers, staff and the organisation is huge. The lifestyle of a person who has suffered a pressure ulcer is changed forever, their ability to lead a pain free, risk free life in the future affected.

What has the journey been like?

How it started back in November 2011 The Stop the Pressure campaign was launched by NHS Midlands and East as part of their ambition to make life better for patients. Ambition 1: To eliminate grade 2,3 and 4 pressure ulcers

NHS Safety Thermometer CQUIN for 2 years An improvement tool, which measures the presence or absence of 4 harms: Pressure Ulcers Urine infections in patients with a catheter Falls New venous thromboembolisms (VTE)

What does the ST data show us? All categories ST prevalence data: April 2012 7.1% Dec 2013 4.7% July 2014 4.5% Dec 2014 4.3% The Incidence of new PUs: April 2012 1.7% Dec 2013 1.0% July 2014 1.0% Dec 2014 1.0%

What is the data showing us? 700,000 patients are affected by PUs 186,617 in hospital settings 46% in newly acquired PUs (ST data)across Country 50% in newly acquired PUs (ST data) M&E 80-95% avoidable Between Apr Sept 2013 5.2% PU reported NRLS Between Apr Sept 2014 4.6% PU reported NRLS..But we are seeing a plateau and variation Grade 4 14% severe harm category SI
