Services Services - Isolating Data for Multi-Site Corporations A Corporate / Region / Area / Office Selector option has been added to the Services Area Selections made will cause the Services Entry (Physicians, Facilities, Funeral Homes, etc.) to associate with one or multiple Offices, Areas, Regions or an entire Corporation. Details regarding the Services enhancement: o This enhancement will be extremely helpful in scenarios where a Consolo Website is used by a multiple site hospice where some Physicians, Facilities, etc. are shared and some are not between Offices. o Prior to the Release if a Services Resource was not associated to an Office, it will continue to be associated to the entire Corporation. o Consolo will no longer display all Resources in the dropdowns (such as Physicians) unless the Physician is assigned to the Corporation or a Hospice only has one Office. o The system will consider a Users and a Patients association to the Corporation when displaying Services in dropdowns. o Multi-Offices should review Users and Services Organizational Assignments to verify access will not be impacted Please Contact Support@Consoloservices.com with questions. ZIP Codes 9 Digit for the Transition to the 5010 Claims Format Due to the 5010 Claims Requirement, in Release 3.0.9 (in approximately November), Consolo will require 9 digit ZIP Codes so that claims may be sent in the mandated format. In order to prepare for the 5010 Claims Format, 9 Digit ZIP Codes are required and should be updated in the following areas: Patients Home Address Facility Address Office Address Note: Please work on updating Patient, Facility and Office Addresses to 9 Digit ZIPCODES. Dashboards Custom Patient Alert Dashboard New A New Custom Alert Dashboard was added to easily see Custom Patient Alerts for all Patients listing on the Classic dashboard. 6 Page

Classic Dashboard New Medical Record Number Alert An Alert was added to display if the Medical Record Number has an Effective Date after the Admission Date. This issue is currently a claim error, but this would allow the issue to be addressed prior to creating claims. Tracker Dashboard Multi-Office Filter A Multi-Office filter added to permit the Tracker Dashboard to display information pertinent to individual offices. 7 Page

Patient Home Page Status Bar Enhanced The Status Bar at the top of the Home Page has been improved to include: Level of Care ICD9 / Diagnosis Click the More/Less Button to display more or less patient details (such as the Certification Beginning and End Dates) Assessments Links were added to the Wound and Pain Assessments from within the Nursing Assessment. Certifications Certifications include Physician Orders from the previous 14 days if a patient is active or a new admission, as they have in the past. Certifications will not include Physician Orders from previous 14 days if patient is a readmission. Change in Care - New Discharge Code A New Change in Care Status was added Discharge Due to CMS Guideline for addressing Face to Face Discharges and Discharges Required for Hospices that are recently Certified by Medicare (Medicare requires patients have an admission date that is not prior to the Hospice s Medicare Certification Date). Below are details regarding Face to Face Visits and Discharging Patients if the F2F is not completed prior to the recertification date: CMS requires a Hospice Physician or Nurse Practitioner have a Face-to-Face encounter with each hospice patient prior to the third recertification period and for every subsequent period, unless exceptional conditions exist. Failure to meet the timeframes results in a failure by the hospice to meet the recertification of terminal illness eligibility requirement. At that point, the patient would cease to be eligible for the Medicare Hospice Benefit. The hospice may continue to serve the patient under hospice care, but in doing so assumes all financial responsibility for that care and must be discharged. The patient can be re-admitted when the eligibility criteria are met. Note: Claims will be coded the same as Discharge-Revocation, which is appropriate for Patients needing to be discharged when a hospice is recently Certified and for Face to Face related discharges. 8 Page

Clinical Indicators and Diagnosis - Electronic Signature Functionality was added to Electronically Sign the Clinical Indicators and Diagnosis Screen. Face Sheet Discharge information Date of Death/Date of DC live/date of Revocation has been added to the Face Sheet. Fax Services - Face to Face Visits Capability added to efax Face to Face Visits. Hospice Aide Assignment Modifications in the Hospice Aide Assignment Screen relating to the ability to schedule in the Hospice Assignment Screen: o Days and Hours Fields will display if present but will no longer be available in new documents o Schedule Changes Field was renamed Comments IDG - Classic Version New Hide/Show Column Functionality 9 Page

