Welcome to My Wellness at Work The Hartford s My Wellness at Work program makes it easy for you to create and maintain a healthy lifestyle. These programs are designed to empower you to make smarter decisions about your health and to connect you with the information and resources you need to maximize your health and quality of life. To learn more and get started, register on the My Wellness at Work portal (www.mywellnessmywork.com). 4

YOUR GOAL: EARN A TOTAL OF 40 POINTS BETWEEN OCTOBER 1, 2017 AND SEPTEMBER 30, 2018 (10 EACH QUARTER) When you participate in wellness activities, you will earn wellness points. If you are enrolled in one of The Hartford s health plan and earn at least ten new points during a calendar quarter, you will receive a $150 healthy lifestyle credit toward your health premiums the following calendar quarter throughout 2018, for a total premium credit opportunity of $600. "Unused" wellness points carryover from one quarter to the next. For example, if you earn 14 points one quarter, 10 will be used toward premium credit the next quarter and 4 will carryover. To receive premium credits, you must complete the well-being assessment which is available on the My Wellness at Work portal (www.mywellnessmywork.com). This is the only required activity; you select which other activities you want to do to build your pathway to wellness. HERE S HOW IT WORKS Each activity shows the number of wellness points that you can earn. Upon completion of the activity, your wellness points are awarded. You can track your progress on the My Wellness at Work health portal. You can log your activity online using your computer or personal smartphone or tablet on the mobile-friendly portal. Use the Program Point Goal to help you stay motivated and engaged to create a healthier lifestyle. You select which activities you want to do. When you reach your goal, keep going and continue to build your pathway to wellness. Welcome Get Started to My by Completing Wellness at Work Your Well-being Assessment! Because we care about your health and well-being, we are offering an assessment to help you (and your dependents if they are covered by The Hartford s health plan) learn about the factors that affect your overall health and well-being. The well-being assessment (WBA) which is available beginning in January 2018 on the My Wellness at Work portal is administered by Optum, a leader in the delivery of healthcare management programs. The results of your WBA will help you learn more about ways you can improve your physical, emotional, social, spiritual and financial well-being. Your results will also allow Optum to tailor your activities on the portal to your individual needs. 8

COMPLETE YOUR WBA Beginning in January, 2018 log on and register on the My Wellness at Work portal (www.mywellnessmywork.com). You will see the WBA module on the home page. The WBA is confidential; your answers will not be shared with The Hartford. Please respond to the questions as candidly as possible to ensure your results accurately reflect your well-being. It should only take 15 to 20 minutes and, once you re done, you will immediately see your results. Please note: Completing your WBA is a required activity to earn premium credits. Even if you earn ten wellness points by participating in other activities, you will not receive premium credits after the first quarter of 2018 until you complete the WBA. The WBA will be available on the My Wellness at Work portal until you complete it. You will receive a premium credit the quarter after you complete the WBA. Retroactive credits will not be provided. Remember, to receive any credits toward your 2018 medical premiums after the first quarter, you will need to complete the WBA. 9

THIS YEAR up to 15 wellness points can be earned based on your health screening results. If you participate in the company-sponsored health screening program and have your screening by the Nov. 3, 2017 deadline*, you can earn three wellness points for each of the five measures below that fall within the healthy range (up to a total of 15 additional wellness points). THE HOMEPAGE IS DESIGNED JUST FOR YOU BASED ON YOUR HEALTH AND PREFERENCES. Learn your health risks when you complete the Well-being Assessment (WBA). Features on the portal will be personalized to you based on your WBA responses. Select programs that will help motivate you to get healthier. You choose how, when and where to make changes. Participate and record activities online using your computer, personal smartphone or tablet on the mobilefriendly My Wellness at Work portal. Track your participation and progress during the program. No goal is too small or too big. IMPROVE YOUR HEALTH WITH A VIRTUAL COACHING PROGRAM Reach your health goals, big or small, working at your own pace with a virtual coach online! Choose your focus area like improving your diet, managing stress, getting more physically active, and more. Set your goal and get started with healthy steps you can take to reach it. You ll find all sorts of fun tools such as healthy tips, meal planners, recipes, and quizzes. Log in to the My Wellness at Work portal and complete at least one healthy action each week in six different weeks and a check-in assessment to reach a milestone. The tracker on your home page helps you manage your progress. You will get four wellness points when you finish a milestone, and you can complete up to three for a total of 12 points. The action plan helps you transition to and from phone based coaching programs by maintaining a consistent action plan and completed progress. 19

If the results of your WBA indicate you would benefit from an additional level of support, the Weight Talk program is built just for you. A dedicated coach will work with you one-on-one to guide you every step of the way. You ll learn how small changes in daily food choices and eating habits can make a big difference when it comes to dropping extra pounds. Throughout the six-month program, our coaches will help you lose weight and keep it off. You will schedule regular calls with your coach. Together you will set realistic goals and make a plan to reach them. You ll also have access to an online program and workbook. The Weight Talk program includes the following strategies for longterm weight loss: Personalized eating plans Weight, food and exercise trackers Tips for a healthier environment Quick wins for addressing barriers Ideas for managing stress With Weight Talk, you ll never be alone. Your coach will be there whenever you need support. You will also earn 10 wellness points when you complete the coach check-in calls along with a status assessment. The Quit For Life Program Take the first step toward quitting today. The Quit For Life Program is the nation s leading tobacco cessation program. It employs an evidencebased combination of physical, psychological and behavioral strategies to help you overcome your addiction to tobacco. Together you and your coach will create an easy-to-follow Quitting Plan to help you get ready, take action and live the rest of your life tobacco free. As an alternative to the Quit For Life Program, all employees and covered dependents can enroll in the Live Tobacco Free online health coaching program. It provides online support and motivation to quit smoking through www.mywellnessmywork.com. It is confidential and can be accessed anytime 24/7. To get started: 1. Log on and select the Coaching link then choose the Live Tobacco Free focus area and get started. 2. Work through your To-Do list each week. 3. Complete the final assessment to receive 4 wellness points (up to a max of 12 for online coaching programs). 22

