Page 7 Arizona Coyotes Foundation Application Guidelines Available: June 1, 2016 Close Date: July 29, 2016 at 5:00PM Introduction The Arizona Coyotes Foundation mission is to enhance the quality of life throughout Arizona communities by supporting non-profit organizations that promote healthcare, education, cultural arts and sports-related programs for children and service men and women. Over the past 20 years, the Coyotes have proudly raised millions of dollars that have helped hundreds of organizations in the State of Arizona. Mission Statement The Arizona Coyotes Foundation seeks to enhance the quality of life throughout Arizona communities by supporting non-profit organizations that promote healthcare, education, cultural arts and sport-related programs for children and service men and women. Arizona Coyotes Foundation will accept proposals that support the following primary interest areas: Healthcare: Organizations that focus on the health and well-being of youth or service men and women through enhancing community awareness, disseminating information, providing support, and treatment of preventative care. Education: Organizations that promote quality education for all children or service men and women by enriching their lives through in-school or after-school programs designed to better equip them for the future. Cultural Arts: Organizations that seek to enhance the quality of children s or service men and women s lives by providing programs designed to provide opportunities for hand-on creativity, development of their artistic awareness and potential, and sharing in vital cultural currents of a community. Youth Sports: Organizations that encourage physical activities, exercise, teamwork, confidence, goal setting, and a healthy lifestyle through community outreach, organized sports, or mentorship programs.
Page 2 Eligibility Requirements Applicant must be an organization classified as a public charity and exempt from federal taxes under 501(c)(3) of the Internal Revenue Code Organization must be an established 501(c)(3) for at least three years Applicant s primary mission must focus on improving the lives of children and/or service men and women in the State of Arizona and be aligned with the Arizona Coyotes Foundation Mission Statement Organization may submit only one grant application All proposals must: o Clearly define specific objectives and community need that will be addressed o Describe how a partnership with the Arizona Coyotes Foundation will be developed o Establish a realistic timetable and budget o Outline a process to measure the effectiveness of the proposal Grants will not be awarded to applications seeking funds that advance a particular political, social or religious positions, or directly support individual sports teams or leagues, schools, or their associated organizations (i.e. PTA s, Booster Clubs, etc.) Organization must be prepared to organize an on-site visit by the Arizona Coyotes Foundation at the location where the work of services will be provided, as well as at existing locations operated by the organization. Organizations must demonstrate a non-discrimination policy regarding staff, employment, governing board and service delivery based on race, ethnicity, religion, gender, sexual orientation, gender identity, age, disability or national origin, and provide evidence of the policy. Organizations are required to have a completed and visible profile on ACF s website: www.azfoundation.org. Organizations without 501(c)(3) status must use a fiscal agent to apply on their behalf. Fiscal agents must have a visible profile on the ACF website, and also be a 501(c)(3) organization. Organizations uncertain of their eligibility may inquire by sending an e-mail to grants@azfoundation.org. Available Funds The source of funds for the grants to be awarded this Season are the donations and fundraising events held during the 2015-2016 Hockey Season. All grant applications need to be consistent with the Arizona Coyotes Foundation Mission Statement. Funding Range The minimum grant award from the Arizona Coyotes Foundation is $5,000. The maximum grant amount from the Arizona Coyotes Foundation is $50,000.
Page 3 Timeline for Grant Making Grant cycle opens Wednesday, June 1, 2016 at 9:00am Applications due Friday, July 29, 2016 at 5:00pm Grant Panels will review applications in August and September 2016 Grant awards and declines announce in November 2016 Final Reports If funded, Final Report is due one year after check is issued. Final reports are expected to include grant impact story and photographs. How to Apply Online If your organization has an ACF profile: Go to the Arizona Community Foundation's grants page on their website at www.azfoundation.org If your organization does not have a registered ACF profile: If your organization needs to be registered, please go to www.azfoundation.org and select Grantee Login to sign up. Once you are registered, you may begin the LOI or application process by starting a new application. You may also edit organizational information once you have logged in and attached your organization to your profile. Tips for Submitting All users will be required to update and complete their organizational profile before being able to submit any grant application. During the last week of any grant cycle s duration, due to the volume of inquiries, you will likely have limited access to ACF technical support so working in advance is strongly advised. It is recommended that the application be prepared offline in a text editor such as Microsoft Word to ensure a duplicate exists. There are new fields in the organizational profile to complete. Be sure to update and complete the profile before submitting your proposal. Narrative responses are limited to the maximum word count. Only provide information that is pertinent. Be sure to review applications for spelling and grammar errors. Be sure to check for an email notification indicating that your application has been submitted.