IDG Wizard Snapshot Enhancements (Pre)Bereavement Care Plans now display in the IDG Snapshot. Spiritual Assessments now display in the IDG Snapshot. POC Notification A New Field was added to the IDG discussion page that states "Primary Physician notified of updated POC." Medications Adverse Effects - Enhancement Adverse Effects details are available in the Medication Record / Adverse Effects Link: 10 Page

Mini Assessments New Body Site Graphic Image A Body Site Graphic was added the Pain and Wound Assessments. Pain Assessment Acceptable Level of Pain An additional Pain Scale was added to the Pain Assessment Verbal Patient Acceptable level of Pain with a Pain Scale of 1 10 which is also included in the QAPI Pain Level Tracking Report. Payer Validate Private Insurance The presence of private insurance information is now validated when private insurance is the payer. 11 Page

Physician Orders - DEA Number Physician DEA numbers now appear on Physician Orders. Spiritual Care Assessment - Reference to Clergy The fields were modified that reference an outside spiritual leader from Chaplain to Clergy. Reports General Report Enhancements The following enhancements were added to various reports and will eventually appear in on all reports: Multi-Office Selector Provides the ability to select Multiple Offices, Areas or Regions Column Hide / Show Enhancement Improved Column Display functionality Timestamps - Time now displays on PDF documents Length of Stay - New A new Length of Stay Report was developed to refine the tracking of the number of Days the Patient Receives Care, the report does the following: Defaults to Discharged Patients Active Patients may be Included Report may be run for Actives Patients Only Subtotaling Breakdown by Office / Region / Area (Grand Total by Corporation) ICD9 Code Average LOS Median LOS 12 Page

Face To Face Report - Title Change The Sub-header Title was modified in F2F Summary Report from "Current Certifications with Face To Face Visits Needed" to "Current Certifications with Face To Face Visits Needed (Past Due)" for clarification purposes. Account Receivable Report - Enhancements The Accounts Receivable report was enhanced in the following ways: o Sent Status Filter Added o Sent Column Added Defining when a Claim was Electronically Sent o Response Message Column Added Defining Reasons Claims were not Sent o Add column and filter to show sent/unsent Payments Report Additional Column A column entitled Balance was added to display the claim s remaining balance on the Payments Posted Report. 13 Page

Hospice Owned Facilities and Inpatient Tracking Hospice Owned Inpatient Unit / Hospice Care Documentation - New A New Area was added to the Patient Home Page designed for Inpatient Care or patients being cared for in a facility owned/staffed by the Hospice and allows efficient tracking of an entire shift. The Inpatient Unit Note does the following: Clinical documents (with Time) are displayed from the Patient Home Page Allows Time Entry with Associated Tasks within the screen itself Details may be viewed by Day Time May be Viewed by Role / Discipline Functions for GIP at any Facility Functions for all Levels of Care in a Hospice Owned Facility (the Facility may be designated as owned in the Services / Facility Screen Company Owned Field) Area In the Services / Facility Screen where a Company Owned Facility may be defined: 14 Page

Consolo Tips for Best Functionality Offline Charting Synching Synch all Off-Line Charting Prior to Software Releases to eliminate errors trying to synch clinical charting after the Consolo System has been updated. Copying and Pasting When Copying and Pasting text use caution as sometimes when creating PDFs symbols will display where there was previously formatting. Cache Clear the PC Cache if the system is not displaying screens or performing properly (complete instructions may be found in Training Tools / Reference Documents). Find Function Use Control / F to Find data on any Web Page. Table Filtering Refine data displaying in a Table by Typing it in the Search box in the upper left area of the table. Search / Help in Consolo (in the upper right corner of the system): o o Search - Type a Patient Name and Click the Magnifying Glass to quickly Search for a Patient By Name. Help - Click the? to find Help relating to the Consolo Screen that is open. 18 Page