EMPLOYEES AND THEIR COVERED DEPENDENTS ENROLLED IN THE HARTFORD S HEALTH PLAN HAVE ACCESS TO THE QUIT FOR LIFE. WHEN ENROLLED, YOU WILL BENEFIT FROM THESE PROGRAM FEATURES: Quit Coach Staff Coaches will support you over the phone whenever you need it. They are experienced in helping to overcome the struggles people face to beat tobacco for good. You can call your Quit Coach staff member as much as you want for 12 months. Quit Guide. You will receive an easy-to-use printed workbook you can reference in any situation to help you stick with your Quitting Plan. Web Coach Website. Enjoy lifetime access to a private online community where you can complete activities, watch videos, track your progress and join in discussions with other program participants. 10 Wellness Points. Earn credit when you complete at least five designated calls with a Quit Coach staff member and a six-month survey. Quitting Aids. Your coach will help you determine which type, dose and duration of nicotine substitute or medication is right for you, and teach you how to use it so it really works. Plus, you can get up to 8 weeks of gum or patch therapy sent directly to your home. Text2Quit SM. In addition to calls with your Quit Coach staff member, you will receive supportive text messages on your mobile phone to help you prepare to quit, use medications correctly, manage urges and avoid relapse. You can also selfenter other activity in your online Activity Journal on the site. Earn Virgin Pulse points. As you upload your steps and your activity accumulates, you ll earn Virgin Pulse points. The more active you are, the more Virgin Pulse points you will earn. You can also earn Virgin Pulse points by participating in individual, team and company-wide challenges. Track your progress. FITNESS REIMBURSEMENT PROGRAM The Fitness Reimbursement Program allows employees of The Hartford to receive reimbursement of up to $90 per quarter towards the cost of certain fitness-related activities including a membership in a fitness centers, health clubs or aquatic centers, as well as session fees for such activities as group/personal training, Pilates, yoga instruction, etc. when offered by a certified instructor. HOW IT WORKS: To receive fitness center membership reimbursement, you must follow all of these steps: If you are not already participating in a fitness activity, get started. To help you identify and understand your personal health risks, The Hartford is offering an annual online well-being assessment (WBA) through Optum a leader in the delivery of healthcare management programs. The WBA is a series of health and lifestyle related questions that will take approximately 15 minutes to answer. The results of your WBA will help you learn more about your physical and emotional health as well as actions you can take to improve your overall well-being. Your results will also allow Optum to tailor your activities on the My Wellness at Work portal to your individual needs. WHY SHOULD I PARTICIPATE? Here are five good reasons to complete the My Wellness at Work well-being assessment (WBA): 1. It s quick. It should only take about 15 minutes to complete the online questionnaire. 2. It delivers immediate results. Within seconds of completing the questionnaire, you ll have your results. 3. It provides personalized information. Your WBA results will give you information about your current health status and identify your risk factors. These risk factors often have no symptoms. You can proactively identify risk factors which can lead to chronic illness. Obtaining this information will show you the areas where you can make a lifestyle change or seek treatment to avoid serious illness. 4. It is your gateway to new wellness programs. By completing the My Wellness at Work WBA you will be eligible to participate in no-cost lifestyle and disease management programs. 5. Last, but not least, you ll save money! HOW DO I ACCESS THE WBA? The WBA is available beginning in January, 2018 on the My Wellness at Work portal, which you can access from iconnect or directly at www.mywellnessmywork.com. If you previously registered on the site, you will use the same login ID and password you created when you registered. If you do not remember the login credentials, use the links in the Sign In box on the homepage. Once you are logged in, you will see the WBA in the center of the page. If you have not yet registered on the site, follow the steps to do so. Once you are registered, you can log in to access the WBA. WHAT IF I CHOOSE NOT TO TAKE THE WBA? Completing the WBA is voluntary; however, taking the WBA is required to earn any premium credits after the first quarter of 2018. Even if you earn ten wellness points by participating in other activities, you will not receive any premium credits until you complete the WBA. The WBA will be available on the My Wellness at Work portal until you complete it. You will receive a premium credit the quarter after you complete the WBA. Retroactive premium credits will not be provided. WILL MY PERSONAL HEALTH INFORMATION IN THE WBA BE SHARED WITH THE HARTFORD? No personal health information is ever shared with The Hartford. The Hartford may receive aggregate de-identified data in the form of reports. Using these reports, The Hartford can develop health and insurance initiatives that are most helpful to its employees. HOW IS MY WBA DATA USED AND PROTECTED? Your WBA responses are used to provide you with a customized feedback report based on your specific health risks and needs. This data may be used to identify optional health and wellness programs relevant to your health needs. Certain information you provide becomes protected health information under the Health Insurance Portability and Accountability Act (HIPAA) and may only be used or disclosed without your authorization for purposes of treatment, payment or health care operations. For example, a use or disclosure for purposes of health care operations would include use of the information by a disease management vendor, to provide services to employees of The Hartford. WHY AM I NOT ABLE TO FILL IN MY HEALTH RESULTS ON THE WBA? If you participate in The Hartford s health screening program during the fall, your results will automatically be populated in your WBA. If you chose not to participate in the company-sponsored program, no results will appear and you will not be able to enter any values. To ensure the integrity of the information, self-reported screening results will not be accepted. However, even without biometric values, you can still complete the WBA, receive your results (though they will not be as comprehensive), participate in the wellness programs, and earn a premium credit. 41
