Principal Site Conference During the site administrator conference this school year, School Nurses are being asked to discuss and provide a copy of the Principal letter regarding the CCSD Revised Health Office Field Trip Procedures. (HS) This is now listed as a requirement of the HS 141 and is listed in the new HS 141A (optional) Review the content of the HS-114 Principal Letter with the administrator to ensure you have addressed specific building concerns.
Key Points to Review with Site Admin Notification of Health Office one month prior to travel: The increased time frame will increase safety, and allow the School Nurse to review the student list for medical concerns, provide teacher training and obtain updated LHCP provider orders if needed. Collaboration with School Nurse and Nursing Supervisor if there is a need for a licensed nurse to attend the field trip. Teacher will pick up a packet from the Health Office. Packet includes checklist of required items needed with timelines of when items need to be turned into the health office. A copy of the teachers checklist should be reviewed with Site administrator. Failure to meet the timelines may impede/prevent the student attendance on the Field Trip.
Field Trip Checklist for Teacher/Sponsor The procedures noted below must be followed, and designated items must be received by the Health Office. Failure to meet these timelines may impede/prevent student attendance on Field Trip. Pick-up Teacher/Sponsor packet from Health Office at least one month prior to trip. Provide list of projected students to the School Nurse for review at least one month prior to trip. Send home HS-116 (Supplement to CCF 455) to all students attending the field trip. HS-116 is only required if attendance is outside the normal school day. If CCF 455 Medical Permission Form is required by the site administrator, provide copies to the School Nurse at least three weeks prior to the trip. Provide copies of the returned Health Office Field Trip Parent Letter to the School Nurse at least three weeks prior to the trip. (If attendance will be outside the normal school day hours) Teacher to receive Field Trip Medication/Procedure training by the School Nurse at least one week prior to the trip. Pick-up medications and portable First Aid Kit from Health Office. (Day of trip)
School Nurse Field Trip Procedures Health Office is notified of Field Trip from Teacher/Sponsor or designee at least one month prior to activity. Field Trip Packet Provided to Teacher/Sponsor from the Health Office. Packet includes the following: HS-116 Supplement to CCF 455 HS-115 Health Office Field Trip Checklist for Teacher/Sponsor School Nurse to review list of projected students attending trip for health conditions, medications or procedures. School Nurse to review returned HS-116 Supplement to CCF 455. (Only needed for students attending field trips outside of normal school hours.) School Nurse to review CCF 455 (if form is required by site administrator) for any medical concerns not previously noted. If new health problem exists, follow up with Health Services policy for setting up special problem/careplan.
School Nurse Field Trip Procedures School Nurse to determine if any medical procedures require a licensed nurse to perform. The School nurse will discuss with Health Services Administration the need for a nurse to attend the field trip. School Nurse to consider the following: Procedure can be legally and/or safely delegated to a UAP. School Nurse to train a UAP (Unlicensed Assistive Personnel) and delegate in accordance with the School Nurse Regulation and Advisory Opinion (Nevada State Board of Nursing). Complete Verification of Field Trip Medication Training (HS 207). Procedure cannot be legally and/or safely delegated to a UAP. Determine if a parent/guardian is willing to attend. If parent/guardian cannot attend, and procedure cannot be safely delegated, Health Services Administration will assign a licensed nurse to provide nursing services on the field trip in accordance with the School Nurse Regulation and Advisory Opinion.
School Nurse Field Trip Procedures School Nurse to obtain LHCP orders for any medication or specialized procedure in accordance with Health Services Procedure. May need additional orders or clarification for procedures that extend beyond the school day. (i.e. diabetic orders) School Nurse to provide Field Trip medication/procedure training to teacher/sponsor. Health Office to provide Medical alerts or other information as necessary. Health office staff to document all medications/procedures administered as outlined in the Health Services Policy/HMS Field Trip Procedure.
HS-116 Supplement to CCF 455 (Previously titled Parent Letter) Distribution of the CCF 455 is an individual site based administration decision HS-116 Supplement to CCF 455 is required for all field trips extending beyond regular school hours School Nurse may print both CCF 455 and HS- 116 double sided.
HMS Field Trip Documentation The nurse will utilize the HS-117 Temporary Field Trip Medication Worksheet for: Medications/Special Needs prescribed outside the normal school day and not maintained at school. PRN medication needed during the field trip but not maintained at school. The nurse will follow the current procedure of printing the HMS 502 or HMS 506 for Medications/Special Needs already set up in HMS and administered during the school day.
Medication/Special Need Not Administered on the Field Trip If Medication/Special Need is not administered: No further documentation is required. Maintain copy of the Temporary Field Trip Medication worksheet, and other field trip documents as per Health Services policy. (3 years) Field Trip Notebook is Optional
Medication/Special Need Administered on a Field Trip If Medication/Special Need is administered. Enter this information in HMS as outlined below: The School Nurse will utilize the information on the Temporary Field Trip Medication Worksheet to document in HMS. Set up a new Prescription or Special Need in HMS if the student does not receive it during the regular school hours. Change the Examiner Name to the staff member that provided the medication or Special Need to the student on the Field Trip. Enter the accurate date and time the med or procedure was given. Check the Outside box in the upper right hand corner of the administration screen
Field Trip Medication and Special Need Administration Set up the Prescription or Special Need if the student does not receive it during the regular school hours Change the Examiner Name to the employee who provided the med or special need Enter the accurate date and time given Check the Outside Box
Medication/Special Needs Already Set up in HMS Copy Report #502/#506 for each student on field trip and highlight the five rights. Send medication/equipment with #502/#506 for each student. Remind teacher to sign and write time med was given directly on #502/#506 and return #502/#506 with medication/equipment to the health office. Check the Outside Box as the administration is outside of normal school hours
